Summary: The long-term results of fenestrated endovascular aortic aneurysm repair (FEVAR) for juxtarenal aortic aneurysm (jAAA) disease are good, with a high rate of technical success and a low mortality rate. However, the need for reintervention remains high. Long-term renal function and survival support the use of FEVAR as a valid treatment option for jAAA disease.

Summary: After evaluating 124 consecutive FEVAR cases, it was found that the aortic diameter significantly increased at the levels of SMA and lowest renal artery after 3 years. However, after adjusted analysis, different intraoperative factors did not show significant differences. The results suggest that aortic neck dilation in the visceral segment can occur after repair of juxtarenal aneurysms.

Summary: A propensity-matched comparison of OSR and FEVAR for elderly patients undergoing JAAA repair found that there was no significant difference in 30-day mortality between the two groups, although OSR was associated with higher rates of pulmonary and renal complications compared to FEVAR.

Summary: This study aimed to explore the diameter changes in the supravisceral aorta after FEVAR, finding that over two-thirds of patients experienced AND, and aortic diameter dilation was common among patients. Device oversizing was identified as the strongest predictor of dilation, while longer seal lengths were found to be protective.

Summary: The study described the initial experience using the TA Treo device for FEVAR, with patients showing high rates of conditions such as hypertension, hyperlipidemia, coronary artery disease, and chronic obstructive pulmonary disease. Results indicated high technical success rates and short hospital stays, making this approach a preferred technique.

Summary: The study found that FEVAR had lower perioperative morbidity and mortality compared to OSR for complex abdominal aortic aneurysms (AAAs). Hospital procedural volume did not impact the 30-day mortality or myocardial infarction rates for FEVAR.

Summary: The study evaluated the five-year outcomes of using the COOK Zenith p-Branch OTS device for juxtarenal AAAs. The technical success rate was high with the most common adverse event being renal injuries during follow-up. Survival rate was 76% with no aneurysm-related deaths.

Summary: The study evaluated the use of COOK Zenith p-Branch device in treating AAA and showed that it is safe and effective in selected patients, both in urgent and elective cases. Long-term results suggest that p-Branch stent could be an alternative to DC, but further innovation is needed to address visceral branch complications.

Summary: This study evaluated the perioperative outcomes of supra- and infraceliac sealing in patients undergoing fenestrated endovascular aneurysm repair for juxta-/pararenal aortic aneurysms. Supraceliac sealing was associated with lower risk of type IA endoleaks but higher perioperative complications compared to infraceliac sealing.

Summary: This study investigated the dynamics of proximal seal dilatation (PSD) following FEVAR, finding faster expansion in the first year and decelerating thereafter. While the clinical implication of PSD seems limited in the initial years post-implantation, further research is needed to assess its impact on long-term FEVAR outcomes.

Summary: This study aimed to compare the medium-term results of open surgery, fenestrated endovascular repair, and chimney endovascular repair in patients with juxta/pararenal abdominal aortic aneurysms. The results showed that open surgery had advantages over endovascular repair in terms of aortic-related intervention and vessel branch/bypass stenosis/occlusion during mid-term follow-up.

Summary: This study assessed outcomes and cerebrovascular events after F-BEVAR with right vs left UE access, showing that using right UE access for F-BEVAR has low morbidity and minimal risk of perioperative ischemic stroke or transient ischemic attacks. Overall, UE access is not associated with an increased risk of perioperative stroke compared with femoral access only.

Summary: During fenestrated endovascular repair (FEVAR), the technique of incorporating mesenteric vessels with a scallop or fenestration does not significantly affect the complexity and outcomes of the procedure. Not stenting the coeliac axis (CA) during FEVAR does not pose perioperative or long-term clinical harm, but it increases CA instability. Increasing the aortic coverage can reduce the instability of non-CA branch vessels.

Summary: We present a case of a 60-year-old male with methamphetamine-induced cardiomyopathy who developed an ischemic stroke due to left ventricular thrombus emboli. The patient had a history of methamphetamine abuse, hypertension, and previous ischemic stroke. Imaging showed acute cortical infarcts in the brain and bilateral ventricular thrombus. Treatment included thrombolysis, heparin drip, and goal-directed medical therapy for heart failure. This case highlights the potential risk of ischemic stroke in patients with methamphetamine-induced cardiomyopathy.

Summary: This study evaluated the role of preoperative vitamin D supplementation in preventing postoperative atrial fibrillation in patients with vitamin D deficiency or insufficiency undergoing coronary artery bypass grafting. Three randomized controlled trials were analyzed, and the results showed that preoperative vitamin D supplementation effectively reduced the rate of postoperative atrial fibrillation in these patients. The duration of hospitalization did not differ significantly between the vitamin D supplementation group and the control group.
