Summary: Avascular necrosis (AVN) of the femoral head commonly leads to symptomatic osteoarthritis of the hip. While hip replacement is a viable option for older patients, it poses significant challenges for pediatric, adolescent, and young adult patients. Early detection is vital, but current diagnostic techniques are not sensitive enough. Various surgical techniques exist to prevent disease progression, but their effectiveness is limited, and more research is needed in tissue engineering for a potential long-term solution.

Summary: This study investigated changes in subchondral bone composition, microstructure, bone marrow adiposity, and microvascular perfusion in end-stage osteonecrosis of the femoral head (ONFH) compared to osteoarthritis (OA). The results showed that, in the necrotic zone, ONFH patients had lower volume fraction of subchondral trabecular bone, less pronounced type-B carbonate substitution, more pronounced collagen maturity, and significantly depleted bone marrow adipocytes compared to OA patients. The sclerotic zone in the ONFH group exhibited greater trabecular thickness and higher dynamic contrast-enhanced MRI area under the curve and Ktrans. The distant zone in the ONFH group had significantly lower volume fraction of subchondral bone, trabecular number, and Kep. This study demonstrated alterations in subchondral bone microstructure, composition, perfusion, and adipose content in all zones of the femoral head.

Summary: This study investigated the regulatory effect and molecular mechanism of VEC-derived Exos modified with VEGF in osteogenic and adipogenic differentiation of BMSCs in SANFH. The results showed that VEGF-VEC-Exos promoted osteogenic differentiation and suppressed adipogenic differentiation of BMSCs. Furthermore, VEGF-VEC-Exos activated the MAPK/ERK pathway in BMSCs, thereby promoting osteoblast differentiation and inhibiting adipogenic differentiation. Hence, VEGF-VEC-Exos could be a potential therapeutic approach for treating SANFH.

Summary: This study investigated the collapse mechanism in osteonecrosis of the femoral head (ONFH) by studying the relationship between changes in femoral head blood circulation and histomorphometry characteristics of the collapse area. The results showed that the collapse in ONFH always involved the margin of the necrotic lesion of the lateral column and started at the lateral column, which was ischemic. Preserving a portion of the blood supply to the lateral column improved the prognosis and delayed collapse. Measurement of blood flow in the lateral area of the femoral head can predict collapse and guide clinical therapy.

Summary: In this study, an icariin-loaded 3D-printed porous Ti6Al4V reconstruction rod was developed and its mechanical and biological efficacy were verified through in vitro and in vivo experiments. The results demonstrated that the reconstruction rod facilitated osteogenesis and neovascularization, providing a positive therapeutic effect for early avascular necrosis of the femoral head.

Summary: This study proposed a beta-TCP system for treating avascular necrosis of the femoral head (ANFH) by promoting revascularization and bone regeneration. The study revealed and quantified the angio-conductive properties and concurrent osteogenesis of the highly interconnected porous beta-TCP scaffold using an in vivo model. Mechanical test and analysis showed that the mechanical loss caused by tissue necrosis and surgery was compensated after implantation, leading to adaptive strength increase and bone regeneration.

Summary: In this study, the results of femoral head core decompression and bone marrow aspirate concentrate (CD-BMAC) injection in pediatric patients with sickle cell disease (SCD) were evaluated. CD-BMAC injection showed significant improvement in pain and functional outcomes in the short term with a very low risk for complications. Skeletal maturity and femoral head collapse were significant predictors of treatment failure.

Summary: This study found that Ato can alleviate SANFH, inhibit apoptosis, and promote autophagy. miR-186 was significantly upregulated after Ato treatment, targeting TLR4 and inactivating the MAPKs/NF-kappa B pathway. Inhibition of miR-186 reversed the protection of Ato on SANFH rats, while inhibition of TLR4 restored the protective effect of Ato. Ato reduced apoptosis and promoted autophagy of OB-6 cells by upregulating miR-186 and inhibiting the TLR4/MAPKs/NF-kappa B pathway.
