Diabetic retinopathy, a vascular and inflammatory disease: Therapeutic implications

Diabetic retinopathy (DR) is the most common microvascular complication of diabetes and the leading cause of visual impairment in the working-age population in the Western world. Diabetic macular oedema (DME) is one of the major complications of DR. Therapy with intravitreal injections of anti-vascular endothelial growth factor (anti-VEGF) drugs has become the gold standard treatment for DR and its complications. However, these drugs have no effect on the pathogenesis of DR and must be administered frequently via invasive intravitreal injections over many years. Thus, there is a pressing need to develop new therapeutic strategies to improve the treatment of this devastating disease. Indeed, an increasing volume of data supports the role of the inflammatory process in the pathogenesis of DR itself and its complications, including both increased retinal vascular permeability and neovascularization. Inflammation may also contribute to retinal neurodegeneration. Evidence that low-grade inflammation plays a critical role in the pathogenesis of DME has opened up new pathways and targets for the development of improved treatments. Anti-inflammatory compounds such as intravitreal glucocorticoids, topical non-steroidal anti-inflammatory drugs (NSAIDs), antioxidants, inflammatory molecule inhibitors, renin-angiotensin system (RAS) blockers and natural anti-inflammatory therapies may all be considered to reduce the rate of administration of antineovascularization agents in the treatment of DR. This report describes the current state of knowledge of the potential role of anti-inflammatory drugs in controlling the onset and evolution of DR and DME. (C) 2019 Elsevier Masson SAS. Effect of COVID-19-related lockdown on ophthalmic practice in Italy: A report from 39 institutional centers

The lymphatic vasculature: An active and dynamic player in cancer progression

Summary: The lymphatic vasculature not only transports immune cells, but also plays important roles in tumor growth and dissemination. Recent studies have uncovered various mechanisms by which the lymphatic system affects cancer progression, from inducing lymphangiogenesis to preparing metastatic niches and modulating the immune response. The interactions between tumor cells and lymphatic endothelial cells, as well as between immune cells and lymphatic endothelial cells, are crucial in both the tumor microenvironment and tumor-draining lymph nodes.

COVID-19 AdenoviralVector Vaccine and Central Retinal Vein Occlusion

Summary: This article reports a case of sudden onset ischemic retinal central vein occlusion after a second dose of COVID-19 adenoviral vector vaccine. The authors suggest that the adenoviral vector vaccine may increase the level of inflammatory cytokines, which could contribute to the development of central retinal vein occlusion. Therefore, patients at risk of ocular diseases should be informed about the possible ocular adverse events after vaccination.

Selective laser capsulotomy complications: Management and prevention

Summary: This study discusses how to manage and prevent incomplete laser cut and subsequent uncut tags of anterior capsular lens when performing selective laser capsulotomy. Results showed that using a 23-G vitrectomy scissors can avoid complications, while peeling the anterior capsulotomy disc with forceps may cause zonular stress and subsequent risks, and using the cystotomy needle tip may result in an irregular capsulotomy rim and micro tears. Therefore, key steps include anterior lens capsule staining and laser beam focusing for prevention.
