Trends and inequalities in maternal and child health in a Brazilian city: methodology and sociodemographic description of four population-based birth cohort studies, 1982-2015

Background Few low-middle-income countries have data from comparable birth cohort studies spanning over time. We report on the methods used by the Pelotas cohorts (1982, 1993, 2004 and 2015) and describe time trends in sociodemographic characteristics of the participant families. Methods During the four study years, all maternity hospitals in the city were visited daily, and all urban women giving birth were enrolled. Data on socioeconomic and demographic characteristics were collected using standardized questionnaires, including data on maternal and paternal skin colour, age and schooling, maternal marital status, family income and household characteristics. The analyses included comparisons of time trends and of socioeconomic and ethnic group inequalities. Results Despite a near 50% increase in the city's population between 1982 and 2015, the total number of births declined from 6011 to 4387. The proportion of mothers aged 35years increased from 9.9% to 14.8%, and average maternal schooling from 6.5 [standard deviation (SD) 4.2] to 10.1 (SD 4.0) years. Treated water was available in 95.3% of households in 1982 and 99.3% in 2015. Three-quarters of the families had a refrigerator in 1982, compared with 98.3% in 2015. Absolute income-related inequalities in maternal schooling, household crowding, household appliances and access to treated water were markedly reduced between 1982 and 2015. Maternal skin colour was associated with inequalities in age at childbearing and schooling, as well as with household characteristics. Conclusions During the 33-year period, there were positive changes in social and environmental determinants of health, including income, education, fertility and characteristics of the home environment. Socioeconomic inequality was also reduced.

Measles vaccination in Brazil: where have we been and where are we headed?

Summary: This study analyzed vaccine coverage, its homogeneity, and measles cases in Brazil from 2011 to 2021, particularly during the COVID-19 pandemic. The findings showed a progressive decline in vaccine coverage and homogeneity after 2015, with a more significant impact after 2020, especially in the North and Northeast regions. Lower vaccine coverage clusters were associated with worse human development indicators, social inequality, and limited access to healthcare services. The pandemic exacerbated health inequalities, highlighting the importance of strengthening primary care, improving health communication, and ensuring vaccine access to reduce missed opportunities and vaccine hesitancy.

Vaccination against poliomyelitis in Brazil from 2011 to 2021: successes, setbacks, and challenges ahead

Summary: The drop in childhood vaccination coverage, including poliomyelitis, is a major health concern. This study analyzed the temporal trend of polio vaccine coverage in the first year of life from 2011 to 2021 and mapped vaccination coverage in Brazil, including the COVID-19 pandemic period. The findings revealed a reduction in vaccination coverage in all regions, especially in the North and Northeast regions and during the COVID-19 pandemic. The drop in coverage was more pronounced in socially vulnerable states and health regions after 2019, indicating a risk of wild virus reintroduction.

Effect of preterm birth on growth and blood pressure in adulthood in the Pelotas 1993 cohort

Summary: This study explored the differences between preterm and term-born individuals in birth size and adult height, weight, and blood pressure in a Brazilian birth cohort. Preterm participants had lower birth size and remained shorter in adulthood for males, but not for females. At 22 years, females born preterm had lower blood pressure compared to term-born females. These differences were not observed in males.

Absolute intensity thresholds for tri-axial wrist and waist accelerometer-measured movement behaviors in adults

Summary: This study aimed to compare raw triaxial acceleration data from two accelerometer brands (GENEActiv and ActiGraph GT3X+), as well as different placement positions (non-dominant wrist, dominant wrist, and waist). Thresholds for differentiating inactivity, moderate intensity, and vigorous intensity were derived. The results showed that the acceleration increases with the intensity of activities, regardless of brand or placement. The thresholds obtained from this study can be used to classify movement behaviors in adults accurately.

Parental stress and dental caries experience in adolescents: analysis of data from a birth cohort study in Pelotas, Southern Brazil

Summary: This study analyzed the association between parental stress and dental caries experience in adolescents in southern Brazil. Interviews and oral health examinations were conducted in the homes of the adolescents to determine the main exposure and outcome. After adjusting for confounding factors, parental stress was found to be associated with a higher mean number of decayed, missing and filling surfaces in the adolescents.

Physical Activity During Adolescence and Mental Health in Early Adulthood: Findings From the 1993 Pelotas Birth Cohort Study

Summary: This study aimed to evaluate the association between physical activity (PA) during adolescence and depression and anxiety disorders in young adulthood. The study found that active participants in leisure-time PA and total PA at 11 years had a lower risk of depression at age 22. Additionally, the more PA recommendations were met during adolescence, the lower the prevalence of depression. However, PA during adolescence was not associated with anxiety in young adults after controlling for potential confounders.

Birth conditions nutritional status in childhood associated with cardiometabolic risk factors at 30 years of age: a cohort study

Summary: This study aimed to investigate the association between birth conditions, nutritional status, childhood growth, and cardiometabolic risk factors at 30 years of age. The researchers also examined whether body mass index (BMI) at 30 years old mediated the relationship between childhood weight gain and cardiometabolic risk factors. The findings suggest that rapid relative weight gain after 2 years of age may have long-term consequences on the risk of metabolic and cardiovascular disorders.

Effects of the Pelotas (Brazil) Peace Pact on violence and crime: a synthetic control analysis

Summary: This study examined the effectiveness of the Pelotas Pact for Peace (the Pacto) in reducing violence and crime in Pelotas, Brazil. The results showed a 9% reduction in homicide and a 7% reduction in robbery. However, these effects were only observed during the COVID-19 pandemic period and no significant effects were found for non-violent property crimes, violence against women, and school dropout.

Prevalence of adolescent risk behaviors at 11 and 15 years of age: data from the 2004 Pelotas birth cohort

Summary: The study aimed to investigate the prevalence of risk behaviors, such as experimentation with cigarettes, electronic cigarettes, alcohol, substances, delinquent behavior, and sex, at age 15, stratified by sex and socioeconomic position. The study also explored the prevalence of cigarette and alcohol experimentation at age 11 and the persistence and cumulative incidence of these behaviors between ages 11 and 15. The results showed gender and socioeconomic disparities in the prevalence of risk behaviors, emphasizing the need for further research on causal pathways and the increasing prevalence among girls.
