Summary: The study explored the efficacy of everolimus and mycophenolate mofetil as CNI-free GVHD prophylaxis, resulting in high rates of GVHD incidence, not recommended as the sole GVHD prophylaxis. Further modifications, such as adding post-transplant cyclophosphamide or anti-thymocyte globulin, should be considered in subsequent studies.

Summary: This study compared the impact of everolimus-based regimens and standard of care (mycophenolic acid + standard calcineurin inhibitor) on cardiovascular outcomes in de novo kidney transplant recipients. The results showed that both treatment approaches had similar rates of cardiovascular morbidity and mortality. Subgroups of patients with pre-existing risk factors had higher incidence of cardiovascular events in both treatment arms.

Summary: This study aims to evaluate the safety, tolerability, and efficacy of metformin and empagliflozin in ameliorating metabolic profiles after kidney transplantation. The primary endpoint is the differences in visceral-to-subcutaneous fat area ratio over 12 months, with secondary outcomes including kidney graft function, glycometabolism, lipid metabolism, and inflammatory parameters. The trial will enroll 105 kidney transplant recipients, providing 90% power to detect the difference at 5% significance.

Summary: This study aims to determine the frequency of pre-existing and de novo anti-HLA IgE antibodies in solid organ transplant recipients. The study found that anti-HLA IgE antibodies occur at low frequency in kidney, liver, heart, and lung transplant recipients and are associated with sensitization at the IgG level.

Summary: The third dose of the BNT16b2 vaccine increased the rate of positive anti-SARS-CoV-2 antibody response in liver transplant recipients, but some patients still did not respond. Predictors of antibody response failure after the third dose included higher dose of mycophenolate mofetil and lower estimated glomerular filtration rate.

Summary: In a randomised clinical trial, the authors compared cyclosporine immunosuppression with adalimumab for the treatment of Vogt-Koyanagi-Harada disease and found that cyclosporine treatment was non-inferior. This highlights the need for head-to-head comparisons between biologics and conventional immunosuppressants in the treatment of this disease.

Summary: This study aimed to compare the efficacy and safety of leflunomide versus azathioprine as maintenance therapy for lupus nephritis. The results showed that leflunomide was non-inferior to azathioprine in terms of time to kidney flare, clinical parameters improvement, extrarenal flare occurrence, and adverse events incidence.

Summary: Withdrawal of steroids or switching to everolimus instead of cyclosporine are two effective strategies to reduce treatment-related side effects and improve long-term outcomes of kidney transplantation. Both regimens provide similar 5-year patient survival and death-censored graft survival, but the everolimus group showed higher (51)CrEDTA clearance at 1 and 5 years post-transplantation.

Summary: This study aimed to investigate the factors affecting the pharmacokinetics of mycophenolate mofetil (MMF) in pediatric patients after liver transplantation and establish a population pharmacokinetic model for clinical dosage adjustment reference. The results showed that body weight and MMFdt dose were important reference factors for adjusting the dosage of MMFdt in pediatric patients.

Summary: This guideline on oxygen use in acute care provides evidence-based recommendations on indication, prescription, monitoring, and discontinuation of oxygen therapy. The main indication for oxygen is hypoxemia, and both hypoxemia and hyperoxemia should be avoided as they increase mortality. The guideline offers target ranges for oxygen saturation in acute oxygen therapy and recommendations for oxygen delivery systems based on patient safety and comfort.

Summary: This study compares the efficacy of three different antibiotic eradication regimens in pediatric CF patients and finds that administration according to the standard operating procedure (SOP) order leads to the best results. Age, previous Pa infections, and Pa therapies are identified as risk factors for eradication failure. Therefore, following the pre-defined SOP order for antibiotic treatment can significantly improve eradication rates.

Summary: This study investigated the effectiveness of CNI-free immunosuppression using mTOR inhibitors in lung transplant recipients. The results showed that the 1-year survival rate was 36% in patients with malignancy who were converted to CNI-free immunosuppression, while the 1-year survival rate was 100% in other patients. Additionally, patients who received this immunosuppression showed improvement in glomerular filtration rate.

Summary: We have developed a set of precise, simple and suitable criteria for evaluating the response to biologic therapy in patients with severe asthma. This evaluation score, called the Biologic Asthma Response Score (BARS), uses the main criteria of exacerbations, oral corticosteroid therapy and asthma control to assess the response. Based on the score, we propose a decision-making scheme for considering switching the biologic therapy.

Summary: Lower respiratory tract infections (LRTI) are common reasons for patient visits, but antibiotics are often overprescribed. Procalcitonin (PCT) point-of-care testing (POCT) is not accurate in identifying bacterial pneumonia in outpatient care. PCT may be a marker for severe bacterial infections, but is not suitable for milder infections.

Summary: The role of cardiopulmonary exercise testing (CPET) and inspiratory pressure measurement in exercise capacity and muscle strength in severe asthmatic patients treated with anti-IL-5 therapy was investigated. After 3 months of antibody treatment, the breathing reserve exhaustion reduced significantly and no desaturation or breathing reserve exhaustion was observed. CPET may show indications of alveolar recruitment and ventilatory efficiency in severe asthma patients treated with antibody therapy.
