Summary: Over 50% of patients in England did not undergo cholecystectomy following index admission for acute cholecystitis. Mortality was higher in the nonoperated group, which was mostly due to non-gallbladder pathologies but total hospital admission time for biliary causes was lower over 12 months.

Summary: This retrospective study of 162 patients undergoing PTGBD between 2010 and 2020 demonstrates promising clinical success rates in the treatment of acute cholecystitis, with PTGBD being a definitive treatment in a significant percentage of cases. However, the severity of gallbladder inflammation, particularly in grade III cases, plays a decisive role in clinical outcomes, with low success rates and high mortality rates to be considered.

Summary: This study aimed to determine associations between pre-operative factors and gangrenous cholecystitis. Older age, male gender, and comorbidities such as coronary disease, hypertension, arrhythmias, renal disease, and antiplatelet medication use were found to be predictive factors for gangrenous cholecystitis. Patients with gangrenous cholecystitis presented with higher white cell counts, higher C-reactive protein levels, and had poorer post-operative outcomes.

Summary: This study compared the clinical outcomes of early laparoscopic cholecystectomy (ELC) within the first two weeks of hospitalization with delayed laparoscopic cholecystectomy (DLC) for Grade II acute cholecystitis patients. Results showed that DLC had better outcomes with shorter operative time and postoperative hospital stay.

Summary: This study demonstrates that MLC is an effective surgical procedure for cholelithiasis. Compared to LC, MLC is a simpler and easier technique with shorter post-operative hospitalization. MLC has the potential to be the preferred surgical technique in developing countries due to its low cost and availability. However, there is a risk of conversion surgery in MLC, especially in patients with higher BMI, leucocyte count, bilirubin level, severity of adhesion, and chronic cholecystitis.

Summary: This nationwide study reveals a significant increase in the implementation of laparoscopic cholecystectomy in pregnant women with acute cholecystitis after 2007, alongside a significant reduction in the time to surgery. Compared to non-operative management, laparoscopic cholecystectomy is associated with significantly lower rates of adverse outcomes for both mothers and fetuses.

Summary: This study evaluated the difficulties leading to conversion during laparoscopic cholecystectomy, strategies to minimize this event, and the impact of subspecialization on conversion rates. Results showed that using salvage techniques such as fundus first cholecystectomy and subtotal cholecystectomy can reduce conversion rates and morbidity in difficult cholecystectomies.

Summary: The timing of early cholecystectomy does not affect post-operative complications and mortality, but it does have an impact on intraoperative complications and partial cholecystectomy rates. As the duration of symptoms increases, the complexity and risk of surgery also increase due to worsening inflammation.

Summary: This study conducted a network meta-analysis to evaluate the timing of cholecystectomy in acute cholecystitis. The results showed that early cholecystectomy within 72 hours from symptoms onset had the lowest conversion rate and reduced postoperative complications. Delaying cholecystectomy to 6 to 12 weeks reduced operating time but increased total in-hospital stay.

Summary: Patients with severe traumatic brain injuries should be primarily admitted to a hub trauma center with neurosurgical capabilities for immediate and specialized care. However, in cases where patients are admitted to a spoke hospital without neurosurgical capabilities, there is limited data on optimal management. This consensus provides practical recommendations for the management of severe isolated TBI patients in centers without neurosurgical capabilities and during transfer to a hub center.

Summary: The study aimed to develop a novel prognostic machine learning framework that utilizes longitudinal information encoded in efficient, cost-effective, and non-invasive markers to identify individuals with mild cognitive impairment (MCI) at risk of developing dementia. The findings showed that incorporating the output of unsupervised learning significantly improved the performance of supervised ML models. This prognostic framework can be applied to improve recruitment in clinical trials and select early interventions for individuals at high risk of developing dementia.

Summary: This study aimed to quantify the reduction in surgical activity for indeterminate thyroid nodules during the COVID-19 pandemic and evaluate whether delays in surgery led to an increased occurrence of aggressive tumors. The study found a significant reduction in surgeries during the pandemic and observed an increased occurrence of larger tumors, lymph node metastases, and high-risk tumors during the decrease phase. The study suggests that surgery for indeterminate thyroid nodules should not be postponed even during future pandemic escalations.

Summary: This study analyzed the outcomes of patients with colorectal peritoneal carcinosis and a peritoneal cancer index (PCI) greater than or equal to 16 treated with cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC). The results showed that patients with a PCI less than 16 had a 2-year overall survival rate of 81%, while patients with a PCI greater than or equal to 16 had a 2-year overall survival rate of only 37%. Therefore, current guidelines need to reassess the exclusion of these patients from CRS and HIPEC.
