Summary: Staging liver fibrosis has always been a challenge in viral hepatitis and other liver diseases. Non-invasive tests have been developed to overcome the limitations of liver biopsy. This review discusses the use of serological and physical markers for liver fibrosis staging in chronic hepatitis B and C. Furthermore, it addresses the issue of evaluating liver fibrosis after HCV cure.

Summary: This study aimed to determine the biomarker performance of proteolytic enzymes Cathepsin B and plasma kallikrein, as well as transforming growth factor-beta, in detecting hepatic fibrosis in chronic hepatitis C patients. The results suggest that Cathepsin B and plasma kallikrein could be used as circulating biomarkers to detect hepatic fibrosis in HCV-infected patients.

Summary: Chronic viral hepatitis is a major cause of liver disease and hepatocellular carcinoma. Despite being caused by different viruses, hepatitis B, C, and D have striking similarities in pathological impact. The advancements in omics and bioinformatics have revealed the important role of signaling networks in viral pathogenesis.

Summary: This study investigated liver histology and proposed a management strategy for patients with chronic hepatitis B (CHB) in the grey zone (GZ). The study found that over 70% of GZ CHB patients had significant histological disease. Antiviral treatment is recommended for HBeAg-positive and HBeAg-negative GZ CHB patients with high prothrombin time.

Summary: Viral hepatitis is a significant global cause of mortality, with 1.4 million deaths each year. Hepatitis B and C account for 90% of these deaths, and the World Health Organization aims to eliminate viral hepatitis as a public health threat by 2030. While current antivirals can suppress viral replication, they cannot eliminate the virus from infected patients.

Summary: Primary liver cancers, specifically Hepatocellular carcinoma (HCC), are associated with significant morbidity and mortality. While HCC mostly develops within the hepatic parenchyma, a portion may arise in a non-cirrhotic milieu known as non-cirrhotic-HCC (NCHCC). NCHCC is often diagnosed late due to lack of surveillance and is linked to factors like non-alcoholic fatty liver disease, diabetes, and viral infections such as chronic Hepatitis B. Early identification and monitoring strategies are crucial for improving outcomes in NCHCC patients.

Summary: In this study, it was found that CHB patients have lower serum PTX-3 levels compared to the control group, and these levels decrease further as fibrosis progresses. PTX-3 can be used as a non-invasive marker for early detection of fibrosis.

Summary: HCV infection often goes untreated in people who inject drugs, resulting in advanced liver fibrosis at a young age. This study aimed to assess the rate of significant fibrosis in PWID starting anti-HCV therapy and identify associated factors. Results showed that a quarter of PWID presented with significant liver fibrosis at treatment initiation, with obesity, long-term drug abstinence, harmful drinking, and older age contributing to advanced liver fibrosis.

Summary: The study analyzed the one-year outcomes of children with chronic hepatitis C (CHC) treated with ledipasvir/sofosbuvir (LDV/SOF), showing that regression of liver fibrosis is possible in patients with cirrhosis but further observations are necessary to find predictors of fibrosis regression.

Summary: This study investigated the prevalence and causes of significant liver damage among 619 people living with HIV. The results showed that 18.2% of the patients had significant liver damage, with 35.7% attributed to nonviral liver damage, mainly metabolic associated steatohepatitis. The study concluded that the burden of liver damage remains high among people living with HIV after HIV/HCV coinfection microelimination.

Summary: This study investigates the relationship between left ventricle, extracellular matrix remodeling, and fibrosis-linked amphiregulin in cirrhotic cardiomyopathy. The results show that ARG levels are significantly increased in patients with LVDD and have associations with indicators of ventricular remodeling and LV functional parameters. Monitoring ARG and other markers may be helpful for controlling myocardial function in patients with cirrhosis.
