Clinical and radiographic outcomes of allogeneic block grafts for maxillary lateral ridge augmentation: A randomized clinical trial

Background A main drawback of bone block graft surgery is the resorption occurring in early stages of healing. To our knowledge, there are no studies comparing outcomes of freeze-dried bone allograft (FDBA) blocks with different architecture. Purpose The aim of this work was to investigate different factors that can affect graft resorption and to compare the resorption rates of two different types of allogeneic blocks, corticocancellous and cancellous. Materials and Methods A randomized clinical trial was designed. Twenty-eight patients referred for onlay bone augmentation prior to implant placement were included in the study. Preoperative computerized tomography (CT) was taken for all patients. Patients received FDBA blocks of either cancellous or corticocancellous bone obtained from the iliac crest. After a 4-month follow-up, postoperative CT was taken. Then, another surgery was performed, with the purpose to place dental implants. The aforementioned groups were compared for bone resorption and implant outcome using analysis of covariance (ANCOVA) and repeated ANOVA measures, respectively. Demographic data, trabecular bone density, and graft sites were also analyzed. Results A total of 93 implants were placed in the augmented bone sites over 28 patients. A 100% survival rate was achieved during a mean follow-up period of 24 months in both groups. Higher bone resorption rate was found with cancellous bone grafts (29.2% +/- 2.6) compared with corticocancellous grafts (19.3% +/- 2.3). Moreover, higher resorption rates in patients with lower bone density (<185 Hounsfield Units) (31.7% +/- 3.1) and smokers (26.39% +/- 2.3) were observed when compared with patients with higher bone density (>185 Hounsfield Units) (16.8% +/- 2.1) and nonsmokers (22.1% +/- 2.3), respectively. Conclusion Within the limitations of this study, these findings indicate that both corticocancellous and cancellous FDBA grafts constitute a clinical acceptable alternative for bone reconstruction, although cancellous grafts present higher resorption rates. Moreover, host factors such as patient's low bone density and smoking habits may also increase graft resorption rates.

Salivary LDH in oral cancer and potentially malignant disorders: A systematic review and meta-analysis

Summary: This study evaluated the increase in salivary lactate dehydrogenase (LDH) levels in patients with oral cancer (OC) or oral potentially malignant disorders (OPMD) compared to a healthy control group (CG). Results showed that LDH levels were higher in the OC group and patients with oral leucoplakia and oral submucous fibrosis also had higher levels. Salivary LDH determination may be a useful method for screening and tracking OC and OPMD, but further studies are needed to establish precise cutoff values.

This technique helps maintain the structure of peri-implant tissues to prevent aesthetic compromise in the anterior maxilla.

Detection of Periapical Lesions on Panoramic Radiographs Using Deep Learning

Summary: This study aimed to develop an artificial intelligence technology to detect periapical lesions that dentists could fail to notice on panoramic radiographs. By annotating and classifying 18618 periapical root areas on 713 panoramic radiographs, a two-stage convolutional neural network model was trained. The model, consisting of a detector and a classifier, successfully localized and classified periapical lesions with an accuracy of 84.6%, sensitivity of 72.2%, and specificity of 85.6%.

Summary: This scoping review systematically maps the evidence supporting chronotherapy in dentistry and identifies knowledge gaps. It found that chrono-radiotherapy and chrono-chemotherapy can reduce treatment side effects and improve therapeutic response in cancer patients, leading to higher survival rates. Animal studies suggest that tooth movement and periodontal tissue response to orthodontic forces follow a diurnal rhythm that may impact bone metabolism. Profound and prolonged local anesthesia can be achieved when injected in the evening. Although the overall quality of the included studies was low, chronotherapy applications in dentistry appear to have favorable outcomes, especially in head and neck cancer treatments.

Electrochemical decontamination of titanium dental implants. An in vitro biofilm model study

Summary: The objective of this study was to investigate the effect of electrochemical treatment on biofilms developed on titanium dental implants. The study found that electrochemical treatment had an impact on the structure and bacterial composition of the biofilms. The reduction in bacterial counts, especially for Fusobacterium nucleatum, demonstrated a bactericidal effect of the treatment.

Patient-reported outcomes and framework fit accuracy of removable partial dentures fabricated using digital techniques: A systematic review and meta-analysis

Summary: The evidence on patient-reported outcomes and clinical performance of digitally fabricated RPDs compared to conventionally fabricated dentures is limited. Some studies suggest that digitally fabricated RPDs are associated with higher patient satisfaction, while others indicate that conventionally fabricated RPDs have better framework fit accuracy. Overall, both digitally and conventionally fabricated metal RPD frameworks showed clinically acceptable fit.

Importance of 3-dimensional imaging in the early diagnosis of chondroblastic osteosarcoma

Summary: The aim of this report is to present a case of chondroblastic osteosarcoma located in the right maxillary premolar region of a 17-year-old female patient. Different imaging techniques, including cone-beam computed tomography (CBCT), medical computed tomography (CT), positron emission tomography (PET), and magnetic resonance imaging (MRI), were used to diagnose and assess the extent of the tumor. Early diagnosis using 3-dimensional imaging can improve the prognosis of chondroblastic osteosarcoma.

Effect of time-dependent ibuprofen administration on the post operatory after impacted third molar extraction: a cross-over randomized controlled trial

Summary: The purpose of this study was to evaluate the time-dependent administration of ibuprofen in a lower third molar extraction model. The results showed that there were no significant differences in postoperative swelling and pain perception between the control group, which took ibuprofen every 8 hours, and the experimental group, which took ibuprofen only at breakfast and lunch and replaced dinner intake with a placebo. However, trismus was significantly higher in the experimental group at 72 hours postoperatively.
