Summary: The study suggests that for low-risk microcarcinomas of DTC, lobectomy may be a suitable approach with the possibility of additional treatments in some cases and longer follow-up for confirming event-free survival compared to total thyroidectomy. On the other hand, total thyroidectomy is preferred for intermediate-risk micro- and macro-DTCs to achieve an excellent response despite higher postsurgical complication rates.

Summary: PTC patients with benign or nonsuspicious contralateral nodules have similar rates of recurrence and survival after lobectomy compared to those without contralateral nodules. Contralateral nodules alone should not be an indication for total or completion thyroidectomy.

Summary: This study found that around 74.21% of low-risk PTC patients do not require hormone replacement therapy after lobectomy, while 40.53% develop temporary or permanent hypothyroidism. Multivariate analysis showed that higher preoperative TSH levels, presence of Hashimoto's thyroiditis, and right lobectomy were associated with hypothyroidism.

Summary: The optimal surgical procedure for unilateral multifocal papillary thyroid carcinomas remains controversial. This study compared the efficacies of total thyroidectomy and lobectomy in patients with this condition. The results showed that while total thyroidectomy led to more vocal cord paralysis and parathyroid injury compared to lobectomy, it resulted in better disease-free survival. Importantly, most cases of vocal cord paralysis and parathyroid injury resolved within 1 year after surgery. There was no significant difference in postoperative lymph node metastasis and postoperative recurrence between the two procedures.

Summary: The study investigated the impact of total thyroidectomy (TT) or thyroid lobectomy (TL) on the prognosis of patients with unilateral papillary thyroid microcarcinoma (PTMC). While TT did not show significant improvement in cancer-specific survival (CSS) and overall survival (OS) compared to TL overall, it may benefit patients with multiple risk factors like cervical lymph node metastasis (CLNM), multifocality, extrathyroidal extension, and younger age.

Summary: The study aimed to compare the HRQoL of PTMC patients undergoing TT and LT. Results showed a significant negative linear association between LT and TT groups in terms of RP, RE, SF, PCS, and MCS. THYCA-QOL scores indicated that the TT group had higher levels of complaints regarding scars compared to the LT group, while there was no significant difference in FoP-Q-SF scores between the two groups.

Summary: This meta-analysis demonstrates excellent long-term recurrence-free survival following both total thyroidectomy and lobectomy in patients with papillary thyroid microcarcinoma. It also suggests that patients undergoing total thyroidectomy have a slightly better 10-year recurrence-free survival compared to patients undergoing lobectomy.

Summary: The 2015 ATA guidelines led to a change in overall practice in surgical management of PTC toward increasing TL. Both urban and rural settings had an increase in TL following the guideline change, emphasizing the importance of clinical practice guidelines to ensure best practice in both rural and urban settings.

Summary: Lobectomy alone may be associated with an elevated recurrence risk in patients with unilateral intermediate- to high-risk PTC compared with larger surgical extents. However, lobectomy and neck dissection may provide similar tumor control compared with the conventional approach of total thyroidectomy and neck dissection.

Summary: This study evaluated the need for additional surgical treatment in patients diagnosed with aggressive variants of papillary thyroid cancer after lobectomy. The study analyzed 249 patients who underwent hemithyroidectomy or bilateral total thyroidectomy for aggressive variants of papillary thyroid cancer. The results suggested that close follow-up may be performed without additional surgical treatment in low-risk patients incidentally detected after hemithyroidectomy. The presence of palpable tumor, clinical N1b, tumor size, gross extrathyroidal extension, and advanced pathological stages were associated with an increased risk of recurrence.

Summary: Nearly 50% of patients undergoing lobectomy for benign disease required thyroid hormone replacement therapy, and one-quarter of these patients started therapy >= 2 years after surgery. Hashimoto thyroiditis was independently associated with thyroid hormone replacement therapy. Appropriate counseling and at least 2 years of follow-up of thyroid function are recommended for patients who undergo thyroid lobectomy.

Summary: In this retrospective study, it was found that RFA had similar outcomes to TT in patients with PTMC in the isthmus. RFA group had lower surgical time, blood loss, hospital stays, and treatment costs compared to the TT group. Although minor pain at the operation site was observed in all RFA patients, no distant metastasis was detected except for one case of recurrence after 6 months.

Summary: Patients undergoing CABG to attain kidney or liver transplant candidacy had significantly higher major adverse postoperative outcomes compared to non-transplant patients, with higher rates of 30-day mortality and neurologic events. Most patients in the pretransplant group did not ultimately undergo organ transplantation, indicating the need for further research on postoperative management strategies in this high-risk population.

Summary: This study found that patients with breast cancer who were Medicaid or self-pay and those in the highest quartile of neighborhood deprivation scores were more likely to discontinue endocrine therapy early. These social determinants of health become more important given the rise in unemployment and loss of insurance coverage during the COVID-19 pandemic. Early identification of at-risk patients and enrollment in insurance optimization programs may improve treatment adherence.
