Summary: This systematic review and meta-analysis compares the postoperative short-term outcomes between preoperative colonic stenting (BTS) and emergency resection (ER) for right-sided malignant large bowel obstruction (MLBO). The analysis indicates that BTS results in a reduction in postoperative complications and mortality rates compared to ER for right-sided MLBO.

Summary: This study aimed to investigate the 5-year oncological outcomes of using self-expanding metallic stents (SEMS) as a bridge to surgery in patients with left-sided colonic cancer obstruction. The results showed comparable 5-year survival and disease-free survival rates between SEMS as a bridge to surgery and upfront emergency surgery, with no significant difference.

Summary: This study aimed to investigate the outcomes of self-expanding metallic stent as a bridge to surgery compared with emergency colonic resection in patients with left-sided colonic cancer obstruction. The results showed that there were no significant differences in perioperative and mid-term oncological outcomes between the two treatment methods.

Summary: This multicenter study investigated the effect of self-expanding metal stents (SEMS) as a bridge to surgery in colorectal cancer patients. The results showed that SEMS placement did not lead to an increase in circulating tumor cells (CTCs) and other factors may be responsible for the poorer oncologic outcomes observed after SEMS placement.

Summary: The study found that both unilateral and bilateral IS placement had high technical and clinical success rates in primary stent placement for UMHBO. However, bilateral IS placement showed a longer time to recurrent bile duct obstruction, indicating that it may be a good option for initial UMHBO drainage.

Summary: This study compared the efficacy and safety of gastrojejunostomy (GJJ) and endoscopic duodenal stent placement (EDSP) in patients with gastric outlet obstruction (GOO) due to unresectable pancreatic cancers (URPC). The results showed that patients who underwent GJJ had longer overall survival (OS) and lower frequency of adverse events compared to those who underwent EDSP. Subgroup analyses demonstrated that GJJ provided longer OS for patients with good performance status, biliary obstruction, duodenal obstruction near the papilla, and those receiving chemotherapy.

Summary: Endoscopic stent placement has been widely used for left-sided colonic or rectal cancer, but its use prior to colectomy for malignant right-sided colonic obstruction is controversial. This case report demonstrates the successful use of a colonic and coronary stent-first strategy in a patient with high cardiovascular risk, achieving good outcomes.

Summary: This study suggests that colonic stenting as a bridge to surgery can reduce the rate of stoma creation and lead to better discharge outcomes for patients, compared to immediate resection. There were no significant differences in 90-day readmission, reoperation, or procedural complications between the two groups.

Summary: In patients with malignant left-sided colon obstruction, undergoing definitive surgery within 2 weeks of colonic stenting may lead to better oncological outcomes without compromising postoperative outcomes. Further prospective studies are needed to compare outcomes between emergency surgery and different bridging intervals.

Summary: This retrospective multicentre cohort study aimed to identify risk factors for a permanent stoma (PS) in patients with left-sided obstructive colon cancer (LSOCC) and develop pre- and postoperative prediction models. The results showed that 37% of LSOCC patients had a PS, and 53% of those with a stoma directly after resection had a PS due to non-reversal. Both baseline characteristics and treatment strategies determine the risk of a PS in LSOCC patients. The developed predictive models will help physicians understand the role of individual variables in the risk of a PS and inform patients about the probability of a PS.

Summary: This study compared the safety and functional outcomes of a removable orthosis and a cast in the nonoperative management of stable type B ankle fractures. The results showed that the removable orthosis group had fewer complications and no significant differences in functional outcomes compared to the cast group at 6 and 12 weeks. However, at 26 weeks or longer, the removable orthosis group had significantly better functional outcomes.

Summary: Geriatric patients, especially frail ones, may benefit from de-escalation of care in a palliative setting instead of curative treatment. Strategies for improved shared decision-making, including appointing legal representatives and having goals of care conversations, can help provide better person-centred care for older patients. Additionally, physicians should tailor the decision-making process to the needs of the patient and their family.

Summary: This study evaluated the quality of evidence in geriatric traumatology, investigated the inclusion of patients with cognitive impairment, and examined methods used to determine cognitive impairment. A total of 2711 publications were screened, and 723 articles were included. The focus was mainly on retrospective studies, with small sample sizes and a lack of prospective studies and clinical trials. There is selective exclusion of cognitively impaired patients in clinical studies, and no consensus on cognitive impairment diagnosis. This review highlights the need for addressing these issues in future research.

Summary: This study aimed to assess the attitudes of surgeons involved in the treatment of synchronous colorectal liver metastases (sCRLM). The study found that there were differences in clinical practices and viewpoints on the management of sCRLM among surgeons from different specialties and continents. However, there was consensus on a growing role for minimally invasive surgery (MIS) and a need for evidence-based input.

Summary: A watch-and-wait approach for patients with rectal cancer after neoadjuvant chemoradiotherapy or radiotherapy is associated with better quality of life and functional outcome. Prospective evaluation was conducted to assess the quality of life and functional outcome of these patients, including bowel, urinary, and sexual function. The results showed that patients who were observed by a watch-and-wait approach had good quality of life, with some experiencing bowel and sexual dysfunction.

Summary: The study aimed to investigate the value of rectal cancer T-staging on MRI in relation to fibrotic transformation of the tumour bed. The results showed that ymrT-staging has moderate accuracy, with higher accuracy in patients with predominant fibrosis. Therefore, radiologists should focus on detecting residual disease and identifying potential candidates for organ preservation.

Summary: This study aimed to evaluate the oncological outcomes of patients with locally recurrent rectal cancer with a pCR, and compare them with patients without a pCR. Patients with a pCR had superior oncological outcomes. A watch-and-wait approach may be considered in highly selected patients to improve quality of life without compromising oncological outcomes.
