Prevalence of comorbidity and associated risk factors in adults with atopic dermatitis

BackgroundAtopic dermatitis (AD) is a common chronic skin disorder, which may persist into adulthood; however, the prevalence of comorbidities in patients with AD is not well characterized. AD is considered a systemic disorder like psoriasis, which has raised a need for data on the comorbidity profile of patients with AD, to assess the potential risks, benefits, and complications in management of patients with AD. We described the occurrence of medical and psychiatric comorbidities and associated risk factors in adults with AD compared with psoriasis and the general population. MethodsAll Danish individuals aged 18 years with a hospital (inpatient or ambulatory) diagnosis of AD or psoriasis during the study period (January 1, 1995-December 31, 2012) were linked in administrative registers. ResultsOverall, prevalence of smoking and alcohol abuse was higher among patients with AD than the general population, but lower than psoriasis patients. Similarly, patients with AD had more risk factors and higher prevalence of comorbidity than the general population, but lower prevalence and reduced risk compared to psoriasis patients, except for use of anxiolytics, which was higher in severe AD. Prevalence of diabetes was lower in AD than psoriasis patients as well as general population controls. ConclusionsDespite an increased risk of various medical and psychiatric comorbidities compared to general population controls, adult patients with AD had markedly lower prevalence of cardiovascular disease than psoriasis patients. However, prevalence of psychiatric comorbidity and tobacco smoking was alarmingly high in severe patients with AD, which might be target for intervention in patient management.

Summary: This study investigated whether the prognostic values of echocardiographic parameters changed with updates in treatment guidelines and the emergence of novel SARS-CoV-2 variants. The results showed that patients hospitalized during the second wave had worse left ventricular function, and reduced global longitudinal strain was associated with COVID-19-related mortality independently of wave, treatment, and SARS-CoV-2 variant.

Safety and Efficacy of Topical Calcineurin Inhibitors in the Treatment of Facial and Genital Psoriasis: A Systematic Review

Summary: Facial and genital psoriasis can significantly impact the quality of life and are difficult to treat. Topical calcineurin inhibitors have been recognized as an effective off-label treatment for these sensitive areas, but limited data on safety and efficacy are available. A systematic review of the literature found positive efficacy in all 24 included studies, with statistically significant reductions in psoriasis severity observed in 11 studies. The most common short-term adverse effects reported were local stinging, burning, and itching. Topical calcineurin inhibitors play an important role in the treatment of facial and genital psoriasis, demonstrating effectiveness and generally good tolerability with minimal adverse effects.

Summary: The real-world effectiveness of biologics in treating psoriasis was examined using a nationwide registry. The results showed that the risk of discontinuation or off-label dose escalation was lower for secukinumab compared to ustekinumab, but higher for adalimumab. However, the risk of discontinuation was higher for both secukinumab and adalimumab. For patients who were bio-naive, the risk of discontinuation was comparable between secukinumab and ustekinumab.

Summary: This study evaluated the efficacy of oral Janus kinase inhibitor upadacitinib in subgroups of patients with moderate-to-severe atopic dermatitis (AD). The results showed that upadacitinib consistently improved skin condition and itch in these subgroups, regardless of age, sex, race, disease severity, body surface area involvement, past treatment history, etc. These findings support upadacitinib as a suitable treatment option for a variety of AD patients.
