Epidemiology and risk factors of methicillin-resistant Staphylococcus aureus and vancomycin-resistant enterococci infections in Zhejiang China from 2015 to 2017

Background: Gram-positive bacteria are dangerous and challenging agents of infection due to their increasing resistance to antibiotics. We aim to analyse the epidemiology and risk factors of methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin-resistant enterococci (VRE) in Zhejiang China. Methods: Gram-positive bacteria (including S. aureus, Enterococcus faecalis and Enterococcus faecium) were collected from eighty-six hospitals of eleven cities in Zhejiang China from 2015 to 2017. The detection rates of MRSA and VRE infection were calculated for the non-duplicated isolate according to year, region, hospital level, patient age, specimen type and patient category. Meanwhile, the detected resistances of MRSA, E. faecalis and E. faecium to different antibiotics from 2015 to 2017 were compared. The risk factors and the differences in MRSA and VRE detection rates were compared using odds ratio (OR) with 95% confidence interval (95% CI) and Chi-square test respectively. Results: From 2015 to 2017, the detection rates of MRSA and VRE decreased gradually. The cities with the highest MRSA and VRE detection rates tended to be adjacent; for example, the neighbouring cities Hangzhou and Quzhou had simultaneously high rates of MRSA and VRE infection. Patients from IIIA hospital who were older than 75 years and in the intensive care unit (ICU) were most at risk. No vancomycin-resistant isolate was found in MRSA. Resistance of E. faecalis and E. faecium to vancomycin and linezolid decreased slightly and then maintained a low level. Conclusions: The detection rates of MRSA and VRE stayed at moderate and low levels during the three year period of this study, while local dissemination was found in MRSA and VRE isolates. Sustained surveillance is necessary to prevent the spread or clonal dissemination of drug-resistant strains in Zhejiang China.

Summary: Increasing infections caused by bla(NDM)-carrying Klebsiella pneumoniae (NDMKP) pose an urgent threat to children with weakened immunity and limited antibiotic use. The isolates exhibit high resistance to carbapenems but are fully sensitive to other antibiotics. Molecular analysis reveals both clonal and horizontal transmission of bla(NDM) among NDM-KP strains in children's hospitals, with key plasmids and sequence type diversity contributing to their spread. This study provides important insights for pediatric clinicians in treating infections caused by NDM-KP.

Multiplexed Target Enrichment Enables Efficient and In-Depth Analysis of Antimicrobial Resistome in Metagenomes

The method was validated in various samples and can serve as an efficient tool for monitoring and evaluating the evolution of ARGs.

Emergence of an Extensive Drug Resistant Pseudomonas aeruginosa Strain of Chicken Origin Carrying bla(IMP-45), tet(X6), and tmexCD3-toprJ3 on an Inc(pRBL16) Plasmid

Summary: This study reports the first occurrence of extensively drug-resistant Pseudomonas aeruginosa strain PA166-2 carrying bla(IMP-45), tet(X6), and tmexCD3-toprJ3 on a single plasmid in chickens. The strain, belonging to ST313, was resistant to last-line antibiotics and the plasmid belonged to the incompatibility group Inc(pRBL16). The emergence of plasmid-mediated tigecycline resistance genes and carbapenemase genes from chickens expanded the global transmission of vital resistance genes. Control measures should be implemented to prevent further dissemination of such strains.

Genomic Shift in Population Dynamics of mcr-1-positive Escherichia coli in Human Carriage

Summary: The emergence of mcr-1, a colistin resistance gene, has gained global attention. While the prevalence of mcr-1-positive Escherichia coli (MCRPEC) strains in human carriage has decreased between 2016 and 2019, the genetic differences in these strains remain largely unknown. Through a comparative genomic study, we found three major differences in MCRPEC strains between the two time points. These differences include changes in plasmid types, emergence of additional antibiotic resistance genes, and an increased proportion of virulence genes, indicating a potential public health threat.

Rapid On-Site Detection of Extensively Drug-Resistant Genes in Enterobacteriaceae via Enhanced Recombinase Polymerase Amplification and Lateral Flow Biosensor

Summary: In this study, a cascade system was developed for the rapid on-site detection of extensively drug-resistant (XDR) genes in Enterobacteriaceae. The system utilizes polyethylene glycol (PEG) 200-enhanced recombinase polymerase amplification (RPA), a modified Chelex-100 lysis method, and a horseradish peroxidase (HRP)-catalyzed lateral flow immunoassay (LFIA) biosensor. The system demonstrated high sensitivity and specificity, allowing for the detection of XDR genes in Enterobacteriaceae in less than 1 hour without the need for large-scale instruments.

Comparison of the NG-Test Carba 5, Colloidal Gold Immunoassay (CGI) Test, and Xpert Carba-R for the Rapid Detection of Carbapenemases in Carbapenemase-Producing Organisms

The NG-test Carba 5, colloidal gold immunoassay (CGI) test, and Xpert Carba-R assay showed reliable performances in carbapenemase detection, with Xpert Carba-R being recommended for the detection of KPC variants.

Phenotypic and Genomic Characterization of ST133 Siderophore-Encoding Extensively Drug-Resistant Enterobacter hormaechei

Summary: We identified a highly drug-resistant and virulent strain of Enterobacter hormaechei, ST133/C210017, using the Galleria mellonella infection model. Genomic analysis revealed the presence of antibiotic resistance genes bla(KPC-2) and mcr-9.1, as well as virulence-related genes iutAiucABCD and iroBCDEN. Comparative genomics with other ST133 E. hormaechei strains showed that they shared serotype O3 and a majority (77.5%) carried the IncHI2 superplasmids, which contribute to resistance, virulence, and host adaptation.

Co-occurrence of multiple plasmid-borne linezolid resistance genes-optrA, cfr, poxtA2 and cfr(D) in an Enterococcus faecalis isolate from retail meat

Summary: This study investigates the molecular characteristics and transferability of plasmid-borne linezolid resistance genes optrA, cfr, poxtA2, and cfr(D) in linezolid-resistant Enterococcus faecalis DM86 from retail meat. The presence of known linezolid resistance genes was detected using PCR analysis. Conjugation experiments were conducted to evaluate the transferability of these resistance genes. The complete genome of DM86 was obtained, and four linezolid resistance genes were identified on three plasmids.

Escherichia coli high-risk clone ST410 harboring bla(NDM-13) isolated from hospital wastewater in China

Summary: A carbapenem-resistant Escherichia coli strain C-SRM-3 was isolated from hospital wastewater effluent in Hangzhou city, China in March 2022. The isolate belonged to the high-risk clone ST410 and carried bla(NDM-13) gene responsible for carbapenem resistance in E. coli. The detection of bla(NDM-13) gene in hospital wastewater highlights the importance of continuous monitoring to prevent the spread of carbapenem-resistant bacteria.

Rapid Identification of Species, Antimicrobial-Resistance Genotypes and Phenotypes of Gram-Positive Cocci Using Long Short-Term Memory Raman Spectra Methods

Summary: Antimicrobial resistance is a global public health problem, causing over 700,000 deaths annually. Researchers have developed a novel method that uses artificial intelligence to analyze Raman spectra and identify microbes and their susceptibility to antibiotics at genotype and phenotype levels. The method achieved high accuracy in bacterial species classification, identification of antimicrobial-resistance genes, and prediction of resistance phenotypes, outperforming other machine learning algorithms. The results suggest that Raman spectroscopy combined with LSTM analysis can be used for rapid bacterial species identification, detection of resistance genes, and assessment of drug-resistance phenotypes.
