Microbial risk factors for treatment failure of pivmecillinam in community-acquired urinary tract infections caused by ESBL-producing Escherichia coli

The aim of this study was to identify microbial risk factors for treatment failure of pivmecillinam in community-acquired urinary tract infections (ca-UTIs) caused by ESBL-producing Escherichia coli. Eighty-nine ESBL-producing E. coli isolated from women suffering from ca-UTIs were included. The susceptibilities to mecillinam were determined using MIC gradient strip. Whole genome sequencing was performed on a MiSeq platform, and genome assembly was performed using SPAdes v3.11.0. Neither mecillinam MICs nor ESBL genotypes were associated with treatment outcome of patients treated with pivmecillinam. Specific STs, however, showed significant differences in treatment outcome. Patients infected with ST131 were more likely to experience treatment failure compared to patients infected with non-ST131 (p 0.02) when adjusted for pivmecillinam dose, mecillinam MIC and severity of infection. Patients infected with ST69 were more often successfully treated compared to patients infected with non-ST69 (p 0.04). Patients infected with bla(CTX-M-15) ST131 strains were more likely to experience treatment failure than those infected with non-bla(CTX-M-15) ST131 strains (p 0.02). The results suggest that specific STs are associated with the clinical efficacy of pivmecillinam. Further studies with a larger number of strains, including a larger number of mecillinam resistant strains, are needed to confirm these results.

Summary: This study investigates the genetic diversity, resistome, and pathogenic potential of Kp isolates from the Norwegian marine environment using whole-genome sequencing. The study provides the first comprehensive account of genetic diversity among Kp from the marine environment and reveals a high diversity of Kp in the Norwegian marine environment and seafood, including globally disseminated pathogenic sequence types carrying clinically relevant antibiotic resistance genes and virulence factors, as well as several heavy metal resistance genes.

Within-patient and global evolutionary dynamics of Klebsiella pneumoniae ST17

Summary: Klebsiella pneumoniae ST17 is a globally disseminated clone that causes multidrug-resistant hospital infections. An outbreak of MDR ST17 occurred at a neonatal intensive care unit in Norway, with long-term colonization observed in the children even after discharge. The within-host evolution of ST17 during colonization showed stability with minimal genetic changes and persistent carriage of a specific plasmid. ST17 has diverse lineages with various AMR genes and virulence loci, but many strains do not acquire AMR. The non-human sources and human colonization may play a crucial role in severe infections.

Highly conserved composite transposon harbouring aerobactin iuc3 in Klebsiella pneumoniae from pigs

Summary: In this study, Klebsiella pneumoniae isolated from healthy fattening pigs in Norway were characterized. The results showed a high detection rate of the pathogen in pig caecal samples, and most of the isolates carried acquired virulence determinants, including the siderophore aerobactin. In contrast, only a few pig isolates from Thailand harbored aerobactin. The virulence genes were located on potentially transferable plasmids and were associated with a predicted composite transposon.

Co-localization of clinically relevant antibiotic- and heavy metal resistance genes on plasmids in Klebsiella pneumoniae from marine bivalves

Summary: This study aimed to determine the genetic background of antibiotic and heavy metal resistance genes in antibiotic-resistant K. pneumoniae isolated from marine bivalves. Complete genome sequencing was conducted on five antibiotic-resistant isolates, and plasmid transfer potential was examined through conjugation experiments. The results revealed the co-occurrence of antibiotic and heavy metal resistance genes on a conjugative IncFIB plasmid, highlighting the potential dissemination route for antimicrobial resistance through the marine environment and seafood.

The diverse population structure of colonizing isolates reflects the bacterium's adaptive capacity and poses challenges for vaccine development and identifying reservoirs as a potential source for human colonization.
