Propensity score-matched analysis of oncological outcome between stent as bridge to surgery and emergency resection in patients with malignant left-sided colonic obstruction

Background: Although self-expandable metal stent (SEMS) placement as bridge to surgery (BTS) in patients with left-sided obstructing colonic cancer has shown promising short-term results, it is used infrequently owing to uncertainty about its oncological safety. This population study compared long-term oncological outcomes between emergency resection and SEMS placement as BTS. Methods: Through a national collaborative research project, long-term outcome data were collected for all patients who underwent resection for left-sided obstructing colonic cancer between 2009 and 2016 in 75 Dutch hospitals. Patients were identified from the Dutch Colorectal Audit database. SEMS as BTS was compared with emergency resection in the curative setting after 1: 2 propensity score matching. Results: Some 222 patients who had a stent placed were matched to 444 who underwent emergency resection. The overall SEMS-related perforation rate was 7.7 per cent (17 of 222). Three-year locoregional recurrence rates after SEMS insertion and emergency resection were 11-4 and 13.6 per cent (P= 0-457), disease-free survival rates were 58-8 and 52.6 per cent (P= 0-175), and overall survival rates were 74-0 and 68-3 per cent (P= 0.231), respectively. SEMS placement resulted in significantly fewer permanent stomas (23.9 versus 45.3 per cent; P < 0-001), especially in elderly patients (29.0 versus 57.9 per cent; P < 0-001). For patients in the SEMS group with or without perforation, 3-year locoregional recurrence rates were 18 and 11.0 per cent (P= 0.432), disease-free survival rates were 49 and 59.6 per cent (P= 0-717), and overall survival rates 61 and 75.1 per cent (P= 0.529), respectively. Conclusion: Overall, SEMS as BTS seems an oncologically safe alternative to emergency resection with fewer permanent stomas. Nevertheless, the risk of SEMS-related perforation, as well as permanent stoma, might influence shared decision-making for individual patients.

Risk factors for a permanent stoma after resection of left-sided obstructive colon cancer- A prediction model

Summary: This retrospective multicentre cohort study aimed to identify risk factors for a permanent stoma (PS) in patients with left-sided obstructive colon cancer (LSOCC) and develop pre- and postoperative prediction models. The results showed that 37% of LSOCC patients had a PS, and 53% of those with a stoma directly after resection had a PS due to non-reversal. Both baseline characteristics and treatment strategies determine the risk of a PS in LSOCC patients. The developed predictive models will help physicians understand the role of individual variables in the risk of a PS and inform patients about the probability of a PS.

Cast versus removable orthosis for the management of stable type B ankle fractures: a systematic review and meta-analysis

Summary: This study compared the safety and functional outcomes of a removable orthosis and a cast in the nonoperative management of stable type B ankle fractures. The results showed that the removable orthosis group had fewer complications and no significant differences in functional outcomes compared to the cast group at 6 and 12 weeks. However, at 26 weeks or longer, the removable orthosis group had significantly better functional outcomes.

Summary: This study found that decreased muscle measurements (PMI and SMI) on preoperative CT scans were associated with increased risk of anastomotic leakage. Perspective; recommendations for improved patient participation in decision-making for geriatric patients in acute surgical settings

Summary: Geriatric patients, especially frail ones, may benefit from de-escalation of care in a palliative setting instead of curative treatment. Strategies for improved shared decision-making, including appointing legal representatives and having goals of care conversations, can help provide better person-centred care for older patients. Additionally, physicians should tailor the decision-making process to the needs of the patient and their family.

Study Quality and Patient Inclusion in Geriatric Orthopaedic Trauma Research: A Scoping Review

Summary: This study evaluated the quality of evidence in geriatric traumatology, investigated the inclusion of patients with cognitive impairment, and examined methods used to determine cognitive impairment. A total of 2711 publications were screened, and 723 articles were included. The focus was mainly on retrospective studies, with small sample sizes and a lack of prospective studies and clinical trials. There is selective exclusion of cognitively impaired patients in clinical studies, and no consensus on cognitive impairment diagnosis. This review highlights the need for addressing these issues in future research.

Long-term Quality of Life and Functional Outcome of Patients With Rectal Cancer Following a Watch-and-Wait Approach

Summary: A watch-and-wait approach for patients with rectal cancer after neoadjuvant chemoradiotherapy or radiotherapy is associated with better quality of life and functional outcome. Prospective evaluation was conducted to assess the quality of life and functional outcome of these patients, including bowel, urinary, and sexual function. The results showed that patients who were observed by a watch-and-wait approach had good quality of life, with some experiencing bowel and sexual dysfunction.
