Autologous bone graft in the treatment of post-traumatic bone defects: a systematic review and meta-analysis

Background: This meta-analysis aimed to determine the bone union rate of bone defects treated with the different autologous bone graft techniques. Methods: The PubMed and the Cochrane Library databases were searched using the terms: 'fracture' AND ('bone loss' OR 'defect' OR'defects') AND 'bone graft', restricted to English language, to human species, and to a publication period from January 1999 to November 2014. Data were extracted by one of the reviewers and then checked by the second. A quality of evidence score and a methodology score were used. Heterogeneity was assessed. A random effects model approach was used to combine estimates. Results: Out of 376 selected studies only 34 met the inclusion criteria. The summary pooled union rate was 91 % (95 % CI: 87-95 %) while union rate after additional procedures raised to 98 % (95 % CI 96-99 %). No association between union rate and bone defect size was found. (Univariable regression model: vascularized: P = 0.677; non-vascularized: 0.202. Multivariable regression model: vascularized: P = 0.381; non-vascularized: P = 0.226). Vascularized graft was associated with a lower risk of infection after surgery when compared to non-vascularized graft (95 % CI 0.03 to 0.23, p < 0.001). Conclusion: The results of this meta-analysis demonstrate the effectiveness of autologous graft for bone defects. Furthermore, from the available clinical evidence bone defect size does not seem to have an impact on bone union when treated with autologous bone graft techniques.

In search of a 'pan-European value set'; application for EQ-5D-3L

Summary: This article describes the aim of estimating a 'pan-European' value set for the EQ-5D-3L to evaluate health care from the perspective of European decision-makers. By comparing and evaluating the methodologies of value sets from 12 European countries, it was found that an OLS model can best represent European preferences, thus enabling the development of a new 'pan-European' value set.

Informal Care Costs According to Age and Proximity to Death to Support Cost-Effectiveness Analyses

Summary: This study estimated informal care costs by age and proximity to death across European regions and showed how these estimates can be included in cost-effectiveness analyses. The results indicated that the use and costs of informal care increase with age and approaching death, and including informal care costs in the cost-effectiveness ratio can have a significant impact ranging from euro200 to euro17,700 per quality-adjusted life-year gained. The conclusion of this study is that considering informal care costs is necessary as it may influence decision-making in life-extending healthcare interventions.

Consensus recommendations on how to assess the quality of surgical interventions

The lack of standardized and universally applied endpoints for evaluating surgical interventions leads to inconsistent and biased interpretations, hindering patient-centered healthcare delivery. A jury-based consensus conference was conducted to develop recommendations for postoperative outcome assessment, incorporating the perspectives of different stakeholders. The Outcome4Medicine consensus group provides a framework for surgical outcome assessment and quality improvement, integrating the perspectives of the main stakeholders, with the goal of enhancing care quality and patient health.

Onlay Technique for Posterior Cruciate Ligament Reconstruction: The Paccola's Technique

Summary: The primary function of the posterior cruciate ligament (PCL) is to restrict the posterior translation of the tibia, and its secondary function is to limit the tibial external rotation, mainly at 90 degrees and 120 degrees of knee flexion. PCL rupture occurs in 3% to 37% of patients with knee ligament tears and is often associated with other ligament injuries. Surgical treatment is recommended for acute PCL injuries associated with knee dislocations or when stress radiographs show a tibial posteriorization greater than or equal to 12 mm.

Reevaluation of the surgical indications for anterior inferior iliac spine avulsion fractures in an acute setting-A narrative review of the current literature

Summary: The study aimed to re-evaluate the appropriate acute surgical treatment of AIIS avulsion fractures considering the three-dimensional anatomy of the supra-acetabular region. Conservative treatment of these injuries has been shown to lead to excellent outcomes, even when there is radiological evidence of displacement. However, some surgeons lack long-term follow-up beyond six months.

Understanding patient experience of distal tibia or ankle fracture: a qualitative systematic review

Summary: This study systematically reviewed qualitative studies on patients with distal tibia or ankle fracture, and explored their experience of injury and recovery. Clinicians should be aware of the patient experience and consider rehabilitation and psychosocial treatment strategies to address these challenges.

Societal Utilities for Cognitive Impairment in Schizophrenia: Developing a Preference-Based Scoring Algorithm Based on the Schizophrenia Cognition Rating Scale

Five key items were selected from the SCoRS and valued through one-on-one video calls using composite time trade-off (cTTO). The results showed that the SCoRS was more sensitive in detecting cognitive changes compared to the EQ-5D.

Low-dose prophylaxis protocol for heterotopic ossification after hip preservation surgery in a sport participants cohort

Summary: This study analyzed data from 98 sport participant patients who underwent arthroscopic or anterior mini-open treatment for femoroacetabular impingement (FAI). The results suggest that anterior mini-open treatment is associated with a higher risk of heterotopic ossification (HO) compared to hip arthroscopy, and that a treatment regimen of 60 mg etoricoxib daily for two weeks is effective in preventing HO formation in sport participant patients.
