Summary: This study aimed to collect PNH patient data from hematology centers across Turkey to identify clinical features and management. After evaluating patients from 19 different institutions, it was found that fatigue and abdominal pain were the most frequent presenting symptoms. This study provided valuable information for understanding the disease and differences between patients in Turkey and existing literature.

Progress in the Management of Pregnancy with Paroxysmal Nocturnal Hemoglobinuria: A Review

Summary: Paroxysmal nocturnal hemoglobinuria (PNH) is a rare hemolytic disease with thrombosis as a major cause of death. Managing PNH in pregnant patients is challenging and there is a lack of consensus on treatment. This article reviews 32 studies on PNH in pregnancy, providing guidance for obstetricians and supporting the management of PNH patients. Eculizumab has become the primary treatment choice for PNH during pregnancy, effectively controlling intravascular hemolysis and minimizing risks to the mother and fetus.

Novel targeted C3 inhibitor pegcetacoplan for paroxysmal nocturnal hemoglobinuria

The new targeted C3 therapy Empaveli (pegcetacoplan) has been approved by the US FDA for adult PNH patients. Pegcetacoplan targets C3 and effectively controls the complement cascade, resulting in improved hemoglobin levels and reduced need for blood transfusions.

Paroxysmal nocturnal hemoglobinuria: Where are we going

Summary: Paroxysmal nocturnal hemoglobinuria (PNH) is a rare hematological disorder characterized by deficiency of GPI-linked complement regulators. Despite the introduction of C5 inhibitors, residual hemolysis still occurs, leading to anemia and transfusion dependency in some patients. The development of longer-acting and subcutaneous formulations of C5 inhibitors, as well as proximal complement inhibitors, have shown promising results in improving hemoglobin levels and reducing hemolysis. Combination treatments have also been explored. This review discusses the current therapeutic options and emerging approaches for PNH.

Discovering C3 targeting therapies for paroxysmal nocturnal hemoglobinuria: Achievements and pitfalls

Summary: The treatment of paroxysmal nocturnal hemoglobinuria (PNH) has been revolutionized by the introduction of the anti-C5 agent eculizumab, but extravascular hemolysis mediated by C3 remains a problem. Compstatin, a peptide that inhibits the complement cascade at the level of C3, has shown promise in improving the clinical response in PNH. Pegcetacoplan, a pegylated form of compstatin, has been evaluated in clinical studies and has shown superior results compared to eculizumab in terms of hemoglobin change and reduction in hemolysis.

The results indicate that respiratory virus infections pose a significant risk for BTH in PNH patients and highlight the need for regular screening and close monitoring of patients with respiratory symptoms.

Case report: Paroxysmal nocturnal hemoglobinuria presenting with hemorrhagic esophageal varices

Summary: We report a case of a female who was cured of hemorrhagic esophageal varices caused by paroxysmal nocturnal hemoglobinuria (PNH) through transjugular intrahepatic portosystemic shunt (TIPS) treatment. PNH complicated by portal vein and visceral veins thrombosis without hepatic veins is extremely rare, and it can be easily misdiagnosed due to lack of awareness. Hemorrhagic esophageal varices due to PNH with portal vein thrombosis have only been reported in one previous case in 1974, and this report presents the second case.

Summary: This study aimed to identify the dysfunctional pathways involved in the pathophysiology of PNH by comparing the metabolic profiles of PNH patients to healthy controls, as well as analyzing the metabolomic profiles before and after eculizumab treatment. The results showed significant differences in metabolomes between PNH patients and healthy controls, and eculizumab treatment appeared to improve defects in the Acyl CoA metabolism, reducing oxidative stress and inflammation.

Summary: The study identified 197 patients with detectable PNH clones over 15 years, with 88% having aplastic anaemia, 8% classical PNH, and 3% myelodysplastic syndrome. The overall incidence rate was estimated at 0.35 cases per 100,000 people per year, equating to 220 newly diagnosed cases in the UK each year. The overall prevalence rate was 3.81 per 100,000, equating to an estimated 2400 prevalent cases in the UK.

Immunophenotypic assessment of PNH clones in major and minor cell lineages in the peripheral blood of patients with paroxysmal nocturnal hemoglobinuria

Summary: This study shows that PNH patients with multiple PNH clones do not always display the same abnormality across all cell lineages routinely tested. There is no simple explanation for this but is likely due to a combination of complex molecular, genetic and biochemical dysfunction in different blood cell types.
