Summary: This study investigated the added value of amyloid-PET measures in predicting future cognitive changes over and above sociodemographics and past cognitive measures. The results suggest that when repeated cognitive assessments are available, the additional utility of amyloid-PET in predicting future cognitive impairment may be limited.

Positron emission tomography is an effective imaging technique that allows for comprehensive observation and evaluation of Alzheimer's disease from diverse perspectives.

A Multi-Criteria Decision Aid Tool for Radiopharmaceutical Selection in Tau PET Imaging

Summary: The use of positron emission tomography (PET) imaging with tau-specific radiotracers allows for the detection and quantification of tau pathology in living subjects, which aids in the early diagnosis of Alzheimer's disease (AD) and monitoring disease progression and treatment effectiveness. This study analyzed, compared, and ranked currently available tau PET radiotracers using a multi-criteria decision-making (MCDM) tool called PROMETHEE. Based on weighted criteria such as specificity, target binding affinity, brain uptake, brain penetration, and rates of adverse reactions, the second-generation tau tracer [F-18]RO-948 was found to be the most favorable. This flexible method can be expanded and updated to assist researchers and clinicians in choosing the optimal tau PET tracer for specific purposes.

Current and Future PET Imaging for Multiple Myeloma

Summary: Positron emission tomography (PET) is a method used for the evaluation of tumor staging and response to therapy in a noninvasive manner. While PET with F-18 labeled fluorodeoxyglucose (F-18-FDG PET) is commonly used for assessing lesions in multiple myeloma patients, it is less sensitive compared to next-generation flow cytometry and sequencing. Novel PET radiotracers, such as Cu-64-LLP2A, Ga-68-pentixafor, and Zr-89-daratumumab, have been developed to target cell surface antigens of MM cells. Recent studies have focused on visualizing tumor-infiltrating lymphocytes using PET imaging in cancer patients, but these studies have not been performed in MM patients. This review highlights the recent research on PET with F-18-FDG and novel radiotracers for MM detection, including preclinical studies using MM mouse models and clinical investigations. Novel PET technologies may have potential for developing therapeutic strategies for MM in the future.

FDG-PET versus Amyloid-PET Imaging for Diagnosis and Response Evaluation in Alzheimer's Disease: Benefits and Pitfalls

Summary: In June 2021, the FDA accelerated approved aducanumab and in January 2023 also lecanemab, based on perceived drug-induced removal of cerebral amyloid-beta as assessed by amyloid-PET and, in the case of lecanemab, limited clinical efficacy assumption. Approval for donanemab is pending further data. However, published trial data indicate minimal and uncertain clinical benefits, similar to conventional medication. Additionally, amyloid-PET signal decrease may reflect increased cell damage instead of amyloid removal, as supported by increased amyloid-related imaging abnormalities and brain volume loss in treated patients compared to placebo. We also question the current AD diagnostic criteria based on amyloid-PET imaging biomarkers and recommend future anti-AD therapy trials to incorporate (1) diagnosis based on cognitive decline and decreased cerebral metabolism assessed by FDA-approved FDG-PET, (2) therapy efficacy determined by cognitive ability, cerebral metabolism by FDG-PET, and brain volumes by MRI, and (3) neuropathologic examination of all deaths occurring in these trials.

The intrinsic variability of AD protein load over time and technical factors contributing to PET measurement uncertainty are detailed. Suggestions for mitigating these factors, including leveraging shared information between serial scans, are provided.

Despite a relatively small sample size, tau-PET imaging could identify patients with potentially more aggressive clinical courses.

Summary: This study explores the role of endothelial cell S1P(1) in the naive and ischemic brain as a potential target for cerebrovascular therapy. Endothelial S1P(1) plays a critical vascular protective role in the mouse brain, sustaining endothelial functions and expanding during ischemia. Disruption of this pathway exacerbates brain injury in ischemic stroke models, highlighting the importance of S1P signaling in maintaining perfusion and microvascular patency in the ischemic penumbra.

Past, Present, and Future of Non-invasive Brain Stimulation Approaches to Treat Cognitive Impairment in Neurodegenerative Diseases: Time for a Comprehensive Critical Review

Summary: Research has shown that non-invasive brain stimulation (NIBS) may help limit cognitive decline associated with neurodegenerative conditions, but the current clinical evidence is not yet sufficient to demonstrate its therapeutic benefit and clinical significance. Therefore, more double-blind placebo-controlled randomized clinical trials and reliable biomarkers are needed to strengthen the research.
