The objective of this study was to evaluate the clinical ability of Near-Infrared Light-Transillumination (NILT) for approximal dentinal caries detection and to compare with direct digital-radiography (DDR), as well as to determine whether the combination of both techniques improves the diagnostic capacity of the lesions. From 88 patients (over 18 years), 138 posterior teeth (76 molars and 62 premolars), that had approximal caries reached into dentine determined by DDR, were included. Lesion extension and DDR images were scored as follows: D0 = sound surface, D1/D2 = caries restricted to the outer/inner-half of the enamel, and D3/4 = caries restricted to the outer/inner-half of the dentin. Opening of the approximal surface using 0.5 mm-in-diameter diamond-bar was used as gold-standard. The lesion extension was then determined by the following criteria: no dentinal caries (D0/1/2) or dentinal caries (D3/4). Seventy-one lesions were D3 and 67 lesions were D4. Sensitivities of overall/D3/D4 were 98.0/95.7/100.0 (NILT) and 100/100/100 (DDR), respectively. Correlations with gold-standard were 0.92 (NILT) and 0.42 (DDR), respectively. The correlation increased to 0.97 (p = 0.045) on combining NILT and DDR. There was no difference in sensitivity between the methods (p > 0.05); but was differences in the correlation (p < 0.01). It can be concluded that NILT showed sensitivity similar to that of DDR and higher correlation than DDR for approximal dentinal caries detection. Accordingly, it may be used to monitor the progression of caries without exposing the patient to ionizing radiation, this being of particular interest in growing patients and in pregnant women. In this respect, NILT can be an effective diagnostic tool adjunctive to bitewing radiographs in detecting approximal dentinal caries. The combination of NILT and DDR represents an increase in the diagnosis of approximal lesions The proposed diagnostic protocol comprises visual examination, followed by NILT and DDR only if the former technique detects approximal caries.

Pulse Oximetry as a Diagnostic Tool to Determine Pulp Vitality: A Systematic Review

Alicia Almudever-Garcia, Leopoldo Forner, Jose Luis Sanz, Carmen Llena, Francisco Javier Rodriguez-Lozano, Julia Guerrero-Girones, Maria Melo

Summary: This systematic review highlights the potential application of pulse oximetry in diagnosing pulp vitality. While showing good sensitivity and specificity, its clinical use in dentistry is hindered by limitations such as the lack of a specific pulse oximeter for dental practice and insufficient evidence on the impact of different systemic and oral pathologies on pulp oxygen saturation levels.

Biomineralization potential and biological properties of a new tantalum oxide (Ta2O5)-containing calcium silicate cement

F. J. Rodriguez-Lozano, A. Lozano, S. Lopez-Garcia, D. Garcia-Bernal, J. L. Sanz, J. Guerrero-Girones, C. Llena, L. Forner, M. Melo

Summary: The new tantalum oxide (Ta2O5)-containing material, NeoMTA 2, exhibited higher calcium release compared to other materials. All groups showed similar effects on hDPSC viability and migration, with substantial cell attachment and spreading. NeoMTA 2 upregulated osteo/odontogenic gene expression and promoted higher ROS production in hDPSCs, indicating its potential for vital pulp therapy.

Biological interactions between calcium silicate-based endodontic biomaterials and periodontal ligament stem cells: A systematic review of in vitro studies

Summary: This study aimed to qualitatively synthesize available in vitro studies assessing the biological interaction of PDLSCs and calcium silicate-based biomaterials. The findings suggest that calcium silicate-based endodontic materials exhibit adequate cytocompatibility and promote the osteo/cementogenic differentiation and mineralization potential of PDLSCs. Further research is needed to explore the impact of compositional differences, additives, sample preparation, and environmental conditions on the biological properties of these materials.

Evaluation of Clinical, Biochemical and Microbiological Markers Related to Dental Caries

Maria D. Ferrer, Salvadora Perez, Aranzazu Lopez Lopez, Jose Luis Sanz, Maria Melo, Carmen Llena, Alejandro Mira

Summary: The study evaluated various markers related to dental caries in adults and found lactate in plaque to be associated with caries, while tooth-brushing frequency was related to levels of Streptococcus dentisani. Women showed better oral health parameters than men, and low pH and high lactate levels were associated with high caries rates. No association was found between bacteria levels and caries indices.

Summary: This study aims to determine the validity of two methods based on mineralisation of the third molar to predict the legal age of majority in a Spanish population. The results show that both the Demirjian method and the Cameriere method can be used with high reliability to determine the actual age of individuals, with a high predictive accuracy for the legal age of majority.

Triana Moreno, Jose Luis Sanz, Maria Melo, Carmen Llena

Summary: The study evaluated the tendency for overdiagnosis and overtreatment among last-year dentistry students at Valencia University in different clinical situations. Findings revealed students' inclination towards overtreatment of caries, inadequate selection of treatment for pulp pathology, and unnecessary treatment in medically compromised patients.

Calcium Silicate-Based Sealer Dentinal Tubule Penetration-A Systematic Review of In Vitro Studies

Israa Ashkar, Jose Luis Sanz, Leopoldo Forner, Maria Melo

Summary: The aim of this systematic review was to evaluate and compare the penetration of calcium silicate-based sealers into dentinal tubules using in vitro studies. A total of 12 eligible studies were identified from a preliminary search of multiple databases. The most commonly used method to assess sealer penetration was the use of fluorochrome and confocal laser microscopy. Calcium silicate-based sealers showed favorable dentinal tubule penetration, although factors such as irrigation protocol, sealer activation, filling method, and root canal morphology could impact the results. EndoSequence BC Sealer demonstrated the highest penetration among the tested sealers. Further research on the influence of different fluorochromes in dentinal tubule penetration studies is needed. The limitations of the included in vitro studies restrict the applicability of the findings to clinical settings. Prospero registration: CRD42022383896.

Volumetric Changes of the Paranasal Sinuses with Age: A Systematic Review

Amaya Iturralde-Garrote, Jose Luis Sanz, Leopoldo Forner, Maria Melo, Clara Puig-Herreros

Summary: This article systematically reviewed the available studies on the volumetric characteristics of paranasal sinuses and their changes with age. The results showed that the volume of the maxillary and ethmoidal sinuses decreases with age, while the findings on the frontal and sphenoidal sinuses were inconclusive. Therefore, further evidence is needed to determine the volumetric changes of the frontal and sphenoidal sinuses.

Remineralization Potential of Three Restorative Glass Ionomer Cements: An In Vitro Study

James Ghilotti, Iciar Fernandez, Jose Luis Sanz, Maria Melo, Carmen Llena

Summary: The aim of this in vitro study was to evaluate the remineralizing ability of three glass ionomers on demineralized dentin with different thicknesses and time periods. The results showed that all materials induced remineralization, with Riva Light Cure showing the most homogeneous results at all depths at 28 days. Equia Forte achieved the greatest effect at 2 mm and Ketac Molar at 1 mm. In terms of Ca/P ratio, Equia Forte and Riva Light Cure remineralized best at 2 mm, whereas for Ketac Molar, it was 1 mm. Carbonate apatite formation was higher at 24 h and 7 days for Ketac Molar, whereas it decreased at 14 days for Ketac Molar and peaked in Riva Light Cure and Equia Forte.
