Acute myeloid leukemia (AML) is the most common form of acute leukemia among adults and accounts for the largest number of annual deaths due to leukemias in the United States. Recent advances have resulted in an expansion of treatment options for AML, especially concerning targeted therapies and low-intensity regimens. This portion of the NCCN Clinical Practice Guidelines in Oncology (NCCN Guidelines) for AML focuses on the management of AML and provides recommendations on the workup, diagnostic evaluation and treatment options for younger (age <60 years) and older (age >= 60 years) adult patients.

Use of Opioids for Adults With Pain From Cancer or Cancer Treatment: ASCO Guideline

Summary: The purpose of this article is to provide guidance on the use of opioids for managing cancer-related pain in adults. The authors conducted a systematic review of the literature and identified the efficacy and safety of opioid analgesics, approaches to opioid initiation and titration, and strategies for preventing and managing opioid adverse events. The findings indicate that opioids effectively reduce pain in patients with moderate-to-severe cancer pain, but there is limited evidence for some questions of interest, leading to consensus-based recommendations or the absence of recommendations.

Patient-Reported Distress and Clinical Outcomes with Immuno-Oncology Agents in Metastatic Non-Small Cell Lung Cancer (mNSCLC): A Real-World Retrospective Cohort Study

Summary: This study provides real-world data on patient-reported outcomes in metastatic non-small cell lung cancer (mNSCLC) with immunotherapies and chemotherapy. The findings suggest that patients experience significant distress before starting treatment, and immunotherapy is associated with higher clinical response rates and lower healthcare utilization compared to chemotherapy. The study also highlights potential unmet palliative care needs in mNSCLC patients receiving immunotherapy.

More than conquerors: a qualitative analysis of war metaphors for patients with cancer

Summary: The study aimed to understand the impact and function of war metaphors among cancer patients. Through semi-structured interviews, it was found that most non-metastatic cancer patients used war metaphors to describe their cancer experience, which facilitated meaning-making and provided a larger context for their illness. However, metastatic cancer patients found war metaphors unhelpful due to their lack of control. Hence, the war metaphor should remain an integral part of cancer care.

Survey of patients and physicians on shared decision-making in treatment selection in relapsed/refractory multiple myeloma

Summary: Shared decision-making (SDM) is a crucial aspect of patient-centered healthcare, particularly in the relapsed and/or refractory multiple myeloma (RRMM) setting where treatment options can be challenging. However, the extent and relevance of SDM and patient-centered communication (PCC) in RRMM have not been extensively studied. In order to examine treatment decision-making patterns, online surveys were conducted with 200 patients and 200 physicians in the USA. Both physicians and patients prioritize extending overall survival and progression-free survival as treatment goals. While some physicians doubt patients' preference for a shared approach, patients express a desire for shared decision-making. Discrepancies in the perceptions of physicians and patients indicate a need for communication tools to facilitate SDM and PCC.

Summary: Caregivers of patients with hematologic malignancies undergoing HSCT face significant caregiving burden, psychological distress, and diminished QOL. Our study found that approach-oriented coping strategies were associated with less anxiety, depression symptoms, and better QOL, while avoidant coping strategies were associated with more anxiety, depression symptoms, and worse QOL.

An Analysis of Dasatinib Treatment Patterns in Patients with Chronic Myeloid Leukemia After Experiencing Pleural Effusion During Dasatinib Therapy

Summary: Treatment with dasatinib for CML may lead to pleural effusion, but the optimal management approach is unclear. In this study, we found that not all patients required dose modification after pleural effusion, and those with dose modifications had longer treatment duration and lower rate of switching to other targeted therapies compared to those with stable dose.

Summary: This study reported the final phase Ib data of magrolimab and azacitidine in patients with untreated higher-risk MDS. The results showed that the combination therapy of magrolimab and azacitidine promotes macrophage-mediated phagocytosis of tumor cells and has synergistic effects. The study demonstrates the good safety and efficacy of this combination therapy in treating higher-risk MDS patients.

Isavuconazonium or posaconazole for antifungal prophylaxis in patients with acute myeloid leukemia

Summary: This study evaluated the use of ISAV prophylaxis for primary IFI prevention in AML patients and compared it to POSA. The results showed no significant difference in the efficacy of the two drugs for prevention. Patient-specific factors should influence the choice of prophylactic agent.

Summary: Palliative care benefits patients undergoing hematopoietic stem cell transplantation but is underutilized. We conducted a survey to assess HSCT recipients' familiarity, knowledge, and perception of PC, as well as their unmet PC needs. Patients who were more knowledgeable about PC were more likely to have positive perceptions of PC.

Innovations for the integration of palliative care for hematologic malignancies

Summary: Specialist palliative care can improve symptom burden, mood, and quality of life for patients with hematologic malignancies. However, these patients continue to have unmet palliative care needs. This paper discusses the benefits of specialist palliative care, the unmet needs of this patient population, and the barriers to integrating palliative care and oncologic care. It also explores innovations and areas of future research to enhance and optimize palliative care integration for patients with hematologic malignancies.

In their own words: a qualitative study of coping mechanisms employed by patients with acute myeloid leukemia

Summary: This study aims to explore the mediating role of positive coping mechanisms in the improved outcomes of integrated palliative care for acute myeloid leukemia (AML). Qualitative analysis of interviews with high-risk AML patients revealed various forms of approach-oriented coping, such as acceptance, positive reframing, active coping, religious coping, and social coping. However, some patients also demonstrated avoidant coping strategies, including denial, behavioral disengagement, and self-blame.
