Diabetes-Related Factors and the Effects of Ticagrelor Plus Aspirin in the THEMIS and THEMIS-PCI Trials

Summary: The THEMIS study and its subanalysis THEMIS-PCI showed that ticagrelor plus aspirin had a favorable net clinical benefit in individuals with type 2 diabetes and stable coronary artery disease, particularly among those who had previous percutaneous coronary intervention. The efficacy of ticagrelor plus aspirin on reducing major adverse cardiovascular events was generally consistent across subgroups, while the major bleeding event rate was increased by ticagrelor across all subgroups. Additionally, in THEMIS-PCI but not in the overall THEMIS population, ticagrelor generally produced a favorable net clinical benefit across different diabetes-related factors.

Summary: In this large prospective cohort study, a nonlinear association was found between HDL-C levels and bleeds in CAD patients who underwent PCI, with the highest risk at intermediate levels. A threshold value of HDL-C =35 mg/dL was associated with a decreased risk for bleeds. Further multicenter studies are needed to validate these findings.

Prognosis of Atrial Fibrillation Patients Undergoing PCI According to Anticoagulants and Antiplatelet Agents

Summary: Limited data evaluating conformation of antithrombotic therapy usage to guideline recommendations indicate that in clinical practice, most patients with atrial fibrillation (AF) who underwent percutaneous coronary intervention (PCI) continued to receive antiplatelet agents beyond 1 year post-PCI. Combination therapy of oral anticoagulants (OAC) with single antiplatelet therapy (SAPT) appears to be more effective than OAC monotherapy, without a difference in safety.

Optimising antithrombotic therapy after ACS and PCI

Summary: Dual antiplatelet therapy is the standard treatment for acute coronary syndrome patients undergoing percutaneous coronary intervention. The type and duration of treatment depend on the patient's risk for ischemic and hemorrhagic complications. De-escalation strategies can be used for high-risk bleeding patients with low risk of recurrent ischemic events, while strategies such as drug switching, dose escalation, or adding a new drug are suitable for patients at high ischemic risk. Individualized treatment decisions and trade-off considerations are necessary for patients at high ischemic and hemorrhagic risk.

Studies have shown that individuals with T2DM have higher baseline platelet activation and reactivity. This study summarizes the pathophysiological continuity between T2DM, platelet GPIb, and atherosclerosis, and highlights the potential of targeting GPIb as a novel antiplatelet therapy for preventing further cardiovascular incidents in T2DM patients.

An accurate and stable numerical method for option hedge parameters

Summary: In this paper, we propose an unconditionally stable numerical algorithm for accurate option pricing and hedge parameters based on the Feynman-Kac formula of the Black-Scholes equation. The algorithm discretizes the asset and time using uniform grid points and utilizes piecewise quadratic polynomial approximation and analytic integration to achieve higher accuracy and convergence. Additionally, the algorithm demonstrates stability and low time complexity through backward propagation of estimation errors following the convection and diffusion characteristics of the Black-Scholes equation.

Summary: This post hoc analysis observed a higher risk of MACE associated with tofacitinib compared to TNFi in RA patients with a history of ASCVD. Among patients without a history of ASCVD but with prevalent CV risk factors, there does not appear to be a difference in MACE risk between tofacitinib 5 mg twice daily and TNFi, although any absolute risk excess is likely low.

Eicosapentaenoic acid (EPA) reduces pulmonary endothelial dysfunction and inflammation due to changes in protein expression during exposure to particulate matter air pollution

Summary: This study investigated the pro-inflammatory effects of air pollution PMs on pulmonary endothelial cell NO bioavailability and protein expression, and whether EPA can restore endothelial cell function. The results showed that EPA can alter protein expression related to inflammatory pathways and reduce the levels of the inflammatory molecule sICAM-1, improving the NO/ONOO- release ratio. These cellular changes may contribute to the anti-inflammatory, cytoprotective, and lipid changes associated with EPA treatment during air pollution exposure.
