Summary: Delayed bleeding is a significant concern in patients with high-grade splenic injuries who undergo non-operative management. The clinical manifestations of delayed bleeding include decreased hemoglobin levels, hypotension, tachycardia, new abdominal pain, and worsening abdominal pain. Hemoglobin decline is the main manifestation when bleeding events are detected in the intensive care unit, while new abdominal pain is the main presentation when patients leave the ICU.

Summary: Splenic rupture is a rare complication of colonoscopy that can lead to mortality if not diagnosed promptly. While most reported cases have been managed operatively, we present a case of a 75-year-old female who underwent conservative management for splenic rupture. Splenic rupture should be considered as a differential diagnosis in patients presenting with abdominal pain, syncope, and hypotension following colonoscopy. Decisions regarding operative versus conservative management should be based on the patient's clinical status, hemodynamics, and available resources.

Summary: This prospective study aimed to assess the incidence of colonoscopy-related perforation and analyze clinical factors associated with poor outcomes. The incidence rate of colonoscopy-related perforation was 0.66/1000, with higher rates in therapeutic colonoscopy. Factors such as diagnostic colonoscopy, abdominal rebound tenderness, sigmoid colon location, delayed recognition, and abdominal tenderness were associated with poor clinical outcomes.

Summary: Splenic injury is often associated with abdominal trauma, but it can also be a complication of colonoscopy. Various factors can increase the risk of this complication. We present a case of splenic injury after outpatient colonoscopy believed to be caused by intra-abdominal adhesions from previous abdominal surgeries. Timely diagnosis and intervention led to improved outcomes for our patient.

Summary: This study explored the efficacy and safety of midazolam combined with dezocine for sedation in IBD patients undergoing colonoscopy. The results showed that compared to ordinary colonoscopy, colonoscopy performed under midazolam and dezocine sedation was associated with lower pain levels, higher satisfaction, and lower complication rates. Therefore, this method is worthy of clinical promotion.
