In recent years, the NCCN Clinical Practice Guidelines in Oncology (NCCN Guidelines) for Adult Cancer Pain have undergone substantial revisions focusing on the appropriate and safe prescription of opioid analgesics, optimization of nonopioid analgesics and adjuvant medications, and integration of nonpharmacologic methods of cancer pain management. This selection highlights some of these changes, covering topics on management of adult cancer pain including pharmacologic interventions, nonpharmacologic interventions, and treatment of specific cancer pain syndromes. The complete version of the NCCN Guidelines for Adult Cancer Pain addresses additional aspects of this topic, including pathophysiologic classification of cancer pain syndromes, comprehensive pain assessment, management of pain crisis, ongoing care for cancer pain, pain in cancer survivors, and specialty consultations.

Combination pharmacotherapy for the treatment of neuropathic pain in adults: systematic review and meta-analysis

Marielle Balanaser, Meg Carley, Ralf Baron, Nanna B. B. Finnerup, R. Andrew Moore, Michael C. C. Rowbotham, Luis E. E. Chaparro, Ian Gilron

Summary: Neuropathic pain causes significant health issues and often cannot be effectively treated with a single medication. Combining different drugs may provide better pain relief and/or tolerability. However, current evidence does not show superiority of combination therapies over monotherapy. Further research is needed to identify effective combinations and ensure their safety and overall benefit for patients.

A retrospective investigation of the relationship between neuroblastoma response to anti-GD2 monoclonal antibodies and exposure to opioids for pain management

Kyle J. Morgan, Andrew Dudas, Wayne L. Furman, M. Beth McCarville, Barry L. Shulkin, Zhaohua Lu, Himani Darji, Doralina L. Anghelescu

Summary: This study examined the association between opioid consumption and tumor response in children with neuroblastoma. The results showed that there was no significant correlation between opioid use and tumor volume or burden.

Perioperative Indications for Gabapentinoids in Pediatrics: A Narrative Review

Elizabeth A. Hall, Hope H. Brandon, Hilary M. Jasmin, Kavitha C. Raghavan, Doralina L. Anghelescu

Summary: There has been a growing interest in using gabapentinoids in pediatric patients during the perioperative period. The outcomes of using gabapentinoids during this period vary and include acute complications such as pain, anxiety, nausea and vomiting, as well as long-term complications like chronic postsurgical pain. This review aims to summarize the current literature on the use of gabapentinoids in pediatric patients and their specific indications in the perioperative setting.

Self-reported physical function is strongly related to pain behavior and pain interference and weakly related to physical capacity in people with chronic low back pain

Nicholas Karayannis, Matthew Smuck, Christine Law, Sean C. Mackey, James J. Gross, Beth D. Darnall, Julia Hush

Summary: The relationship between self-reported factors of physical function and pain with objective physical capacity measures was examined in this study. The results showed a weak correlation between self-reported physical function and walking speed, lower extremity strength, and a moderate correlation with pain interference, pain behavior, and fear-avoidance beliefs. These findings suggest that self-reported limitations in physical function may have an impact on individuals' physical capacity.

Pain acceptance and psychological inflexibility predict pain interference outcomes for persons with chronic pain receiving pain psychology

Nicholas Vasilis. Karayannis, John A. Sturgeon, Mike K. Kemani, Sean C. Mackey, Carol M. Greco, Rikard K. Wicksell, Lance M. McCracken

Summary: Awareness, acceptance, and engagement are three dimensions of psychological flexibility. This study aimed to test the predictive capacity of these dimensions on the longitudinal trajectory of pain interference in patients receiving pain psychology treatment.

Should Buprenorphine Be Considered a First-Line Opioid for the Treatment of Moderate to Severe Cancer Pain

Marcin Chwistek, Dylan Sherry, Leigh Kinczewski, Maria J. Silveira, Mellar Davis

Summary: Cancer pain is a significant problem worldwide, and opioids, particularly morphine, are the first-line treatment for moderate to severe cancer pain. Buprenorphine has emerged as an alternative opioid for chronic pain and substance use disorder, but its role as a first-line opioid for cancer pain remains controversial. In this article, three expert clinicians independently discuss the use of buprenorphine in cancer pain and highlight its potential benefits and unique characteristics. They agree that buprenorphine can be used as a first-line opioid in specific populations and mention the need for further research, particularly randomized clinical trials comparing buprenorphine with other opioids in different pain syndromes.

ADCC's Improving Goal Concordant Care Initiative: Implementing Primary Palliative Care Principles

Summary: The Alliance of Dedicated Cancer Centers (ADCC) initiated the Improving Goal Concordant Care Initiative (IGCC) to address the lack of formal communication training and institutional support among oncologists. IGCC requires four core components for participation, aiming to improve patient outcomes and promote care that aligns with patient preferences.

Characterization and burden of localized back pain versus back pain with chronic overlapping pain conditions

Abdullah Sulieman Terkawi, Rita A. Popat, Sean Mackey

Summary: This study aimed to compare patients with isolated chronic low back pain (cLBP) to those with cLBP and associated chronic overlapping pain conditions (COPCs) across multiple domains of functioning. The results showed that patients with COPCs had significantly worse outcomes in physical, psychological, social, and global health.

Preoperative Versus Perioperative Risk Factors for Delayed Pain and Opioid Cessation After Total Joint Arthroplasty: A Prospective Cohort Study

Jennifer M. Hah, Julien D. Veron Vialard, Bradley Efron, Sean C. Mackey, Ian R. Carroll, Derek F. Amanatullah, Balasubramanian Narasimhan, Tina Hernandez-Boussard

Summary: This study aimed to analyze the impact of preoperative and postoperative risk factors on persistent postoperative pain and opioid use. The results showed a significant association between preoperative opioid use and delayed postoperative opioid cessation, while postoperative increases in depressive symptoms and pain catastrophizing were independently associated with delayed postoperative pain resolution. Therefore, risk stratification of patients before and after surgery can identify those at highest risk for these distinct outcomes, and interventions targeting these perioperative risk factors may prevent prolonged postoperative pain and opioid use.

Patient-Caregiver Portal System in Palliative Oncology: Assessment of Usability and Perceived Benefit

Margaret L. Longacre, Marcin Chwistek, Cynthia Keleher, Mark Siemon, Brian L. Egleston, Molly Collins, Carolyn Y. Fang

Summary: The study demonstrates the usability and satisfaction of a patient-caregiver portal system in engaging caregivers systematically. The system allows patients to specify their caregiver and communication preferences, connects caregivers to a unique portal page, and provides electronic notifications to the care team. The findings highlight the need for further research on caregivers of patients with different illnesses.
