Inpatient admissions and costs for adolescents and young adults with congenital heart defects in New York, 2009-2013

Summary: Most individuals born with congenital heart defects survive to adulthood, but healthcare utilization patterns for adolescents and adults with CHDs have not been well described. In New York State, there were 5,100 unique individuals with >= 1 CHD diagnosis codes recorded during inpatient admissions between 2009-2013. The median inpatient cost was $10,720 and the average length of stay was 3.0 days per admission; 55.1% were emergency admissions. Admission volume increased by 48.7% from 2009 to 2013, while total inpatient costs increased by 91.8% over the same period.

Summary: Individuals with congenital heart defects are more likely to seek care at pediatric/adult or adult-only cardiac surgery centers if they have severe conditions, private health insurance, higher severity of illness, need surgical or cardiac treatments, and shorter travel distances. These findings can help improve access to specialized care for this high-risk population.

Summary: COVID-19 mortality in adults with congenital heart disease (CHD) is similar to the general population. Factors associated with death include male sex, diabetes, cyanosis, pulmonary hypertension, renal insufficiency, and previous hospital admission for heart failure. Worse physiological stage is linked to mortality risk.

Summary: Advancements in surgery and management have led to an increasing number of older patients with tetralogy of Fallot, resulting in various late complications. With the rising incidence of age-related diseases, the management of these late complications will continue to evolve as research provides insights into long-term outcomes. Futuristically, there may be more data to support novel heart failure therapies and consensus guidelines on the management of refractory arrhythmias and aortic complications in this aging TOF population.

Summary: CDC's Surveillance of Congenital Heart Defects Across the Lifespan project aims to understand the epidemiology and outcomes of congenital heart defects (CHD) using large databases. The study found acceptable positive predictive values (PPV) for CHD identification based on ICD-9-CM codes, but further evaluation and algorithm development are needed for accurate identification.

Summary: This study characterized health care usage for adolescents with congenital heart defects (CHDs) and found that the usage was higher for those with severe CHDs compared to nonsevere CHDs. Adolescents with CHDs have a substantial burden of disease, especially with public insurance, severe CHDs, and more comorbidities.
