Minimally Invasive Myomectomy: An Overview on the Surgical Approaches and a Comparison with Mini-Laparotomy

Uterine fibroids are common benign tumors in the female genital tract, often managed through minimally invasive surgical techniques. Myomectomy is the standard surgical treatment for preserving fertility, and various minimally invasive options can be chosen based on the characteristics of the fibroids and the surgeon's skill level. A less invasive surgical approach known as mini-laparotomy has also been proposed as an alternative to established minimally invasive options. This review aims to discuss recent advances in minimally invasive techniques for myomectomy and compare them with the mini-laparotomy approach.
Uterine fibroids (UFs) are the most common benign solid tumors of the female genital tract manageable by surgical of pharmacological approach. When the medical management is ineffective or surgery is primarily requested, several surgical approaches can be used. Among these, minimally invasive surgery might be preferred. Myomectomy is the standard surgical treatment when fertility sparing is claimed. It can be performed via laparoscopy, robotic surgery and hysteroscopy and the choice depend on UFs features and surgeon's skill. Alongside these minimally invasive options, mini-laparotomy has been proposed as a less invasive surgical approach comparable to the well-established minimally invasive options. The aim of this review is to describe the most recent advances in minimally invasive techniques to perform myomectomy, comparing them with mini-laparotomy approach.

The Impact of a Web-Based Lifestyle Educational Program ('Living Better') Reintervention on Hypertensive Overweight or Obese Patients

Parenchymal liver metastasis in advanced ovarian cancer: Can bowel involvement influence the frequency and the related mortality rate?

Summary: This retrospective study estimates the frequency of parenchymal liver metastasis (PLM) and the overall survival (OS) rate of patients with advanced epithelial ovarian cancer (EOC) with bowel involvement. The results show that PLM occurred in 33.0% of the cases, with an average onset time of 13 months. PLM was associated with increased mortality risk and a significantly shorter OS.

SUCCOR Nodes: May Sentinel Node Biopsy Determine the Need for Adjuvant Treatment?

Summary: This study compared the use of adjuvant therapy in women with stage IB1 cervical cancer based on different methods for diagnosing lymph node metastasis. The results showed that patients who underwent sentinel node biopsy + lymphadenectomy (SNB+LA) were less likely to receive adjuvant therapy and had higher risks of relapse and death compared to those who underwent lymphadenectomy alone (LA).

Towards reproducible research in recurrent pregnancy loss immunology: Learning from cancer microenvironment deconvolution

Summary: The most recent international guidelines on recurrent pregnancy loss (RPL) do not recommend immunological tests due to lack of evidence-based support. The ambiguity in RPL definition, etiological factors, diagnostic work-up, and treatments hampers metanalysis and systematic reviews. The heterogeneity in cohorts, inadequate sample size, and data quality contribute to the non-standardized research quality in the RPL field, particularly in immunological research.

Outpatient Hysteroscopic Polypectomy-A Retrospective Study Comparing Rigid and Semirigid Office Hysteroscopes

Summary: Endometrial polyps are a common gynecological condition and hysteroscopy is the standard diagnostic and treatment procedure. This study compared pain perception in patients undergoing hysteroscopic endometrial polypectomy with different hysteroscopes. The results showed that the use of a semirigid hysteroscope led to significantly greater pain. Cervical stenosis and menopausal status were identified as risk factors for pain. The study suggests that rigid instruments may be better tolerated for this procedure.

Biomechanical analysis of breastfeeding positions and their effects on lumbopelvic curvatures and lumbar muscle responses

Summary: This study aimed to analyze the position and muscle activity of the lumbopelvic region and lumbar muscles during breastfeeding. The results showed that side lying and clutch-hold positions led to more flexion of the lumbar spine compared to standing. Sitting postures caused retroversion of the pelvis compared to standing and side lying. Muscle activity analysis revealed that side lying position resulted in lower activation intensity of the right erector muscle compared to other breastfeeding positions.

Patterns of recurrence in FIGO stage IB1-IB2 cervical cancer: Comparison between minimally invasive and abdominal radical hysterectomy

Summary: The objective of this study was to evaluate whether different surgical approaches in FIGO stage IB1-IB2 cervical cancer led to different patterns of disease recurrence. The results showed that minimally invasive surgery was not associated with different recurrence patterns compared to abdominal radical hysterectomy, and there was no higher risk of distant metastasis or significant difference in terms of disease-free survival (DFS) and overall survival (OS).

It is time to implement molecular classification in endometrial cancer

Summary: A significant effort has been made to redefine the risk classes of endometrial cancer (EC) in recent years, but the current prognostic factors and biomolecular classification are insufficient in predicting outcomes and recurrences. Although biomolecular classification has improved the risk assessment and treatment selection, it fails to fully explain the differences in recurrence profiles. There is also a lack of evidence in EC guidelines. This article summarizes the reasons why molecular classification alone is not enough in managing endometrial cancer and highlights some innovative examples with potential clinical significance in scientific literature studies.

However, their adherence to the Mediterranean diet did not change significantly.
