Background Paramedics need to be in good physical health to be able to respond to emergencies. We hypothesized that near misses by paramedics and unanticipated incidents that cause no harm may be associated with presenteeism during emergency rescues. Aims To examine the association between presenteeism and near misses among paramedics. Methods A cross-sectional study was conducted among 254 paramedics using a self-administered questionnaire. Presenteeism was measured using the work functioning impairment scale (WFun). Results Twelve per cent of respondents experienced high work function impairment indicating presenteeism. The rate of near-miss experiences steadily increased with increasing work function impairment among paramedics (WFun score category: middle, odds ratio (OR) 3.62, 95% confidence interval (CI) 1.62-8.11; high, OR 4.36, 95% CI 1.37-13.84; very high, OR 8.62, 95% CI 2.09-35.48). Conclusions This study demonstrated a positive relationship between impaired work function indicating presenteeism and near misses among paramedics. This suggests that presenteeism may be associated with errors that have the potential to cause serious accidents. Improved access to treatment for frequent health problems may reduce the number of incidents among paramedics.

Summary: This study evaluated the mortality risk in elderly hip fracture patients, focusing on comorbidities and nursing care levels. By using a combined database of medical and long-term care insurance claims data in Japan, a total of 6125 patients aged 65 years and older with hip fracture were selected. The study found that high nursing care level and comorbidities such as malignant diseases, heart diseases, pneumonia, chronic obstructive pulmonary disease, renal failure, and dementia increased mortality in elderly hip fracture patients.

Summary: It is important to understand the basic epidemiology of mitochondrial diseases in Japan, including the number of affected patients. A study using a national healthcare claims database estimated that there were 3,629 patients with mitochondrial diseases from April 2018 to March 2019, with a prevalence of 2.9 per 100,000 population. This suggests that about half of all patients might be overlooked, indicating the need for additional support in the Japanese healthcare system.

Summary: This study aimed to investigate the association between telecommuting environments and shoulder pain in Japanese telecommuters. An Internet-monitored, cross-sectional study was conducted with 2537 participants who mostly telecommuted at least 4 days per week. The presence of shoulder pain was assessed using a numerical rating scale, and the characteristics of the telecommuting environment were evaluated through eight questions. Multilevel logistic regression analysis revealed that inadequate desk brightness, inadequate foot space, and uncomfortable temperature and humidity were associated with shoulder pain.

Arisa Harada, Seiichiro Tateishi, Tomohiro Ishimaru, Masako Nagata, Hisashi Eguchi, Mayumi Tsuji, Kazunori Ikegami, Ryutaro Matsugaki, Yoshihisa Fujino

Summary: This study examined the relationship between the type of chronic disease and the provision of workplace accommodations. A cross-sectional study in Japan involving 6775 full-time workers who needed work accommodations was conducted. The findings revealed that workers with cancer, mental illness, and benign gynecological disorders were more likely to receive workplace accommodations, while those with low back pain and skin diseases were less likely. Therefore, the provision of workplace accommodations depends on the type of chronic disease, and those with inadequate accommodations should be supported.

Summary: This study aimed to identify factors associated with the acceptance of the COCOA app for COVID-19 in the working-age population in Japan. The study found that married individuals, those with a university degree or higher, higher income, and desk jobs were more likely to adopt the COCOA app. Fear of COVID-19 transmission, mask usage, hand disinfection, willingness to be vaccinated, and knowing someone infected with COVID-19 were also associated with a higher likelihood of app adoption. These findings highlight the importance of promoting digital contact tracing apps among working-age populations in Japan and beyond.

Summary: This study aimed to investigate the effects of biological/target synthesized DMARDs (b/tsDMARDs) on bone metabolism in patients with rheumatoid arthritis (RA). The results showed that without anti-osteoporotic treatment, b/tsDMARDs treatment could lead to the progression of osteoporosis in RA patients. Bone turnover markers (BTMs) may not be able to reflect changes in bone mineral density (BMD). Therefore, regular monitoring of BMD in RA patients is recommended for early management of osteoporosis.
