Introduction Between 2009 and 2011 a series of 26 articles on evidence-based medicine for interventional pain medicine according to clinical diagnoses were published. The high number of publications since the last literature search justified an update. Methods For the update an independent 3rd party, specialized in systematic reviews was asked in 2015 to perform the literature search and summarize relevant evidence using Cochrane and GRADE methodology to compile guidelines on interventional pain management. The guideline committee reviewed the information and made a last update on March 1st 2018. The information from new studies published after the research performed by the 3th party and additional observational studies was used to incorporate other factors such as side effects and complications, invasiveness, costs and ethical factors, which influence the ultimate recommendations. Results For the different indications a total of 113 interventions were evaluated. Twenty-seven (24%) interventions were new compared to the previous guidelines and the recommendation changed for only 3 (2.6%) of the interventions. Discussion This article summarizes the evolution of the quality of evidence and the strength of recommendations for the interventional pain treatment options for 28 clinical pain diagnoses.

Severe mental illness and palliative care: patient semistructured interviews

Inge Knippenberg, Nasira Zaghouli, Yvonne Engels, Kris C. P. Vissers, Marieke M. Groot

Summary: A study conducted in the Netherlands found that patients with severe mental illness (SMI) have different perceptions, experiences and expectations regarding palliative care. The study revealed a lack of awareness and planning among these patients when it comes to end-of-life issues. Therefore, the study suggests focusing on the values, current and future wishes, and needs of these patients to provide more personalized medical and mental treatment and care.

Spinal Cord Stimulation With Additional Peripheral Nerve/Field Stimulation Versus Spinal Cord Stimulation Alone on Back Pain and Quality of Life in Patients With Persistent Spinal Pain Syndrome

Esther P. Z. van Heteren, Bert-Kristian W. P. van Roosendaal, Eric-Jan J. A. A. van Gorp, Ewald M. Bronkhorst, Jan Willem Kallewaard, Jessica T. Wegener, Katja Burger, Otto P. M. Teernstra, Hendrik P. J. Buschman, Tanja E. Hamm-Faber, Kris C. P. Vissers

Summary: Persistent spinal pain syndrome (PSPS) or failed back surgery syndrome (FBSS) refers to new or persistent pain following spinal surgery for back or leg pain in a subset of patients. Spinal cord stimulation (SCS) can be considered for patients with leg pain refractory to conservative treatment, while subcutaneous stimulation or peripheral nerve field stimulation (PNFS) is an alternative for chronic low back pain. This study compared the effects of SCS with additional PNFS to SCS alone on pain and quality of life in patients with PSPS after 12 months. The results showed that SCS alone provided similar pain relief and quality-of-life improvement as PNFS in addition to SCS.

Prognostic factors for high societal costs: a register-based study on 561,665 patients with shoulder disorders

Lotte Sorensen, Johanna Maria van Dongen, Maurits van Tulder, Lisa Gregersen Oestergaard

Summary: The study aimed to identify prognostic factors for high societal costs and high costs of sick leave, and to assess the validity of the model across different diagnostic categories. The strongest prognostic factor for high societal costs was sick leave at the time of diagnosis. Other factors such as high age, comorbidity, and hospital admission the year before diagnosis were associated with high healthcare costs. The model showed good discriminative ability and explained a significant proportion of the outcome variation.

Cost-effectiveness of occupational therapy return-to-work interventions for people with mental health disorders: A systematic review

Suzanne Johanson, Lisa Gregersen Oestergaard, Ulrika Bejerholm, Carita Nygren, Maurits van Tulder, Magnus Zingmark

Summary: This study aims to identify and summarize the cost-effectiveness evidence of return-to-work (RTW) interventions provided by occupational therapists (OTs) for persons with mental health disorders. A systematic search resulted in 9 articles that met the inclusion criteria and underwent quality assessment using an economic evaluation tool. The results showed that supported employment, Individual Placement and Support was cost-effective in several contexts, while some interventions had larger effects but higher costs. The study concludes that conducting more methodologically robust economic evaluations is crucial to strengthen the economic perspective in OT RTW interventions.

Angela Jornada Ben, Sylvia Pellekooren, Judith E. Bosmans, Raymond W. J. G. Ostelo, Esther T. Maas, Mohamed El Alili, Maurits W. van Tulder, Frank J. P. M. Huygen, Teddy Oosterhuis, Adri T. Apeldoorn, Miranda L. van Hooff, Johanna M. van Dongen

Summary: The study aimed to develop and validate approaches for mapping Oswestry Disability Index responses to 3-level version of EQ-5D utility values and evaluate their impact on cost-utility results compared with published regression models. Three response mapping approaches were developed and their performance was assessed. The results suggest that the developed approaches are not valid for estimating individual patients' utility values and may significantly affect cost-utility results.

Identifying pain problems, healthcare professional perceptions, expectations and challenges in multidisciplinary pain center establishment

Ristiawan M. Laksono, Taufiq A. Siswagama, Aswoco A. Asmoro, Hasan Sjahrir, Andi M. T. Musba, Willy Halim, Walter van der Weegen, Kris Vissers

Summary: Multidisciplinary pain centers (MPC) are beneficial but not established in developing countries like Indonesia. A national survey on 1058 healthcare professionals (HCP) revealed the issues of pain management in Indonesia, including insufficient pain management, lack of pain data, and unawareness about MPC. However, most HCP agreed on the need for pain centers and recommended referring patients with "pain that requires intervention" to MPC. Financial barriers, such as insufficient patient insurance, were identified as the most important obstacle for referring patients to MPC in Indonesia. The high incidence of pain, inadequate pain management, and lack of pain data are the core problems underlying the need for MPC establishment in Indonesia. Overall, HCP have positive perceptions and expectations regarding MPC, but financial barriers are a potential burden.
