Bone morphogenetic protein-2 (BMP-2) is a potent growth factor affecting bone formation. While recombinant human BMP-2 (rhBMP-2) has been commercially available in cases of non-union fracture and spinal fusion in orthopaedics, it has also been applied to improve bone regeneration in challenging cases requiring dental implant treatment. However, complications related to an initially high dosage for maintaining an effective physiological concentration at the defect site have been reported, although an effective and safe rhBMP-2 dosage for bone regeneration has not yet been determined. In contrast to protein delivery, BMP-2 gene transfer into the defect site induces BMP-2 synthesis in vivo and leads to secretion for weeks to months, depending on the vector, at a concentration of nanograms per milliliter. BMP-2 gene delivery is advantageous for bone wound healing process in terms of dosage and duration. However, safety concerns related to viral vectors are one of the hurdles that need to be overcome for gene delivery to be used in clinical practice. Recently, commercially available gene therapy has been introduced in orthopedics, and clinical trials in dentistry have been ongoing. This review examines the application of BMP-2 gene therapy for bone regeneration in the oral and maxillofacial regions and discusses future perspectives of BMP-2 gene therapy in dentistry.

Risk factors impacting the survival of implants replaced following failure: A retrospective study

Dongseob Lee, Young-Chang Ko, Yang-Jo Seol, Ki-Tae Koo, Yong-Moo Lee, Jungwon Lee

Summary: This study aimed to investigate factors influencing the survival of replaced dental implants. The results showed that a history of periodontitis, guided bone regeneration (GBR) at first implant placement, and non-GBR at primary implant replacement were identified as independent risk factors for second implant removal. The 5-year survival rate was significantly lower in replaced implants at sites with a periodontitis history compared to those without.

Effect of polydeoxyribonucleotide on early bone formation in lateral bone augmentation with immediate implant placement: an experimental in vivo study

Dongseob Lee, Jungwon Lee, Yang-Jo Seol, Yong-Moo Lee, Ki-Tae Koo

Summary: This study investigated the early bone formation using collagenated biphasic calcium phosphate (CBCP) and the potential effects of polynucleotide (PDRN) on bone formation. The results showed that the use of CBCP/PDRN in the repair of bone defects around implants can facilitate bone growth.

The impact of polydeoxyribonucleotide on early bone formation in lateral- window sinus floor elevation with simultaneous implant placement

Dongseob Lee, Jungwon Lee, Ki-Tae Koo, Yang -Jo Seol, Yong -Moo Lee

Summary: This study aimed to evaluate the impact of polydeoxyribonucleotide (PDRN) on histologic outcomes when implant placement and lateral sinus floor elevation are performed simultaneously. The results showed that using PDRN for lateral sinus floor elevation resulted in better new bone formation and bone-to-implant contact.

Summary: This study aimed to investigate the proper wavelengths for safe levels of light emitting diode (LED) irradiation with bactericidal and photobiomodulation effects in vitro. LED irradiation at 470 nm showed bactericidal effects, while LED irradiation at 525 and 630 nm showed preventive and treatment effects on LPS-induced RAW 264.7 inflammation.

Repeated irradiation by light-emitting diodes may impede the spontaneous progression of experimental periodontitis: a preclinical study

Hyemee Suh, Jungwon Lee, Sun-Hee Ahn, Woosub Song, Ling Li, Yong-Moo Lee, Yang-Jo Seol, Ki-Tae Koo

Summary: Repeated LED irradiation at 470-nm and 525-nm wavelengths can suppress the progression of experimental periodontitis. The study used a beagle dog model and monitored the spontaneous progression of periodontitis for 3 months under specific treatment regimens. The results showed significant differences in probing pocket depth (PPD) and clinical attachment level (CAL) between the LED treatment group and the control group, while gingival recession (GR) did not differ significantly. Histomorphometric analysis demonstrated that the lengths of junctional epithelium, connective tissue (CT) zone, and total soft tissue (ST) were significantly higher in the LED treatment group.

Ling Li, Jungwon Lee, Young-Dan Cho, Sungtae Kim, Yang-Jo Seol, Yong-Moo Lee, Ki-Tae Koo

Summary: Hyaluronic acid (HA) affects angiogenesis and promotes the migration and differentiation of mesenchymal cells, thereby activating the osteogenic ability of osteoblasts. This study found that a dose of 75 mg/mL HA showed the best bone formation in a rat critical-size defect model.

Summary: Direct He-APPJ treatment for up to 3 minutes showed no harmful effects at either the cell or tissue level. Immediate cell detachment of human gingival fibroblasts and osteoblasts was observed regardless of treatment time, but areas detached in the 1 or 3 minute treatment groups were completely repopulated within 7 days. Alkaline phosphatase activity was not affected by 1 or 3 minutes of plasma treatment, but significantly lower in the 5 minute-treated group.
