Background: Male infertility is a widespread condition among couples. In about 50% of cases, couple infertility is attributable to the male partner, mainly due to a failure in spermatogenesis. In recent times, the crucial role that modifiable lifestyle factors play in the development of infertility have generated a growing interest in this field of study, i.e. aging, psychological stress, nutrition, physical activity, caffeine, high scrotal temperature, hot water, mobile telephone use. Several studies have investigated associations between semen quality and the presence of lifestyle stressors i.e. occupational, life events (war, earthquake, etc.) or couple infertility; overall, these studies provide evidence that semen quality is impaired by psychological stress. In this review, we will discuss the impact of quality of life (modifiable lifestyle factors) and psychological stress on male fertility. In addition, the role that increased scrotal temperature along with inappropriate nutritional and physical exercise attitudes exert on male fertility will be presented. Conclusion: The decline of male fertility, particularly associated with advancing age, incorrect lifestyles and environmental factors plays an important role on natality, and its consequences on the future on human population makes this an important public health issue in this century. Thus, modification of lifestyle through a structured program of educational, environmental, nutritional/physical exercise and psychological support, combined with the use of nutraceutical antioxidants can prevent infertility and therefore, may help couples to obtain better quality of life and improved possibility to conceive spontaneously or optimize their chances of conception.

Summary: Loss of function mutations in the PHEX gene can lead to X-linked dominant hypophosphatemia, the most common form of genetic rickets. This condition is characterized by renal phosphate wasting, increased fibroblast growth factor 23 (FGF-23), growth retardation, bone deformities, and musculoskeletal manifestations. Despite the discovery of numerous mutations in the PHEX gene, there are currently no clear genotype-phenotype correlations in patients with hypophosphatemic rickets (XLH).

Sexual and Reproductive Outcomes in Obese Fertile Men with Functional Hypogonadism after Treatment with Liraglutide: Preliminary Results

Sandro La Vignera, Rosita A. Condorelli, Aldo E. Calogero, Rossella Cannarella, Antonio Aversa

Summary: This study prospectively investigates the effects of liraglutide, a GLP1 analog, on reproductive and sexual function in men with metabolic hypogonadism. The results show that liraglutide significantly improves sperm parameters and erectile function, as well as increasing testosterone and sex hormone-binding globulin levels. This suggests that liraglutide is a useful pharmacological treatment for male patients with metabolic hypogonadism.

Effects of tadalafil on skeletal muscle tissue: exploring interactions and novel mechanisms of action

Cristina Antinozzi, Emanuela A. Greco, Paolo Sgro, Ivan Dimauro, Antonio Aversa, Luigi Di Luigi

Summary: In addition to its mechanical roles, the skeletal muscle system plays a crucial role in thermoregulation, secondary sexual characteristics, hormone metabolism, glucose uptake and storage, and energetic metabolism. PDE5i drugs, such as tadalafil, can influence skeletal muscle physiology and endocrine-metabolic pathways by increasing the intracellular level of cGMP. These drugs can modulate insulin-related metabolic pathways and hormone responses, protein metabolism, and anabolism/catabolism during and after physical exercise-related stress.

Nutritional and physical activity issues in frailty syndrome during the COVID-19 pandemic

Maria Chiara Massari, Viviana Maria Bimonte, Lavinia Falcioni, Antimo Moretti, Carlo Baldari, Giovanni Iolascon, Silvia Migliaccio

Summary: 'Frailty' is a state of increased vulnerability caused by impairment of homeostasis due to stress. Frail individuals are more likely to experience significant health decline and have a higher risk of adverse outcomes such as osteoporosis, falls, and disability. Frailty is becoming increasingly important due to global population aging. The pathophysiology of frailty involves cumulative molecular and cellular damage and is partially independent of aging but closely related. Uncorrected nutrition and sedentary behavior worsen frailty.
