Coronary perforation complicating percutaneous coronary intervention in patients presenting with an acute coronary syndrome: An analysis of 1013 perforation cases from the British Cardiovascular Intervention Society database

Background: The evidence base for coronary perforation occurring during percutaneous coronary intervention in patients presenting with an acute coronary syndrome (ACS-PCI) is limited and the specific role of acute pharmacology in its clinical presentation unclear. Methods and results: Using the BCIS PCI database, data were analysed on all ACS-PCI procedures performed in England and Wales between 2007 and 2014. Multiple regressions were used to identify predictors of coronary perforation and its association with outcomes. Propensity score matching was used to evaluate the association between differing P2Y12 inhibitors or glycoprotein inhibitors (GPI) and CP. During 270,329 ACS-PCI procedures, 1013 coronary perforations were recorded (0.37%) with a stable annual incidence. In multiple regression analysis, covariates associated with increased frequency of coronary perforation included age, female gender, CTO intervention, number and length of stents used, and rotational atherectomy use, whilst differing P2Y12 inhibitors were not predictive. Using propensity score matching, use of a GPI was independently associated with tamponade (OR 1.50, [1.08-2.06], p = 0.014). The adjusted odds ratios for all clinical outcomes were adversely affected by coronary perforation. Conclusions: Coronary perforation is an infrequent event during ACS-PCI but is closely associated with adverse clinical outcomes. GPI use was associated with higher rates of tamponade. Temporal trends in disease-specific causes of cardiovascular mortality amongst patients with cancer in the USA between 1999 and 2019

Summary: This study reports disease-specific cardiovascular causes of mortality among cancer patients in the USA between 1999 and 2019, considering temporal trends by age, sex, and cancer site. Ischaemic heart disease was the most common cardiovascular cause of death across all cancer types, followed by cerebrovascular disease and hypertensive diseases. Differential cardiovascular mortality risk was observed based on cancer site and demographics.

Impact of pre-existing vascular disease on clinical outcomes

Summary: Little is known about the care outcomes and processes of patients with non-ST-segment myocardial infarction (NSTEMI) who have polyvascular disease. A study analyzed 287,279 NSTEMI patients and found that those with polyvascular disease had higher rates of major adverse cardiovascular events and in-hospital mortality. Patients without a history of vascular disease were less likely to receive statins or ACE inhibitors/ARBs, but more likely to receive dual antiplatelet therapy.

Examining non-linearity in the association between age and reported opioid use in different socioeconomic strata: cohort study using Health Survey for England waves from 1997 to 2014

Summary: This study used data from the Health Survey for England waves 1997-2014 to analyze the relationship between age and prescription opioid use in different socioeconomic groups. The findings showed that the association between age and opioid use varies across income and education levels. Higher income and education were associated with a lower probability of reported opioid use. The highest probability of opioid use was observed in the 40-60 age group among those with lower income and education. Employment status did not significantly affect opioid use. Further research is needed to understand opioid use patterns in older people, especially those from disadvantaged socioeconomic backgrounds.

Use of locum doctors in NHS trusts in England: analysis of routinely collected workforce data 2019-2021

Summary: This study aimed to quantify and describe the use of locum doctors in NHS trusts in England from 2019 to 2021. The results showed significant variations in locum use among different trusts, with lower-rated and smaller trusts using locums more intensively. The study also highlighted a growing demand for locum doctors, as evidenced by the increasing number of unfilled shifts.

Summary: This study examines whether adults referred to the NHS Healthier You Diabetes Prevention Programme in England are less likely to develop type 2 diabetes mellitus. The findings show that people referred to the programme have a 20% lower risk of developing type 2 diabetes compared to those who were not referred. These results support the rapid implementation of the programme in England and suggest its continuation and expansion to other parts of the United Kingdom.

Summary: The incidence of NSTEACS in young patients under fifty in the UK has not decreased over the past seven years. Despite a reduction in smoking and family history of coronary artery disease, there has been an increase in the proportion of patients from ethnic minority backgrounds, those with diabetes, and female patients. In-hospital mortality from NSTEACS remains unchanged.

Prevalence and outcomes of type 2 myocardial infarction in patients with cancer: A retrospective analysis from the National Inpatient Sample dataset

Summary: This study investigated the prevalence, clinical characteristics, and outcomes of type 2 myocardial infarction (T2AMI) in patients with and without cancer. The results showed that T2AMI patients with cancer had a higher incidence of acute respiratory failure, acute pulmonary embolism, major bleeding, and renal failure compared to those without cancer. They also had higher rates of in-hospital all-cause death.
