Relapse of AML after hematopoietic stem cell transplantation: methods of monitoring and preventive strategies. A review from the ALWP of the EBMT

Allogeneic hematopoietic stem cell transplantation (allo-SCT) remains the therapeutic method with the most potent anti-leukemic activity mediated by the graft versus leukemia effect. However, a significant proportion of patients with AML will relapse after allo-SCT. The prognosis for these patients is dismal, with a probability of long-term survival of <20%. Data from previous studies have shown that disease-specific prognostic factors, are in general, the same as those in patients treated with conventional chemotherapy. Minimal residual disease (MRD) and chimerism status monitoring after allo-SCT may be used as predictors of impending relapse and should be part of routine follow-up for AML patients. A significant number of studies have shown that pre-emptive administration of donor lymphocyte infusion (DLI) based on MRD and chimerism monitoring, as well as prophylactic DLI in AML patients at high risk of relapse is effective in preventing relapse. In this review, we discuss strategies for the identification of high-risk patients, review current therapeutic options and provide our recommendations for the management of post-SCT AML.

Summary: Total body irradiation (TBI)-based conditioning regimens are commonly used in acute lymphoblastic leukemia (ALL) patients, but studies have shown comparable outcomes with thiotepa-based conditioning regimen. In this retrospective study, we evaluated the outcome of adult ALL patients who underwent allogeneic hematopoietic stem cell transplantation (allo-HCT) with a thiotepa-busulfan-fludarabine (TBF) myeloablative conditioning regimen. The results showed promising overall survival and acceptable toxicity in these patients.

Summary: This study compared two different myeloablative conditioning regimens, FluTBI12 and FB4, before allo-HCT in AML patients. The results showed no significant differences in leukemia-free survival, overall survival, relapse incidence, and non-relapse mortality between the two groups. The incidence of acute and chronic GVHD also showed no statistical differences. In conclusion, FluTBI12 and FB4 are comparable myeloablative regimens for AML patients transplanted in CR1 and CR2.

Summary: This registry-based study compared the outcomes of allo-HCT for adult ALL patients using TBI combined with either Cy or Flu as conditioning. TBI/Cy was associated with a reduced risk of relapse and increased risk of acute GVHD compared to TBI/Flu. Matched-pair analysis showed that TBI/Cy had a significantly lower relapse rate without significant effects on non-relapse mortality, GVHD, and survival, suggesting it as a preferable regimen for ALL patients in remission.

Summary: For elderly patients with acute lymphoblastic leukemia (ALL) in first complete remission (CR1), autologous hematopoietic cell transplantation (auto-HCT) may be considered as an alternative to reduced intensity conditioning allogeneic HCT (RIC-allo-HCT). A retrospective study compared outcomes of RIC-allo-HCT from matched sibling donors (MSD) or matched unrelated donors (MUD) with auto-HCT for patients aged 55 years or more. While the probabilities of leukemia-free survival (LFS) and overall survival (OS) were not significantly different between RIC-allo-HCT and auto-HCT, the risk of non-relapse mortality (NRM) was higher for both MSD-HCT and MUD-HCT, and for MUD-HCT, the chance of LFS and OS was lower.

Summary: This study evaluated the outcome of second transplantation (HSCT2) for the treatment of primary graft failure (pGF) in 243 patients with acute leukemia. The median age was 44.8 years. 73.7% of patients achieved engraftment after HSCT2. The 5-year nonrelapse mortality was 51.6%, relapse incidence was 18.8%, leukemia-free survival was 29.6%, and overall survival was 30.7%.

Summary: This study comprehensively investigates the clinical and economic burden of graft-versus-host disease (GVHD) following allogeneic hematopoietic stem cell transplantation in France. The results demonstrate that patients with GVHD have a higher risk of severe infection, higher mortality rates, higher hospital admission rates, and higher direct costs compared to patients without GVHD.

Summary: The study investigated the incidence, diagnostic policy, management, and outcome of VOD/SOS at EBMT centers. It included 71 participating centers with a total of 2886 allogeneic transplantations and 93 cases of VOD/SOS in 2018. The findings showed a low incidence of VOD/SOS and a predominance of severe and very severe grades, indicating the importance of early diagnosis and treatment to improve outcomes.
