Investigating cytokines in tear fluid and saliva may offer valuable information for understanding the pathogenesis of primary Sjogren's syndrome (pSS). Cytokine profiles in both tear fluid and saliva of pSS patients, non-Sjogren's syndrome (non-SS) subjects with sicca symptoms, and healthy controls without sicca complaints were analysed. Furthermore, relationships associating the severity of clinical ocular and oral manifestations with the upregulated cytokines were assessed. In tear fluid, pSS patients showed elevated levels of IL-1ra, IL-2, IL-4, IL-8, IL-12p70, IL-17A, IFN-gamma, IP-10, MIP-1b, and Rantes compared to non-SS subjects and healthy controls. The increased cytokine levels (except IP-10) correlated significantly with reduced tear production, less stable tear film, and greater ocular surface damage. In saliva, pSS patients had a higher IP-10 level, which correlated with higher candida score; and an elevated MIP-1a level, which correlated significantly with lower unstimulated and stimulated whole saliva secretion rates. The upregulated cytokines identified in tear fluid and saliva of pSS patients show a clear interplay between innate and adaptive immune responses that may contribute to disease pathogenesis. The increase of IP-10 and MIP in both tears and saliva further emphasises the essential role of macrophages and innate immunity in pSS.

Xerostomia and hyposalivation among a 65-yr-old population living in Oslo, Norway

Summary: The study found that xerostomia and hyposalivation were significantly associated with factors such as medication use, diseases, and radiation therapy. Although these conditions were not prevalent in the population, clinicians should pay special attention to the salivary conditions of patients taking multiple medications, diagnosed with type II diabetes, and who have received radiation therapy to the head/neck region.

Summary: The study revealed significant differences in the lipidomic profiles of saliva and tears in pSS patients compared to healthy controls, with individual lipid species in stimulated saliva showing similarities to glandular biopsies. This opens up intriguing avenues for further research and may lead to innovative diagnostic methodologies and treatment modalities for alleviating pSS-related sicca symptoms.

Summary: Sjogren's syndrome is a complex autoimmune disease associated with lymphocytic infiltration and secretory dysfunction of salivary and lacrimal glands. Molecular changes in damaged salivary epithelia of SS patients may offer insights into the signals contributing to SS development and progression.

Summary: In this study, meibography of 65-year-old residents in Oslo, Norway was analyzed, and it was found that moderate meibomian gland atrophy was common and not gender-specific. Meibography alone cannot discriminate between meibomian gland dysfunction (MGD) and non-MGD, thus both morphological and functional tests are necessary for screening MGD.

Summary: Through metabolomics analysis, we found different metabolic profiles in patients with dry mouth conditions caused by head and neck cancer and primary Sjogren's syndrome. The dysregulation of purinergic signaling and amino acid metabolism may play a role in the pathophysiology of dry mouth. This finding is important for diagnosis and understanding the underlying mechanisms of dry mouth.

Summary: Nanotechnology has found unique applications in drug delivery, cosmetics, fabrics, water treatment, and food packaging. Chitosan, a nanoparticle derived from natural polysaccharide chitin, has gained attention in modern medicine for its unique properties. This review examines the cytotoxicity of chitosan nanoparticles in biomedical research, revealing low cytotoxicity regardless of particle composition or assay method. However, thorough characterization and cytotoxicity assessment are recommended for new chitosan derivatives and compositions before market implementation.

Summary: The present study explored the relationship between dry eyes and dry mouth and found that participants with a history of systemic diseases had more ocular and oral symptoms. There was a correlation between subjective ocular and oral parameters, and a significant correlation was also observed between the total number of drugs and ocular and oral symptoms. Participants troubled with severe dry eyes had poorer values for oral parameters.

Summary: The clinical presentation of primary Sjogren's syndrome (pSS) varies widely. A multicentre study identified two patient subgroups based on the presence of SSA/SSB antibodies and genetic markers in the HLA region, with strong association signals found in HLA-DQA1 locus for the antibody-positive subgroup. Replication confirmed the association with MHC class I and II variants, indicating distinct clinical manifestations in pSS subgroups.
