Risk of impaired nutritional status and flare occurrence in IBD outpatients

Background: Inflammatory bowel disease (IBD) patients are at risk of an impaired nutritional status. The impact thereof on the IBD relapse risk is clinically relevant, though sparsely investigated. Aim: The aim was to explore the association between an impaired nutritional status risk and the occurrence of disease flares in IBD outpatients participating in a longitudinal telemedicine study. Methods: IBD outpatients were recruited from the myIBDcoach study cohort, with one year clinical follow-up. Through myIBDcoach, a telemedicine tool, patients reported on disease activity and risk of impaired nutritional status (i.e. Short Nutritional Assessment Questionnaire > 1 and/or BMI < 18.5 kg/m(2)) every one to three months. Data was analysed by generalized estimating equation modelling. Results: In total, 417 patients were included. During follow-up, 49 patients (11.8%) flared after initial clinical remission and 53 patients (12.7%) showed an increased risk of impaired nutritional status. The risk of impaired nutritional status was associated with flare occurrence (OR 2.61 (95% CI 1.02-6.69)). Conclusions: The risk of an impaired nutritional status was associated with subsequent flares in IBD outpatients. This emphasizes the importance of monitoring disease activity in IBD patients at risk of impaired nutritional status. (C) 2019 Editrice Gastroenterologica Italiana S.r.l. Published by Elsevier Ltd. Summary: The aim of this study was to evaluate the effectiveness and safety of 24 months of tofacitinib use in patients with ulcerative colitis in the Netherlands. The results showed that 31.8% of patients achieved corticosteroid-free clinical remission after 24 months of tofacitinib treatment. The main side effects were skin reactions and headaches.

Corneal Transplantation for Infectious Keratitis: A Prospective Dutch Registry Study

Summary: This study analyzed real-world practice patterns and graft survival after corneal transplantation for infectious keratitis in the Netherlands. The most common indication for transplantation was viral keratitis, followed by bacterial and Acanthamoeba keratitis. HLA matching did not provide a significant survival benefit, while emergency procedures and grafts sized >8.5 mm had poor survival. Acanthamoeba had a worse prognosis in therapeutic keratoplasty compared to viral and bacterial keratitis.

Intra and interobserver agreement of the Dynamic Imaging Grade of Swallowing Toxicity Scale (DIGEST) in fiberoptic endoscopic evaluation of swallowing (FEES): the importance of observer-tailored training

Summary: This study described the training process of observers for the Dynamic Imaging Grade of Swallowing Toxicity (DIGEST) scale in fiberoptic endoscopic evaluation of swallowing (FEES), determined observer agreement on DIGEST in FEES, explored the effect of bolus consistency on observer agreement, and explored criterion validity of DIGEST in FEES. The study found that observer agreement on DIGEST improved after specific training, and bolus consistency had an effect on observer agreement. A manual with well-defined descriptions can optimize the reproducibility of DIGEST measurements.

Summary: Computer-aided diagnosis systems (CADx) can improve colorectal polyp (CRP) optical diagnosis. An explainable AI CADx was developed to automatically generate textual descriptions of CRPs, and its performance was analyzed using BLI images of 55 CRPs. The CADx showed high performance for surface descriptors, while the description of size and pits-distribution need improvement. Explainable AI can enhance the understanding and trust in CADx.

Participant Characteristics as Moderators of the Effects of Cognitive Behavioral Interventions on Concerns About Falling: Secondary Analyses of Two Randomized Controlled Trials

Summary: This study examined whether participant characteristics moderated the effects of two cognitive behavioral interventions on concerns about falling in older community-dwelling individuals. Secondary analyses of two RCTs were conducted, involving the group intervention A Matter of Balance - Netherlands (AMB-NL) (n = 540) and individual AMB - Home (n = 389) intervention. Marginal models were used for moderation assessment, including single and multiple moderator models. A total of 19 characteristics were assessed. Moderating effects were found for living situation, fall history, symptoms of depression, perceived general health, ADL disability, cognitive status, and consequences of falling-loss of independence subscale. Effects varied by intervention, time point, and type of model.

Summary: The study aimed to assess the clinical outcomes of increased adalimumab dose intervals versus conventional dosing in patients with stable remission of Crohn's disease. The results showed that increasing the adalimumab dose intervals to 3 weeks and further to 4 weeks was non-inferior for maintaining stable remission, and this approach reduced the risk of infections and healthcare costs.
