Sleep Duration Is Associated With Subclinical Carotid Plaque Burden

Summary: This study found an association between long sleep duration and subclinical carotid atherosclerosis, which may explain the previously observed link between long sleep and stroke. Summary: This study using shear-wave elastography (SWE) found that asymptomatic stable carotid plaques had significantly higher elasticity compared to asymptomatic progressive and symptomatic plaques. Osteopontin targeted non-invasive nanoprobes with amplified surface plasmon resonance for photothermally enhanced multimodal precision imaging of vulnerable atherosclerotic plaques

Summary: An innovative SAmOCP nanoprobe with versatile functions has been developed for precise diagnosis of vulnerable atherosclerosis (AS) plaques at the molecular level. The SAmOCP nanoprobes actively recognize foam cells and specifically target vulnerable AS plaques in vivo. By utilizing the enhanced localized surface plasmon resonance effects, the encapsulated liquid perfluoropentane (PFP) can transform into abundant microbubbles, enabling greatly enhanced ultrasonic imaging capacity. The SAmOCP nanoprobes exhibit a triple-modality diagnostic efficacy of ultrasound/fluorescence/photothermal imaging, offering a promising solution for the diagnosis of vulnerable AS.

POST-IVUS: A perceptual organisation-aware selective transformer framework for intravascular ultrasound segmentation

Summary: In this paper, a novel perceptual organisation-aware selective transformer framework is proposed for accurate and robust segmentation of vessel walls in IVUS images. The framework utilizes temporal context-based feature encoders to extract efficient motion features of vessels and incorporates a perceptual organisation-aware selective transformer module to extract accurate boundary information. The obtained results are fused in a temporal constraining and fusion module to determine the most likely correct boundaries with robustness to morphology. Extensive evaluation on non-selected IVUS sequences demonstrates that the proposed methods outperform the state-of-the-art. The work has been integrated into a user-friendly software for automatic IVUS image segmentation.

Plaque Structural Stress: Detection, Determinants and Role in Atherosclerotic Plaque Rupture and Progression

Summary: Atherosclerosis is a major cause of death globally, with myocardial infarctions often resulting from rupture or erosion of coronary plaques. Imaging modalities alone have limited ability to accurately assess plaque risk, necessitating the use of additional biomarkers. Plaque structural stress (PSS), which incorporates various variables, has been shown to be correlated with acute coronary syndrome, plaque rupture, and major adverse cardiovascular events, offering valuable prognostic information beyond imaging. This review explores the methods for calculating PSS, the imaging modalities used for modeling PSS, and the potential for refining treatment options and predicting future events based on PSS.

Summary: Carotid contrast-enhanced ultrasound (CEUS) has become increasingly important in evaluating carotid plaque, detecting surface irregularities and ulcerations, as well as intraplaque neovascularization and its alignment with histology. Initial indications suggest that CEUS might have the potential to predict cerebrovascular incidents, but further studies are needed to explore the relationship between CEUS observations and patient clinical outcomes in cases of carotid atherosclerotic disease.

Estimation of viscoelasticity of a carotid artery from ultrasound cine images and brachial pressure waveforms: Viscous parameters as a new index of detecting low plaque burden

Association between cumulative exposure to different lipid parameters and risk of newly developed carotid plaque

Summary: This study found that non-HDLC burden had the highest ORs for newly developed carotid plaque. TC burden, TG burden, LDLC burden, and non-HDLC burden are all independent risk factors, especially for vulnerable plaques. Among lipid parameters, non-HDLC burden is an optimal predictor.

Effects of norepinephrine on plaque hypoxia in atherosclerotic rabbits

Summary: This study investigated the effects of norepinephrine on the hypoxia of atherosclerotic plaques. The results showed that long-term administration of norepinephrine led to a decrease in plaque blood flow and an increase in the expression of hypoxia-related factors in the plaques, suggesting that norepinephrine induced plaque hypoxia by constricting the vasa vasorum.

Spectral analysis of ultrasound backscatter for non-invasive measurement of plaque composition

Summary: Carotid atherosclerotic plaque composition can predict the risk of future cerebrovascular events. This study explores the use of ultrasound spectral analysis and machine learning for noninvasive characterization of carotid plaque, focusing on the nonlinearly generated second harmonic portion of the backscatter signal. The results indicate that this approach is effective for tissue characterization and that a reference phantom with attenuation slope of 0.5 dB/cm-MHz works as well as more complex methods.

Management of Patients with Embolic Stroke of Unknown Source: Interpreting the Evidence in the Light of Clinical Judgement

Summary: The purpose of this review is to evaluate the validity of anticoagulation therapy for embolic stroke of unknown source (ESUS) and to assess the benefits and safety of direct-acting oral anticoagulants (DOACs). Recent findings suggest that the lack of benefit from anticoagulation may be due to misclassification of large artery atherosclerosis as ESUS. DOACs have important differences, and apixaban may be more effective and safer than rivaroxaban. Plaque burden should be considered in the definition of ESUS, and anticoagulation is more effective than antiplatelet agents for cardioembolic stroke.

Summary: This study evaluated the effectiveness and risks of lower versus higher doses of pioglitazone in patients with stroke. The results showed that lower doses of pioglitazone had fewer adverse effects while still providing substantial effects in preventing cardiovascular events and new-onset diabetes.

Carotid Vessel-Wall-Volume Ultrasound Measurement via a UNet plus plus Ensemble Algorithm Trained on Small Data Sets

Summary: In this study, a UNet++ ensemble approach was developed for automated vessel wall volume (VWV) measurement, trained on five small data sets and tested on 100 participants with coronary artery disease. The results showed that our approach could provide accurate and reproducible carotid VWV measurements using relatively small training data sets, supporting deep learning applications for monitoring atherosclerosis progression.

Ultrasound carotid plaque segmentation via image reconstruction-based self-supervised learning with limited training labels

Summary: This article proposes an image reconstruction-based self-supervised learning algorithm (IR-SSL) for carotid plaque segmentation when few labeled images are available. Through evaluation on two independent datasets, it was found that IR-SSL improved segmentation performance when trained on limited labeled images and showed strong correlation with manual results.
