Novel Biomarkers for Evaluation of Endothelial Dysfunction

Endothelial dysfunction is one of the earliest indicators of cardiovascular (CV) dysfunction, and its evaluation would be of considerable importance to stratify CV risk of many diseases and to assess the efficacy of atheroprotective treatments. Flow-mediated dilation is the most widely used method to study endothelial function. However, it is operator-dependent and can be influenced by physiological variations. Circulating biomarkers are a promising alternative. Due to the complexity of endothelial function, many of the biomarkers studied do not provide consistent information about the endothelium when measured alone. New circulating markers are being explored and some of them are thought to be suitable for the clinical setting. In this review, we focus on novel biomarkers of endothelial dysfunction, particularly endothelial microparticles, endocan, and endoglin, and discuss whether they fulfill the criteria to be applied in clinical practice.

Summary: In patients with HFrEF, the use of liraglutide may increase the risk of cardiovascular adverse effects, possibly driven by an increased risk of arrhythmias and worsening heart failure events. These results should be interpreted as exploratory and hypothesis-generating, and further randomized controlled trials are needed before drawing definitive conclusions.

Summary: This meta-analysis aims to assess the effect of GLP1-RA on heart failure in patients with and without a history of HF. The results showed that GLP1-RA can reduce the risk of HF and mortality in T2D patients without a history of HF. However, the effect of GLP1-RA on HF in patients with a history of HF was not significant.

The role of Graves' disease in the development of thyroid nodules and thyroid cancer

Summary: The prevalence of thyroid nodules and cancer in patients with Graves' disease was evaluated in a retrospective observational study. It was found that patients with multiple and larger nodules had a greater risk of thyroid cancer diagnosis, with most cases being low-grade papillary thyroid cancer. Further studies are needed to clarify the clinical relevance of these findings.

Cardiovascular risk factors during pregnancy impact the postpartum cardiac and vascular reverse remodeling

Summary: Pregnant women with cardiovascular risk factors are at high risk of developing cardiovascular disease later in life. Our study found that pregnant women undergo cardiovascular remodeling during pregnancy, which reverses after delivery. Arterial hypertension, smoking, and obesity are independent predictors of increased left ventricular mass. Additionally, circulating biomarkers such as C-reactive protein and ST2/IL33 receptors may be associated with worse cardiac remodeling.

Immune checkpoint inhibitor-induced endocrinopathies: a possible indicator of improved survival

Summary: This retrospective observational study found an association between the development of endocrine-related adverse effects (ERAE) and mortality in patients undergoing immunotherapy. The study showed a significant negative association between ERAE development and death. However, no associations were found between ERAE and characteristics such as age, sex, diabetes, medical history, immunotherapy duration, and immune checkpoint inhibitor type.

Summary: Decreased FT4 to FT3 conversion is associated with clinical features such as increased body fat, elevated pulmonary arterial systolic pressure, and reduced left ventricular ejection fraction in patients with HFpEF. Lower FT3/FT4 ratio predicts a higher risk of diuretic intensification, urgent HF visits, HF hospitalization, or cardiovascular death.

Adherence to glucose monitoring with intermittently scanned continuous glucose monitoring in patients with type 1 diabetes

Summary: This study aims to predict the adherence behavior of patients with Type 1 Diabetes to intermittently scanned continuous glucose monitoring (isCGM). The findings suggest that age, time in target, isCGM necessity, body mass index, and sex are predictors of adherence. Therefore, personalized strategies should be implemented to promote treatment adherence in patients with these characteristics.
