Summary: Rare immunophenotypically immature B-cell precursor acute lymphoblastic leukemia (BCP-ALL) with immunoglobulin-MYC rearrangement (IG-MYC-r) can be confused with Burkitt lymphoma/leukemia and treated with unproven efficacy. A comparison of molecular characteristics and clinical outcome data of 90 registered cases with IG-MYC-r in a national BCP-ALL clinical trial/registry demonstrated diverse cytogenetic backgrounds and high-risk BCP-ALL features. Effective management strategies, such as contemporary clinical trial protocols, should be developed for these patients.

Incidence and Risk Factors of Venous Thromboembolism in Childhood Acute Lymphoblastic Leukaemia - a Population-Based Analysis of the Austrian Berlin-Frankfurt-Munster (BFM) Study Group

Summary: Venous thromboembolism (VTE) is a common and serious complication in the treatment of pediatric acute lymphoblastic leukemia (ALL). This study analyzed 1026 ALL patients aged 1-18 years and identified the incidence and risk factors of VTE. The results showed that VTE was most commonly observed during induction and early consolidation therapy, and older age was the main patient-related risk factor.

Summary: This study aimed to investigate the prognostic factors and efficacy of allogeneic hematopoietic stem-cell transplantation (allo-HSCT) in the first remission of patients with noninfant childhood acute lymphoblastic leukemia (ALL) with 11q23/KMT2A rearrangements. The results showed that the prognosis of patients with noninfant 11q23/KMT2A-rearranged ALL has improved compared to historical data, but allo-HSCT did not improve outcomes. Targeted therapies are needed to reduce relapse and treatment-related mortality rates.

Summary: This article reports a male patient with SCN4 caused by G6PC3 gene deficiency, manifested as recurrent bacterial infections and multi-systemic complications, and showed rare large platelets on peripheral blood smear. The authors recommend considering G6PC3 gene mutation in cases of congenital, unexplained neutropenia.

Summary: In this study, we assessed the usage of antimicrobials in pediatric hematology and oncology centers in Germany and Austria. We found a high prevalence of inappropriate antimicrobial use, with incorrect dosing and (de-)escalation/spectrum-related errors being the most common reasons. Furthermore, we identified febrile neutropenia diagnosis and antimicrobial stewardship programs as factors associated with a lower likelihood of inappropriate therapy.

Summary: Increasing evidence suggests multilineage cytopenias may be caused by inborn errors of immunity, such as SASH3 deficiency. We studied a patient with autoimmune haemolytic anaemia and immune thrombocytopenia and found a germline mutation in the SASH3 gene. The patient's younger brother also carries the mutation but is currently asymptomatic.

Low-dose Bisphenol A exposure alters the functionality and cellular environment in a human cardiomyocyte model

Summary: This study investigated the effects of environmentally relevant doses of Bisphenol A (BPA) on developing cardiomyocytes using a human induced pluripotent stem cell (hiPSC)-derived model. The results showed that long-term treatment with low doses of BPA interferes with hiPSC-CMs functionality and alters the surrounding cellular environment, leading to potential diseases. This study contributes to the understanding of BPA effects on developing human foetal cardiomyocytes and provides a suitable model for environmental chemical hazard and risk assessment.

Summary: This study reports the first patient with functionally confirmed deficiency in the PMVK gene, leading to an autoinflammatory disease. A homozygous missense variant in PMVK was identified and its pathogenicity was supported by genetic algorithms and modeling analysis. The patient exhibited clinical similarities to other patients with MVK deficiency but also showed distinct features. The study highlights the importance of considering PMVK deficiency in the diagnosis and genetic testing for systemic autoinflammatory diseases.

Summary: This study aimed to examine the thymopoiesis in healthy children by measuring recent thymic emigrants (RTE) and establish the RTE reference values in Turkish children. The results showed that the absolute count and relative ratios of RTE cells were highest during the first year of life, decreasing significantly with age. These findings may contribute to early diagnosis and monitoring of immune reconstitution.
