Pregnancy in patients with autoimmune disease: A reality in 2016

Autoimmune rheumatic diseases are chronic systemic conditions often affecting young women during their reproductive years, so that pregnancy is a major issue in their management. For a long time pregnancy has been discouraged in these women, mainly for two reasons: gestation could aggravate maternal disease and, vice versa, the disease could negatively influence the gestational outcome. The great improvement in the approach to pregnancy done in the past few decades has allowed a progressively increasing number of affected women to fulfill their family plan. Women should be informed about potential risks related to their disease, but they should also be reassured that a good pregnancy outcome is possible if conception occurs in a stable remission state, teratogenic medications have been properly withdrawn and safe drugs have been mantained to prevent disease flare. A brief excursus regarding the main issues regarding SLE/APS, Systemic Sclerosis and Systemic Vasculitis is provided, in the attempt to delineate the main risk factors for adverse pregnancy outcome, the onset of maternal complications and the role played by a close multi-specialistic monitoring. Summary: Concerns about the health of offspring are one reason for reduced family sizes among women with rheumatic diseases (RD). However, data gathered from a survey of 398 patients diagnosed with RD during childbearing age indicate that the risk of autoimmune diseases (AD) and neurodevelopmental disorders (ND) in their children is relatively low.

Summary: Systemic sclerosis can affect reproductive health, especially in women. Preconception counseling, treatment adjustment, and close monitoring during pregnancy are important to minimize risks and ensure successful outcomes. Preterm birth and spontaneous abortion are common in women with systemic sclerosis. Overall disease activity usually remains stable during pregnancy, but women with severe organ involvement should be closely monitored. Sexual dysfunction is frequently reported in systemic sclerosis patients.

Managing puerperium in patients with systemic autoimmune diseases: an update

Summary: This article summarizes the recent research on the psychological and pharmacological management of patients with autoimmune rheumatic diseases during puerperium, as well as the possible risk factors for disease flares. Physicians involved in the management of patients during puerperium and in the follow-up of babies need to agree on maternal treatment to assure mothers about the safety of prescribed medications. Furthermore, women with rheumatic diseases may experience musculoskeletal limitations and psychological problems, such as postpartum depression, which can affect their ability to fulfill their maternal tasks. Families and physicians should be aware of these possible complications and provide proper counseling and practical help to support new mothers.

Impact of COVID-19 and COVID-19 vaccination on high-risk patients with antiphospholipid syndrome: a nationwide survey

Summary: This nationwide multicenter survey evaluated the course of COVID-19 and adverse events after vaccination in patients with triple-positive thrombotic APS. The results showed that these patients did not suffer from severe COVID-19 outcomes and COVID-19 vaccination was well tolerated. These findings can help reassure patients and physicians and reduce hesitancy in unvaccinated individuals.

Summary: This study assessed the efficacy of belimumab in treating joint and skin manifestations in a nationwide cohort of SLE patients. The results showed that belimumab significantly improved joint and skin symptoms and reduced the use of glucocorticoids. A significant proportion of patients who did not achieve complete remission initially achieved remission during follow-up.

Summary: This study investigated the safety of COVID-19 vaccines in pregnant and breastfeeding women with autoimmune diseases. The results showed that adverse events were more frequent in pregnant women compared to non-pregnant patients, but there was no significant difference compared to pregnant healthy controls. There was no difference in adverse events between breastfeeding women and healthy controls. This study provides reassuring insights into the safety of COVID-19 vaccines for women with autoimmune diseases during pregnancy and postpartum periods.

Summary: This study aimed to investigate the differences in COVID-19 prognosis between patients affected by SLE, RA, and SPA. The results showed no difference in COVID-19 outcome between SLE and RA patients, while SPA patients had a more favorable disease course compared to SLE patients. Risk factors for severe outcomes in SLE patients included age >65 years, hypertension, and prednisone use, while hydroxychloroquine use was found to be protective.
