Computer Assisted Three-Dimensional Reconstruction for Laparoscopic Resection in Adult Teratoma

Objective: The laparoscopic surgery for adult teratoma is difficult and requires more precise surgery. The purpose of this study is to explore the guiding effect of computer assisted three-dimensional (3D) reconstruction for laparoscopic surgery in the treatment of adult teratoma. Methods: A total of 16 adult patients with teratoma in recent 3 years were retrospectively analyzed. All patients underwent abdominal dynamic contrast-enhanced computed tomography (CT) scanning before operation. Among them, 10 patients underwent 3D reconstruction, and then the accurate preoperative diagnosis and assistant surgical planning were made to determine the best scheme of laparoscopic surgery. The remaining 6 cases not reconstructed were as control group. The differences of operation time, intraoperative blood loss, blood transfusion and length of hospital stay were compared between two groups. Results: Preoperative computer assisted 3D reconstruction clearly showed the abdominal vessels, parenchymal organs and tumors. The tumors could be observed stereoscopically from any angle, and the adjacent tissues and organs could be displayed separately, so as to understand the adjacent relationship between the tumors and the surrounding tissue space. Compared with the control group, the operation time of patients guided by 3D reconstruction was significantly shortened, the amount of intraoperative bleeding was significantly reduced, the length of hospital stay was shortened. In addition, a special patient with retroperitoneal teratoma was reported. Conclusion: Computer assisted 3D reconstruction can transform 2D plane images into 3D images, which can improve the accuracy and safety of laparoscopic surgery for adult teratoma.
