Summary: Despite physical therapy being the recommended treatment for patients with degenerative meniscal tears over 45 years old, a significant percentage still opt for surgery, especially those with worse knee function, lower education level, and better general physical health status. However, the prognostic models developed in this study had low explained variance and insufficient predictive accuracy to accurately predict non-responders to physical therapy.

Higher risk of 2-year cup revision of ceramic-on-ceramic versus ceramic-on-polyethylene bearing: analysis of 33,454 primary press-fit total hip arthroplasties registered in the Dutch Arthroplasty Register (LROI)

Justin van Loon, Inger N. Sierevelt, Anneke Spekenbrink-Spooren, Kim T. M. Opdam, Rudolf W. Poolman, Gino M. M. J. Kerkhoffs, Daniel Haverkamp

Summary: This study analyzed data from the Dutch Arthroplasty Register and found that ceramic-on-ceramic (CoC) bearings had a higher 2-year cup revision rate compared to ceramic-on-polyethylene (CoPE) bearings in press-fit total hip arthroplasty. The main reason for revision in CoC bearings was cup loosening.

Willingness to pay for attributes of Templestay and implications on marginal utility of different meditation forms

Se-Hyuk Kim, James W. Mjelde, Tae-Kyun Kim, Choong-Ki Lee, Byunggil Chun

Summary: Participants in Templestays are willing to pay a premium for private accommodations and to experience natural mountain settings. The marginal willingness to pay varies depending on the type of meditation, with Seon meditation showing increasing marginal utility.

Are Detailed, Patient-level Social Determinant of Health Factors Associated With Physical Function and Mental Health at Presentation Among New Patients With Orthopaedic Conditions?

David N. Bernstein, Amanda Lans, Aditya V. Karhade, Marilyn Heng, Rudolf W. Poolman, Joseph H. Schwab, Daniel G. Tobert

Summary: This study investigated the patient-level social determinants of health (SDoH) factors associated with musculoskeletal health disparities, and found that transportation issues and financial hardship were related to poorer physical and mental health outcomes. Therefore, targeted interventions, such as providing transportation subsidies, financial assistance, and social support services, should be implemented to reduce health disparities.

An aptamer-based sandwich assay for detection of alpha-defensin human neutrophil protein 1 on a microfluidic platform

Rishabh Gandotra, To -Wen Chen, Feng-Chih Kuo, Mel S. Lee, Gwo-Bin Lee

Summary: In this study, a new microfluidic platform based on magnetic beads and fluorescent probes was developed to detect α-defensin and human neutrophil protein 1 (HNP 1) in synovial fluid. The method showed high sensitivity and specificity, and could rapidly and accurately detect infection biomarkers within 60 minutes. This compact device with a wide dynamic range has great potential for future diagnostics of periprosthetic joint infection.

Arthroscopic partial meniscectomy vs non-surgical or sham treatment in patients with MRI-confirmed degenerative meniscus tears: a systematic review and meta-analysis with individual participant data from 605 randomised patients

S. R. W. Wijn, G. Hannink, H. Osteras, M. A. Risberg, E. M. Roos, K. B. Hare, V. A. van de Graaf, R. W. Poolman, H. -W. Ahn, J. -K. Seon, M. Englund, M. M. Rovers

Summary: The objective of this study was to identify subgroups of patients with MRI-confirmed degenerative meniscus tears who may benefit from APM compared to non-surgical or sham treatment. The results showed a small improvement in knee pain for the APM group compared to the non-surgical or sham group at 24 months follow-up. However, there were no significant differences in overall knee function and health-related quality of life between the two groups. No relevant subgroup of patients who benefitted from APM was identified.

Influence of the Type of Bone Cement Used in Two-Stage Exchange Arthroplasty for Chronic Periarticular Joint Infection on the Spacer Replacement and Reinfection Rate

Meng-Wei Chang, Cheng-Ta Wu, Shih-Hsiang Yen, Timothy L. Tan, Po-Chun Lin, Feng-Chih Kuo

Summary: This study aimed to investigate whether a spacer made from commercial ALBC or plain bone cement with additional antibiotics could affect the spacer exchange rate before reimplantation. The results showed that the spacer exchange rate was lower in the commercial ALBC group compared to the plain bone cement group. However, the use of commercial ALBC was not associated with a lower reinfection rate after reimplantation.

Challenges in developing national orthopedic health research agendas in the Netherlands: process overview and recommendations

Annabelle R. Iken, Rudolf W. Poolman, Rob G. H. H. Nelissen, Maaike G. J. Gademan

Summary: The growing demand for clinical research has led to healthcare evaluation to improve evidence-based medicine. Health research agenda (HRA) plays a valuable role in determining funding and resource allocation, helping researchers and policymakers design successful research programs. This perspective provides an overview of the development process of HRAs in orthopedic surgery in the Netherlands and offers recommendations for future development.

Expression and distribution of generated neurons and endogenous precursors in rat cerebral cortical venous ischemia

Tae-Kyun Kim, Yasuhiro Takeshima, Yukiteru Ouji, Fumihiko Nishimura, Ichiro Nakagawa, Young-Soo Park, Masahide Yoshikawa, Hiroyuki Nakase

Summary: Neurogenesis confirmed in rat models with cerebral arterial ischemia but unclear for cerebral venous ischemia. Using rat models created by two-vein occlusion, neurogenesis was evaluated by immunohistochemistry. The study found significant numbers of newly generated neurons and precursors in the ischemic area, suggesting that the cortex could be the main origin of neurogenesis after cortical venous ischemia.

Summary: Moderate to heavy wound drainage and persistent wound drainage were strongly associated with prosthetic joint infection (PJI), and additional parameters are needed to guide the decision to reoperate on patients for suspected acute PJI.

A paper-based aptamer-sandwich assay for detection of HNP 1 as a biomarker for periprosthetic joint infections on an integrated microfluidic platform

Rishabh Gandotra, Feng-Chih Kuo, Mel S. Lee, Gwo-Bin Lee

Summary: This study introduces a paper-based microfluidic chip for the quantitative detection of HNP 1 in synovial fluid, providing a rapid and cost-effective diagnostic tool for periprosthetic joint infections (PJI) cases. The chip demonstrates high sensitivity and selectivity, with a wide dynamic range, making it a promising solution for PJI diagnosis in clinical settings.

Clockwise torque results in higher reoperation rates in left-sided femur fractures

Jacobien H. F. Oosterhoff, Hidde Dijkstra, Aditya V. Karhade, Rudolf W. Poolman, Inger B. Schipper, Rob G. H. H. Nelissen, Daphne van Embden, Ruurd L. Jaarsma, Joseph H. Schwab, Job N. Doornberg, Marilyn Heng, Bhavin Jadav

Summary: This study aimed to compare the reoperation rates within 30 days after surgery for left-versus right-sided proximal femur fractures in patients undergoing cephalomedullary nailing. The results showed a higher risk of reoperation for left-sided fractures compared to right-sided fractures. This finding may support the hypothesis that clockwise rotation during implant insertion and weightbearing may lead to higher reoperation rates.

Survivorship, complications, and outcomes following distal femoral arthroplasty for non-neoplastic indications

K. R. Sobol, B. R. Fram, J. T. Strony, S. A. Brown

Summary: This study reports the short-term implant survivorship and complications in patients who underwent distal femoral arthroplasty (DFA) for non-neoplastic indications. The results show that DFA is a viable option for distal femoral bone loss from non-neoplastic causes, but with high overall complication rates.
