Stability and Impact of Symptom Clusters in Patients With End-Stage Renal Disease Undergoing Dialysis

Context. Patients with end-stage renal disease undergoing dialysis experience multiple concurrent symptoms. These symptoms cluster together and have negative impacts on patient outcomes. However, information on changes in symptom clusters over time is limited. Objectives. This longitudinal study examined the stability of symptom clusters and their impacts on health-related quality of life and functional status over a period of one year. Methods. Eligibility criteria were patients diagnosed with end-stage renal disease; had received dialysis consecutively for at least three months; and had given written informed consent. Dialysis Symptom Index, Kidney Disease Quality of Life 36, and Karnofsky Performance Status Scale were used to evaluate the impacts of symptom clusters and outcomes. Exploratory factor analyses and multiple regression analyses were used to determine symptom clusters and their associations with patient outcomes. Results. Among the 354 recruited patients, 271 completed the 12-month assessment. Four symptom clusters were identified across the three assessments, namely, uremic, gastrointestinal, skin, and emotional. Within each cluster, the specific symptoms were varied. The uremic symptom cluster accounted for the largest amount of variability. Across the three assessments, a higher uremic cluster factor score was associated with poorer physical well-being, whereas a higher emotional cluster factor score was consistently associated with poorer mental well-being. Conclusion. Symptoms in patients on dialysis clustered in relatively stable patterns. The four symptom clusters identified had consistent negative effects on various aspects of patients' well-being. Our findings suggest the need for ongoing symptom assessment and early recognition of symptoms that may contribute to adverse patient outcomes. Summary: Few experimental studies have tested Acceptance and Commitment Therapy (ACT) in parents of children with cancer. ACT is feasible considering the high rates of recruitment, adherence and retention. ACT is well accepted with high levels of satisfaction. ACT is effective in improving psychological flexibility and reducing guilt and worry. Future full-scale RCTs are warranted to confirm the results of this work.

Exploring social capital for family caregivers of patients with chronic organ failure: study protocol for a concept mapping study

Summary: This study aims to explore the social capital available for family caregivers of patients with chronic organ failure. A participatory design will be utilized, and the study will be divided into three phases to prepare, generate statements, create a concept map, prioritize statements, and formulate action plans based on the concept map. Recommendations on policies to improve the caregiving experience and enhance social care will be produced based on the findings.

Unmet needs, anxiety, depression, and quality of life among caregivers of adolescents and young adults with cancer: A cross-sectional study

Summary: This study examined the unmet needs, anxiety, depression, and quality of life of caregivers of adolescents and young adults with cancer in a tertiary cancer hospital in China. The results showed that many caregivers had unmet needs and a significant proportion reported anxiety and depression. After adjusting for other factors, personal and emotional needs, healthcare access and continuity, and financial needs were significantly associated with the caregivers' mental health and quality of life.

The results showed statistically significant interaction effects of ACT on health-related quality of life, cancer-related fatigue, depressive symptoms, anxiety, and distress.

Summary: This study aimed to explore the factors associated with COVID-19 vaccination adherence among Indonesian adolescents. The results showed that higher levels of knowledge, positive attitudes, and confidence in the vaccine were significantly associated with higher vaccination adherence in adolescents. On the other hand, younger age and habitation in private housing were related to lower adherence to the vaccination program.

Use of a Linguistically Appropriate Decision Aid for Cervical Cancer Screening of South Asian Ethnic Minority Women in Hong Kong: A Pilot Randomised Controlled Trial

Summary: This study investigated the feasibility and acceptability of a linguistically appropriate printed decision aid for cervical cancer screening in South Asian women, as well as its effects on decisional conflicts, clarity of values, risk perception, the screening decision, and uptake. The results showed that the decision aid was useful in aiding decision-making and led to reduced decisional conflicts and increased screening uptake. It is recommended to develop decision aids in various forms, such as printed and mobile applications, to improve convenience.

Loneliness at end-of-life: A scoping review

Summary: This scoping review synthesized the literature on loneliness at end-of-life and identified knowledge gaps. Data on the prevalence of loneliness among adults at end-of-life is lacking. Factors predisposing adults to loneliness at end-of-life include withdrawal from social networks, inability to share emotions, and inadequate spiritual support. Strategies to alleviate loneliness need further substantiation through clinical trials.

Health inequity associated with financial hardship among patients with kidney failure

Summary: This cross-sectional study aimed to investigate the impact of financial status on patient-reported and clinical outcomes among kidney failure patients. The study found that patients with poorer financial status experienced increased distress in specific symptoms and higher healthcare utilization. Hope, quality of life, and psychological distress in patients on peritoneal dialysis: A cross-sectional study

Summary: This study aimed to assess the level of hope among patients on peritoneal dialysis and evaluate its association with health-related quality of life and psychological distress. The findings showed that employed, higher income, and patients receiving automated peritoneal dialysis reported higher hope scores. Hope was significantly correlated with age and social support. A higher hope score was associated with better mental well-being and less severe depressive symptoms.

Perceptions and experiences of exercise among pregnant women

Summary: This qualitative study aimed to explore the perceptions and experiences of physical exercise among pregnant women in China. The study found that pregnant women recognized the need for exercise but faced barriers such as lack of knowledge, confidence, support, and concerns about safety. Tailored exercise programs that include education, reassurance, motivational strategies, and professional support could help address these barriers and improve exercise participation among pregnant women.

'Life became slow down': A descriptive qualitative study of the experiences of cancer-related fatigue amongst people with advanced lung cancer

Therefore, there is an urgent need for culturally sensitive educational interventions to help South-East and East Asian parents in managing their children's fever.

Nursing students' perceived anxiety and heart rate variability in mock skill competency assessment

Summary: Skill competency assessments induce stress and anxiety, which may affect nursing student performance. In this study, it was found that heart rate variability significantly decreased during the assessment and increased significantly 10 minutes after the assessment. Higher performers showed significantly lower heart rate variability during and after the assessment. The assessment score was negatively correlated with heart rate variability during and after the assessment.
