The North American Thrombosis Forum Atrial Fibrillation Action Initiative consensus document is a comprehensive yet practical briefing document focusing on stroke and bleeding risk assessment in patients with atrial fibrillation, as well as recommendations regarding anticoagulation options and management. Despite the breadth of clinical trial data and guideline recommendation updates, many clinicians continue to struggle to synthesize the disparate information available. This problem slows the uptake and utilization of updated risk prediction tools and adoption of new oral anticoagulants. This document serves as a practical and educational reference for the entire medical community involved in the care of patients with atrial fibrillation. (C) 2016 Elsevier Inc. All rights reserved.

The association between hepatic steatosis and incident cardiovascular disease, cancer, and all-cause mortality in a US multicohort study

Summary: This study aimed to investigate the association between NAFLD and cardiovascular disease, cancer, and all-cause mortality. The results showed that hepatic steatosis was associated with overall mortality, but the association was weakened when considering time-varying covariates. There was no significant association between hepatic steatosis and cardiovascular disease or cancer. The study highlights the importance of considering concurrent cardiometabolic disease in studying the association between NAFLD and cardiovascular outcomes.

Psychosocial stressors and current e-cigarette use in the youth risk behavior survey

Summary: This study examines the association between psychosocial stressors and current e-cigarette use among adolescents in the United States. The results show that adolescents who experience psychosocial stressors are more likely to use e-cigarettes, similar to the association between stressors and combustible cigarette use. These findings highlight the importance of targeted school-based interventions to reduce adolescent e-cigarette use.

Reversal agents for current and forthcoming direct oral anticoagulants

Summary: In the past 20 years, there has been a transition from vitamin K antagonists to direct oral anticoagulants (DOACs). DOACs include dabigatran, apixaban, edoxaban, and rivaroxaban. Although DOACs are associated with less serious bleeding, reversal agents may be needed in cases of serious bleeding or urgent surgery/intervention. This paper provides insight into reversing the effects of DOACs, discusses new reversal agents, and outlines a strategic framework for reversing factor XI inhibitors in clinical trials.

Vitamin D in atherosclerosis and cardiovascular events

Summary: In this study, it was found that blood eosinophil count and eosinophil cationic protein (ECP) concentration are risk factors for cardiovascular diseases. Eosinophils and ECP were shown to contribute to vascular calcification and atherogenesis through the BMPR-1A/1B-Smad-1/5/8-Runx2 signaling pathway.

Accuracy of the Apple watch for detection of AF: A multicenter experience

Summary: The aim of this study was to determine the accuracy of the irregular rhythm notification (IRN) feature of the Apple watch in subjects with a previous diagnosis of nonpermanent atrial fibrillation (AF). The results showed that the IRN had a low rate of false positive detections and high specificity in a population with known AF. However, increased sensitivity and wear times would be necessary for disease management.

Summary: This study examined the prognostic value of clonal hematopoiesis of indeterminate potential (CHIP) in individuals with established atherosclerotic cardiovascular disease (ASCVD). The results showed that any CHIP and large CHIP were associated with adverse outcomes, with TET2 and spliceosome CHIP having the strongest associations.

Summary: Atrial fibrillation (AF) is a strong risk factor for ischemic stroke, which is a major cause of disability and death. With the increasing prevalence of AF and improved survival in cardiovascular disease, the number of people affected by AF will continue to rise. This report summarizes a workshop focused on identifying key research opportunities related to stroke prevention in AF, addressing areas such as improving risk stratification tools, overcoming challenges with oral anticoagulants, and determining the optimal roles of percutaneous left atrial appendage occlusion and surgical closure/excision. The report aims to promote innovative research that leads to personalized and effective stroke prevention strategies in individuals with AF.

PAR4 Inhibition Reduces Coronary Artery Atherosclerosis and Myocardial Fibrosis in SR-B1/LDLR Double Knockout Mice

Summary: This study investigated the effects of PAR4 inhibitory pepducin (RAG8) on coronary artery disease in SR-B1/LDLR double knockout mice fed a high-fat, high-cholesterol diet. The results showed that RAG8 reduced coronary artery atherosclerosis and myocardial fibrosis, as well as platelet and leukocyte accumulation in atherosclerotic coronary arteries.

Prevention of atrial fibrillation: a call to action

Summary: Cardiometabolic risk factors, particularly obesity and hypertension, have the highest population-attributable risk for atrial fibrillation (AF). Hypertension, obesity, acute illness, cardiovascular disease, and inflammation are the top five individual risk factors for AF. We propose five key strategies to manage these risk factors for primary prevention of AF.
