Safety of on- vs off-clamp robotic partial nephrectomy: per-protocol analysis from the data of the CLOCK randomized trial

Purpose To compare the safety of on- vs off-clamp robotic partial nephrectomy (RAPN). Methods 302 patients with RENAL masses <= 10 were randomized to undergo on-clamp (150) vs off-clamp (152) RAPN (CLOCK trial-ClinicalTrials.gov NCT02287987) at seven institutions by one experienced surgeon per institution. Intra-operative data, complications, and positive surgical margins were compared. Results Due to a relevant rate of shift from the assigned treatment, the per-protocol analysis only was considered and the data from 129 on-clamp vs 91 off-clamp RAPNs analyzed. Tumor size (off-clamp vs on-clamp, 2.2 vs 3.0 cm, p < 0.001) and RENAL score (5 vs 6, p < 0.001) significantly differed. At univariate analysis, no differences were found regarding intra-operative estimated blood loss (off- vs on-clamp, 100 vs 100 ml, p = 0.7), post-operative complications rate (19% vs 26%, p = 0.2), post-operative anemia (Hb decrease > 2.5 g/dl 26% vs 27%, p = 0.9; transfusion rate 3.4% vs 6.3%, p = 0.5; re-intervention due to bleeding 1.1% vs 4%, p = 0.4), acute kidney injury (4% vs 6%, p = 0.8), and positive surgical margins (3.5% vs 8.2%, p = 0.1). At multivariate analysis accounting for tumor diameter and complexity, considering the on-clamp group as the reference category, a significant difference was noted in the off-clamp group exclusively for blood loss (OR 0.3, 95% CI 0.09-0.52, p = 0.008). Conclusions The on-clamp and off-clamp approaches for RAPN showed a comparable safety profile.

Graves? disease induced by IFN-?1a therapy: A case report, review of literature and new insights into the pathogenesis

Summary: Since 1997, it has been known that Interferon (IFN)-beta 1b therapy can induce autoimmune hyperthyroidism, especially in patients with preexisting thyroid autoimmune disorders (AITD). In this case study, a 60-year-old female with a history of euthyroid autoimmune thyroiditis and Multiple Sclerosis (MS) developed hyperthyroidism and Graves' ophthalmopathy (GO) after receiving IFN-beta 1a therapy. This is the first reported case of GD and ophthalmopathy associated with IFN-beta 1a treatment in a patient with MS, providing new insights into GD immunopathogenesis.

Summary: Collecting duct carcinoma (CDC) is more aggressive than clear cell renal cell carcinoma (ccRCC). In this study, we compared cancer specific mortality rates between CDC and ISUP 4 ccRCC histological subtypes, and found that CDC patients had a survival disadvantage even after adjusting for various factors.

Summary: This study examined the impact of disease-free interval (DFI) duration on cancer-specific mortality (CSM)-free survival in surgically treated adrenocortical carcinoma (ACC) patients. The results showed that longer DFI duration was associated with improved survival probabilities, especially in advanced stage patients. Considering DFI may be valuable in patient counseling.

Robotic Off-Clamp Simple Enucleation Single-Layer Renorrhaphy Partial Nephrectomy (ROSS): Surgical Insights after an Initial Experience

Summary: Robotic technology allows beginner surgeons to perform minimally-invasive partial nephrectomy with a shorter learning curve compared to pure laparoscopy. This video-article presents the surgical technique and outcomes of the first 11 cases performed by a young surgeon using robotic PN. The off-clamp approach and simple enucleation technique were successfully employed, yielding comparable results to existing literature.

Summary: This study aimed to evaluate the feasibility of radical cystectomy (RC) in patients with a history of previous radical prostatectomy (RP) for prostate cancer (PCa), and investigate the surgical outcomes including morbidity, operative time (OT), and length of stay (LOS). The results showed that RC in patients with a history of RP is feasible but associated with remarkable morbidity, especially in patients who received radiotherapy (RT) after RP.

Summary: This study evaluated the influence of endogenous testosterone density (ETD) and tumor load density (TLD) in the surgical specimen of prostate cancer patients. The results showed a positive association between ETD and TLD, with increasing ETD levels being associated with a higher risk of high TLD. This suggests that at the diagnosis of prostate cancer, ETD is positively associated with the risk of high TLD, which may adversely affect the natural history of prostate cancer.

Results showed that male patients were more frequent in the northern region compared to the central and southern regions. The southern region had a higher prevalence of comorbidities such as diabetes, hypertension, chronic pulmonary and kidney diseases.
