3D imaging applications for robotic urologic surgery: an ESUT YAUWP review

Context Despite the current era of precision surgery in robotics, an unmet need still remains for optimal surgical planning and navigation for most genitourinary diseases. 3D virtual reconstruction of 2D cross-sectional imaging has been increasingly adopted to help surgeons better understand the surgical anatomy. Objectives To provide a short overview of the most recent evidence on current applications of 3D imaging in robotic urologic surgery. Evidence acquisition A non-systematic review of the literature was performed. Medline, PubMed, the Cochrane Database and Embase were screened for studies regarding the use of 3D models in robotic urology. Evidence synthesis 3D reconstruction technology creates 3D virtual and printed models that first appeared in urology to aid surgical planning and intraoperative navigation, especially in the treatment of oncological diseases of the prostate and kidneys. The latest revolution in the field involves models overlapping onto the real anatomy and performing augmented reality procedures. Conclusion 3D virtual/printing technology has entered daily practice in some tertiary centres, especially for the management of urological tumours. The 3D models can be virtual or printed, and can help the surgeon in surgical planning, physician education and training, and patient counselling. Moreover, integration of robotic platforms with the 3D models and the possibility of performing augmented reality surgeries increase the surgeon's confidence with the pathology, with potential benefits in precision and tailoring of the procedures.

Summary: This study aimed to develop a model based on mp-MRI and target biopsy (TB) alone to predict the risk of lymph-node invasion (LNI). Using logistic regression analysis, a model was created and a nomogram was generated to predict the risk of LNI. The model showed good discrimination and was able to identify patients with LNI risk above 7%.

Summary: This study compared the outcomes of robot-assisted partial nephrectomy (RAPN) and percutaneous thermal ablation (PTA) in the treatment of renal tumors cT1 in solitary kidney. The results showed that there were no significant differences in surgical, functional, and oncological outcomes between the two methods, but PTA may carry a higher risk of recurrence and may require retreatment.

6D object position estimation from 2D images: a literature review

Summary: The 6D pose estimation of an object from an image is a central problem in Computer Vision (CV), and this review provides a comprehensive overview of different techniques for solving this problem. The review analyzes methods from various research fields and categorizes them into template-based, feature-based, and learning-based methods. Learning-based methods, which involve training neural networks for specific tasks, have received the most focus in recent years and are further classified into three sub-categories. The main goal of this review is to guide readers and highlight the best-performing techniques for implementing high-quality applications, even in challenging scenarios such as occlusions, symmetries, occlusions between multiple objects, and poor lighting conditions.

Summary: In this systematic review and meta-analysis, the cumulative bladder cancer (BC) recurrence rates and efficacy profile of different Bacillus of Calmette-Guerin (BCG) strains were compared. The study found that BCG Tokyo 172 had the lowest recurrence rate among studies with <= 3-year recurrence-free survival (RFS). No significant difference was observed among strains in >3-year RFS outcomes. The genetically different BCG strains RIVM, Tice, and Tokyo 172 showed some potential enhanced benefits.

Summary: Lymph node invasion is a poor prognostic factor for salvage prostatectomy patients. This study found that lymph node invasion significantly affects the oncologic outcomes after salvage prostatectomy. On the other hand, there is no benefit for lymph node dissection during salvage prostatectomy. These findings highlight the importance of cautious selection of lymph node dissection and strict postoperative monitoring for salvage prostatectomy patients with lymph node invasion.

Summary: The study compared perioperative and oncologic outcomes among patients who underwent open, laparoscopic, and robotic RNU for non-metastatic upper urinary tract urothelial carcinoma. The results showed worse bladder recurrence-free survival in patients who had laparoscopic and robotic RNU compared to open RNU. However, recurrence-free, cancer-specific, and overall survival were similar between the three surgical approaches. Laparoscopic and robotic RNU were associated with shorter hospital stay and fewer major postoperative complications. Further research is needed to investigate the factors responsible for the inferior bladder recurrence-free survival in patients treated with minimally invasive techniques.

Summary: The study aimed to determine the accuracy of a proposed two-tier re-classification of the M domain in the TNM staging of RCC. The analysis of patients from the REMARCC database showed that the M2 classification was an independent risk factor for worsened mortality. Comparisons between M1 and M2 groups demonstrated significant differences in overall survival and cancer-specific survival.

Surgery is mainly offered for functional or malignant disease and the choice of surgical approach varies by physician specialty. The software, called BLAIR, achieved over 90% accuracy in predicting bleeding events, demonstrating the potential of AI in assisting surgeons during interventions. This study exemplifies the positive impact AI applications can have on surgical procedures.
