Periodontitis is a chronic inflammatory disease, initiated by the presence of a bacterial biofilm, called dental plaque, which affects both the periodontal ligaments and bone surrounding teeth. In the last decades, several lines of evidence have supported the existence of a relationship between periodontitis and systemic health. For instance, as periodontitis acts within the same chronic inflammatory model seen in cardiovascular disease (CVD), or other disorders, such as diabetes, several studies have suggested the existence of a bi-directional link between periodontal health and these pathologies. For instance, people with diabetes are more susceptible to infections and are more likely to suffer from periodontitis than people without this syndrome. Analogously, it is now evident that cardiac disorders are worsened by periodontitis, both experimentally and in humans. For all these reasons, it is very plausible that preventing periodontitis has an impact on the onset or progression of CVD and diabetes. On these grounds, in this review, we have provided an updated account on the current knowledge concerning periodontal disease and the adverse effects exerted on the cardiovascular system health and diabetes, informing readers on the most recent preclinical studies and epidemiological evidence.

Pharmacological treatment of type 2 diabetes in elderly patients with heart failure: randomized trials and beyond

Angela Sciacqua, Elena Succurro, Giuseppe Armentaro, Sofia Miceli, Daniele Pastori, Giuseppe Rengo, Giorgio Sesti

Summary: Heart failure and type 2 diabetes are two important public health problems, especially among the elderly. Certain antidiabetic drugs should be used with caution in elderly individuals due to the risk of hypoglycemia and fluid retention. GLP-1 RAs and SGLT2 inhibitors have shown positive effects on cardiovascular outcomes. Treatment of diabetic elderly patients with heart failure should be based on scientific evidence, clinical judgment, and consideration for their dignity and quality of life.

Summary: This study investigated the prognostic impact of testosterone deficiency (TD) on women with heart failure with reduced ejection fraction (HFrEF). It found that around one-third of women with HFrEF had TD, which significantly affected their morbidity and mortality. TD was associated with a worse clinical profile including exercise capacity, right ventricular-pulmonary arterial coupling, and renal function.

Cardiac sympathetic innervation and mortality risk scores in patients with heart failure

Summary: The study found a weak association between cardiac sympathetic innervation and risk scores for predicting mortality in patients with heart failure. Therefore, it is recommended to integrate tools indicative of both arrhythmic and general mortality risks when evaluating patients eligible for device implantation.

Neuropsychiatric symptoms and their neural correlates in individuals with mild cognitive impairment

Natascia De Lucia, Giovanni Carbone, Benedetta Muzii, Nicola Ferrara, Giuseppe Rengo, Nelson Mauro Maldonato, Grazia Daniela Femminella

Summary: This study aimed to evaluate the association between neuropsychiatric symptoms, cognitive function, regional tau deposition, and brain volumes in MCI subjects. The results showed that 61.4% of MCI subjects had at least one neuropsychiatric symptom, with depression, irritability, and sleep disturbances being the most prevalent. MCI subjects with neuropsychiatric symptoms showed significant differences in cognitive tests of frontal and executive functions and reduced brain volumes in the orbitofrontal and posterior cingulate cortices.

Three-Dimensional Assessment of Temporomandibular Joint Morphology and Facial Asymmetry in Individuals with Different Vertical Skeletal Growth Patterns

Rohan Diwakar, Rosaria Bucci, Ankur Kaushik, Anubhav Bansal, Paolo Bucci, Anuraj Singh Kochhar, Gianrico Spagnuolo

Summary: The aim of this study was to use CBCT to investigate the condyle-fossa relationship, TMJ morphology, and facial asymmetry in subjects with different vertical skeletal growth patterns. Results showed that the hyperdivergent group had larger anterior joint space and smaller condylar dimensions compared to the normodivergent and hypodivergent groups. Additionally, the right condylar distances to the midsagittal plane were significantly larger than the left distances in all three groups. Facial asymmetry values did not differ among the different vertical groups.

Chemical, structural and cytotoxicity characterisation of experimental fluoride-doped calcium phosphates as promising remineralising materials for dental applications

Salvatore Sauro, Gianrico Spagnuolo, Carmela Del Giudice, Davino M. Andrade Neto, Pierre B. A. Fechine, Xiaohui Chen, Sandro Rengo, Xiaojing Chen, Victor P. Feitosa

Summary: This study evaluated the cytotoxicity, chemical and structural properties of fluoride-doped calcium-phosphates as potential remineralising materials for dental applications. Experimental calcium phosphates were tested for their ability to crystallise into apatite-like and their fluoride release. The materials showed biocompatibility and the ability to promote cell proliferation, indicating their potential use as remineralising materials in dentistry.

Evaluating the Mandibular Trabecular Structure in Patients with and without Bruxism: A Cross-Sectional Study

Alessandra Valletta, Kioumars Tavakoli Tafti, Kimia Baghaei, Amirhossein Moaddabi, Parisa Soltani, Gianrico Spagnuolo, Akhilanand Chaurasia

Summary: Fractal analysis was used to evaluate the trabecular pattern of the mandibular condyle, angle, and dental region in individuals with and without bruxism using panoramic radiographs. No significant differences were found in the fractal dimension values of the regions of interest between the two groups. Fractal analysis of panoramic radiographs is not useful in detecting patients with bruxism.

Surface electromyography in the assessment of masticatory muscle activity in patients with pain-related temporomandibular disorders: a systematic review

Liliana Szyszka-Sommerfeld, Magdalena Sycinska-Dziarnowska, Gianrico Spagnuolo, Krzysztof Wozniak

Summary: Temporomandibular disorders (TMD) are prevalent painful conditions affecting the orofacial region. Surface electromyography (sEMG) is a useful method for assessing patients with TMD-P. Results showed differences in masticatory muscle activity (MMA) between TMD-P patients and healthy controls. The diagnostic efficacy of sEMG in TMD-P remains unclear.

The Photocoagulation-Assisted Aesthetic Treatment of Phlebectasias of the Lips Using a Nd:YAG Laser with a Low Energy Level

Niccolo Giuseppe Armogida, Alessandra Valletta, Elena Calabria, Federica Canfora, Carlo Rengo, Gianrico Spagnuolo

Summary: This study proposes the use of Nd:YAG laser with low energy levels to treat labial venous lakes. All patients achieved complete healing within 30 days, with patients with a diameter <= 6 mm showing complete resolution after 7 days. The study suggests that Nd:YAG laser treatment is an effective and well-tolerated approach to aesthetic treatment of labial venous lakes.

Summary: This study identified drug interaction perpetrators among drugs prescribed to older adults and analyzed the prevalence of gene polymorphisms that can increase their interacting potential. The study also found that few commercial clinical decision support systems include pharmacogenomic factors in drug-drug interaction risk evaluation, and none of them are designed for use in older adults.

Summary: The accuracy of measuring gingival thickness was investigated by comparing clinical evaluation and intraoral ultrasonography. The study found that there was no significant difference in gingival thickness between ultrasonography and clinical evaluation in the anterior region, but there was a significant difference in the posterior region. The area under the ROC curve values indicated that ultrasonography had acceptable accuracy in determining gingival thickness, particularly in the anterior region.
