Patients who carry the BRCA1 and BRCA2 gene mutations have an underlying genetic predisposition for breast and ovarian cancers. These deleterious genetic mutations are the most common genes implicated in hereditary breast and ovarian cancers. This monograph summarizes the evidence behind current screening recommendations, reviews imaging protocols specific to this patient population, and illustrates some of the imaging nuances of breast and ovarian cancers in this clinical setting. Summary: The O-RADS ultrasound and MRI risk stratification systems were developed to aid radiologists in assessing adnexal lesions. Both modalities can accurately characterize benign lesions and provide additional information for further management. MRI plays a crucial role in determining the risk of invasive cancer and guiding personalized treatment options.

Summary: Laparoscopic myomectomy, a common gynecologic operation, has been regulated since 2014 due to the discovery of a higher prevalence of uterine leiomyosarcoma (LMS). Gynecologic surgeons are in need of an evidence-based, noninvasive evaluation for LMS to avoid the dissemination of aggressive malignant neoplasms.

Summary: Transgender and gender diverse (TGD) individuals face health disparities and often avoid necessary medical care due to fears of discrimination or mistreatment. The medical community's limited understanding of gender-affirming hormone therapy (GAHT) and gender-affirming surgery further exacerbates disparate care. Specifically in radiology, TGD patients report more negative imaging experiences compared to general health encounters, indicating a need for inclusive imaging care practices. This article highlights opportunities for improvement in radiology departments, including staff education, welcoming environments, trauma-informed care, and appropriate reporting considerations for TGD patients.
