Although there have been significant advances in the treatment of visceral leishmaniasis (VL) and several novel compounds are currently in pre-clinical and clinical development for this manifestation of leishmaniasis, there have been limited advances in drug research and development (R & D) for cutaneous leishmaniasis (CL). Here we review the need for new treatments for CL, describe in vitro and in vivo assays, models and approaches taken over the past decade to establish a pathway for the discovery, and pre-clinical development of new drugs for CL. These recent advances include novel mouse models of infection using bioluminescent Leishmania, the introduction of PK/PD approaches to skin infection, and defined pre-clinical candidate profiles.

The oral protease inhibitor (PF-07321332) protects Syrian hamsters against infection with SARS-CoV-2 variants of concern

Summary: There is an urgent need for potent and selective antivirals against SARS-CoV-2. Pfizer has developed PF-07321332 (PF-332), a powerful inhibitor of the viral main protease (Mpro, 3CLpro), which can be taken orally and is currently in clinical development. In this study, it was found that PF-332 has equal in vitro activity against the four SARS-CoV-2 variants of concern and can completely halt replication of the alpha variant in human airway epithelial cells. The treatment with PF-332 protected Syrian Golden hamsters against infection with the beta and delta variants and prevented transmission of the SARS-CoV-2 (B.1.617.2) variant to untreated co-housed sentinels.

Summary: In acute malaria, the loss of erythrocytes primarily occurs after therapy, leading to anemia. The mechanisms behind this post-treatment anemia are still unknown. This study investigated the changes in RBC subpopulations during severe malaria treated with artemisinin derivatives. The findings showed decreased deformability of RBC, and the loss of RBC was mainly attributed to the elimination of uninfected RBC.

Immune recovery-related patterns of post kala-azar dermal and ocular leishmaniasis in people living with HIV

Summary: This study reported cases of post kala-azar dermal leishmaniasis (PKDL) in people living with HIV (PLHIV) and compared the characteristics of two groups based on whether PKDL occurred in the context of immune recovery under antiretroviral therapy (ART) or not. The study found that PKDL can occur as part of an immune restoration disease under ART or as a relapse of visceral leishmaniasis in the context of AIDS. Comparisons between the two groups showed that patients in the immune recovery group had higher CD4(+) cell counts, lower rates of concomitant visceral leishmaniasis, and a trend towards more frequent ocular involvement.

Summary: The Fulani ethnic group has a higher deformability of circulating erythrocytes compared to non-Fulani, indicating enhanced clearance of rigid erythrocytes by the spleen. This phenotype is observed in individuals with markers of Plasmodium falciparum infection. The heritability of this trait is high with a strong multigenic component, and genes expressed in the spleen may be involved in the splenic clearance of erythrocytes.

Treatment outcome of imported cutaneous leishmaniasis among travelers and migrants infected with Leishmania major and Leishmania tropica: a retrospective study in European centers 2013 to 2019

Summary: This article describes and evaluates the treatment outcomes of cutaneous leishmaniasis (CL) among travelers and migrants in Europe caused by Leishmania major and Leishmania tropica. The study found that L. major infections were mostly found in individuals visiting friends and relatives, while L. tropica infections were mainly identified in migrants. Pentavalent antimony had a cure rate of 73% (L. major) and 78% (L. tropica) among patients.

Miltefosine and Nifuratel Combination: A Promising Therapy for the Treatment of Leishmania donovani Visceral Leishmaniasis

Summary: Visceral leishmaniasis is a neglected tropical disease caused by Leishmania parasites, which is prevalent in East Africa, Asia, South America, and the Mediterranean Basin. The current pharmacological control of the disease is limited by the availability of old, poorly adherent drugs with adverse effects and low oral bioavailability. Resistance to first-line drugs has emerged due to misuse. However, recent research has identified nifuratel, a drug used for vaginal infections, as a promising candidate for repurposing due to its efficacy against leishmaniasis.

Summary: Probing multiple proprietary pharmaceutical libraries in parallel through virtual screening led to the rapid expansion of structure-activity relationship (SAR) around hit compounds with moderate efficacy against Trypanosoma cruzi. The SAR was further developed through a potency-improving scaffold hop and design guided by phenotypic virtual screening, resulting in the identification of two promising hit compounds 54 and 85. Compound 85 showed a significant reduction in parasitemia in an in vivo model, confirming its potential as an anti-trypanosome treatment.

Susceptibility to malaria in fulani, Bariba, Otamari and gando individuals living in sympatry in Benin: Role of opsonizing antibodies to Plasmodium falciparum merozoites

Summary: This study explores a potential mechanism of natural protection against malaria in the Fulani population, suggesting that antibody-mediated merozoite phagocytosis may play a role. Results show that adults from the Bariba, Otamari, and Gando groups have higher levels of opsonic phagocytosis compared to children, while the Fulani population shows the opposite trend. Fulani children have higher levels of opsonic phagocytosis and anti-GLURP antibodies compared to non-Fulani children, with no significant difference observed in Fulani adults. Higher levels of opsonic phagocytosis before the malaria transmission season are associated with negative malaria Rapid Diagnostic Test results after the season.

Further Investigations of Nitroheterocyclic Compounds as Potential Antikinetoplastid Drug Candidates

Summary: Due to the lack of specific vaccines, neglected tropical diseases caused by trypanosomatids, such as sleeping sickness, Chagas disease and leishmaniasis, rely solely on pharmacological treatments. The current drugs have drawbacks including adverse effects, parenteral administration, chemical instability, and high costs. However, the discovery of new drugs for these diseases is rare due to the lack of interest from big pharmaceutical companies. Nitroheterocyclic compounds, such as benznidazole and nifurtimox, have provided potent effects against Chagas disease, and fexinidazole has been developed as a new drug against African trypanosomiasis. Despite being initially discarded due to their mutagenic potential, nitroheterocycles now show promise as a source of inspiration for developing oral drugs to replace existing ones.

Gel-Free Tools for Quick and Simple Screening of Anti-Topoisomerase 1 Compounds

Summary: With the increasing demand for effective compounds against cancer and pathogen-borne diseases, new tools for investigating the enzymatic activity of biomarkers are necessary. A rolling circle amplification-based method has been developed for fast and easy screening of compounds against type 1 topoisomerases. These tools have proven to be sensitive and directly quantitative, opening up possibilities for new diagnostic and drug screening protocols in research and clinical settings.
