Summary: The concept of textbook outcome (TO) is gaining popularity in surgical research as a way to evaluate the quality and success of different surgical procedures, including hepatopancreatobiliary (HPB) operations. TO is made up of individual outcome parameters reflecting different care domains, and can be used to assess patient-level outcomes, hospital performance, and quality metrics. Efforts are needed to establish a universal definition of TO in HPB surgery for comparing outcomes and improving patient results.

Patients were categorized into low, intermediate, and high-risk groups based on body mass index and main pancreatic duct diameter, which accurately predicted the risk of postoperative pancreatic fistula.

Summary: This study compared the diagnostic value of four tools (GLIM criteria, SGA, PG-SGA, and PNI) in malnutrition among hospitalized patients undergoing hepatobiliary-pancreatic surgery, and observed the nutritional intervention of these patients. The results showed that the prevalence of malnutrition varied among different tools, with GLIM showing the lowest prevalence. The concordance between PG-SGA and GLIM was relatively high, while SGA and PNI had lower concordance with GLIM.

Summary: This study aimed to assess predictors of acute postoperative complications for breast cancer patients and develop a predictive model that calculates complication probability using patient risk factors. The models built showed good performance in predicting various types of complications with high accuracy and calibration measures. The risk calculator developed has the potential to help stratify patients with higher complication risks and improve decision-making process expectations.

Summary: Although robot-assisted HPB surgery has gained momentum globally, the adoption of robotic surgery in the UK lags behind due to cost implications in a publicly funded NHS. This study describes the successful establishment of a robotic HPB programme with structured training and clinical outcomes. The team's training pathway has been effective in setting up a safe robotic HPB programme and could serve as a roadmap for new Robotic HPB services.

Summary: This study aimed to assess the utility of preoperatively available radiological features and serological biomarkers in choosing an optimal surgical indication for non-functioning pancreatic neuroendocrine tumors (PanNETs). The combination of hetero/hypo-attenuation and main pancreatic duct involvement accurately predicted high-risk pathological factors. These findings suggest the potential for guiding surgical decisions for non-functioning PanNETs.

Summary: The objective of this study is to describe the techniques and prove the feasibility of performing complex hepatobiliary and pancreatic surgery on a Jehovah Witness (JW) population, and propose a strategy to offset surgical blood loss. A retrospective review of JW patients undergoing liver and pancreatic resections was performed, and perioperative management and patient outcomes were described. The study concludes that deliberate perioperative management makes transfusion-free liver and pancreatic resections feasible.

Summary: Routine testing of staff may be the most effective and practical approach in controlling COVID-19 transmission in care homes, more effective than testing residents alone and with negligible additional effect when testing both staff and residents. The modeling results are sensitive to the transmission probability per contact, but the qualitative finding remains robust to varying parameter values in uncertainty analysis.

Summary: Population and patient factors contributing to challenges in delivering UHC for hypertension in Ghana include unhealthy lifestyles burdening the health system, poor health-seeking behavior, and poor medication adherence leading to uncontrolled cases. Health system factors such as inadequate capacity for early diagnosis, inequitable distribution of healthcare facilities, financial sustainability issues with the National Health Insurance Scheme, and use of non-standardized blood pressure measuring equipment by healthcare providers also pose significant challenges. Ghana will need to address these issues to achieve universal health coverage.
