Modelling potential cost savings from use of real-time continuous glucose monitoring in pregnant women with Type 1 diabetes

Aim To investigate potential cost savings associated with the use of real-time continuous glucose monitoring (RT-CGM) throughout pregnancy in women with Type 1 diabetes. Methods A budget impact model was developed to estimate, from the perspective of National Health Service England, the total costs of managing pregnancy and delivery in women with Type 1 diabetes using self-monitoring of blood glucose (SMBG) with and without RT-CGM. It was assumed that the entire modelled cohort (n = 1441) would use RT-CGM from 10 to 38 weeks' gestation (7 months). Data on pregnancy and neonatal complication rates and related costs were derived from published literature, national tariffs, and device manufacturers. Results The cost of glucose monitoring was 588 pound with SMBG alone and 1820 pound with RT-CGM. The total annual costs of managing pregnancy and delivery in women with Type 1 diabetes were 23 pound 725 648 with SMBG alone, and 14 pound 165 187 with SMBG and RT-CGM; indicating potential cost savings of approximately 9 pound 560 461 from using RT-CGM. The principal drivers of cost savings were the daily cost of neonatal intensive care unit (NICU) admissions (3743) pound and the shorter duration of NICU stay (mean 6.6 vs. 9.1 days respectively). Sensitivity analyses showed that RT-CGM remained cost saving, albeit to lesser extents, across a range of NICU costs and durations of hospital stay, and with varying numbers of daily SMBG measurements. Conclusions Routine use of RT-CGM by pregnant women with Type 1 diabetes, would result in substantial cost savings, mainly through reductions in NICU admissions and shorter duration of NICU care.

Summary: This secondary analysis of the Twin Birth Study demonstrated that the planned delivery mode (CD or VD) in twin pregnancies with the first twin in cephalic presentation may have different effects on perinatal outcomes depending on gestational age at delivery. For pregnancies delivered at 32-37 weeks, planned VD appeared to be favorable, while from around 37 weeks onwards, planned CD might be safer. Evaluating the risks and benefits of each delivery mode is crucial to optimize perinatal outcomes in twin pregnancies.

Same-day discharge after cryoballoon ablation of atrial fibrillation: A multicenter experience

Summary: A retrospective study of 2374 consecutive patients who underwent cryoballoon ablation for atrial fibrillation at three US hospitals found that same-day discharge (SDD) protocol is safe and cost-effective. SDD patients were younger, had lower body mass index and CHADS-VASc scores compared to overnight stay (ONS) patients, with no significant difference in 30-day total complication rate. Sensitivity analysis showed potential yearly cost savings when 50% of patients were discharged the same day.

Can placental growth factors explain birthweight variation in offspring of women with type 1 diabetes?

Summary: In women with type 1 diabetes, infant birthweight is influenced by both glycaemic status and placental function. In women with suboptimal glycaemia, infant birthweight was heavier when placentas were healthy. Suboptimal placental function should be considered in the setting of suboptimal glycaemia and apparently 'normal' birthweight.

Environmental factors associated with freshwater recreational water quality in Niagara Region, Ontario, Canada: A path analysis

Systematic review of predictive models of microbial water quality at freshwater recreational beaches

Summary: This study systematically reviewed predictive models of fecal indicator bacteria at freshwater recreational sites in temperate climates, finding that most studies were conducted in the United States using E. coli as the indicator bacteria, with multiple linear regression being the most commonly used method to build predictive models. The frequently used predictors in best-fitting models included rainfall, turbidity, wave height, and wind speed and direction. The majority of predictive models had high accuracy rates and were more accurate than traditional methods, but limitations identified included unvalidated models, limited reporting of modeling assumptions, and lack of reporting on handling missing data. Further research is needed on the utility and accuracy of more advanced predictive modeling methods.

Region-Specific Associations between Environmental Factors and Escherichia coli in Freshwater Beaches in Toronto and Niagara Region, Canada

Quality of life instruments in atrial fibrillation: a systematic review of measurement properties

Summary: The study aims to identify the most frequently used atrial fibrillation-specific quality of life (QoL) instruments in atrial fibrillation (AF) ablation studies and evaluate the measurement properties of the most frequently used instrument. AFEQT and ASTA demonstrated the strongest measurement properties, but not all properties were assessed. Future research should focus on cross-cultural validation, measurement error, responsiveness, and interpretability.

Early mortality after inpatient versus outpatient catheter ablation in patients with atrial fibrillation
