Antimicrobial resistance of major clinical pathogens in South Korea, May 2016 to April 2017: first one-year report from Kor-GLASS

The Korean government established an antimicrobial resistance (AMR) surveillance system, compatible with the Global AMR Surveillance System (GLASS): Kor-GLASS. We describe results from the first year of operation of the Kor-GLASS from May 2016 to April 2017, comprising all non-duplicated clinical isolates of major pathogens from blood,urine, faeces and urethral and cervical swabs from six sentinel hospitals. Antimicrobial susceptibility tests were carried out by disk diffusion, Etest, broth microditution and agar dilution methods. Among 67,803 blood cultures, 3,523 target pathogens were recovered. The predominant bacterial species were Escherichia coil (n = 1,536), Klebsiella pneumoniae (n = 597) and Staphylococcus aureus (n = 584). From 57,477 urine cultures, 6,394 E. coil and 1,097 K. pneumoniae were recovered. Bloodstream infections in inpatients per 10,000 patient-days (10TPD) were highest for cefotaxime-resistant E. coil with 2.1, followed by 1.6 for meticillin-resistant Sta. aureus, 1.1 for imipenem-resistant Acinetobacter baumannii, 0.8 for cefotaxime-resistant K. pneumoniae and 0.4 for vancomycin-.resistant Enterococcus faecium. Urinary tract infections in inpatients were 7.7 and 2.1 per 10TPD for cefotaxime-resistant E. coil and K. pneumoniae, respectively. Kor-GLASS generated welt-curated surveillance data devoid of collection bias or isolate duplication. A bacterial bank and a database for the collections are under development.

Risk factors and microbiological features of recurrent Escherichia coli bloodstream infections

Summary: Understanding the risk factors and microbiological features in recurrent E. coli BSI can aid clinicians in their management planning. Data from 2017 to 2018 were collected to assess antimicrobial resistance rates and molecular epidemiology. Risk factors for recurrence included vascular catheterization, ESBL phenotype, and high SOFA score. The highest proportion of ST131 and ESBL genotype was observed in early onset recurrent BSI. Whole genome sequencing revealed the presence of various virulence factors, resistant genes, and plasmid types in all types of BSI. This knowledge is important for clinicians in managing recurrent E. coli BSI.

Guidelines for the Laboratory Diagnosis of Monkeypox in Korea

Summary: Since May 2022, there has been a rapid spread of monkeypox in non-endemic countries during the ongoing COVID-19 pandemic. Guidelines proposed by the Korean Society for Laboratory Medicine and the Korea Disease Prevention and Control Agency provide recommendations for diagnosing monkeypox in clinical laboratories in Korea. These guidelines cover various aspects such as test types, specimen selection and collection, diagnostic methods, interpretation of results, and biosafety. Molecular tests are recommended as confirmatory tests, and different specimens are recommended for testing at different stages of the disease.

Summary: IMP-6-producing Pseudomonas aeruginosa sequence type 235 is a dominant clone of carbapenemase-producing P. aeruginosa in Korea. The resistance rate of P. aeruginosa isolates from blood cultures has increased and there has been a shift in the molecular epidemiology of carbapenemase-producing P. aeruginosa.

Comparative Analysis of the Molecular Characteristics of Group B Streptococcus Isolates Collected from Pregnant Korean Women Using Whole-genome Sequencing

Summary: In this study, the distribution of GBS isolates in pregnant Korean women was investigated. CC1 was the dominant lineage, followed by CC19, CC12, and CC17. The low prevalence of CC17 GBS colonization may explain the low frequency of neonatal GBS infections. Whole-genome sequencing is a useful tool for predicting antimicrobial susceptibility and capsular serotypes in GBS isolates.

Under-diagnosis of vector-borne diseases among individuals suspected of having Scrub Typhus in South Korea

Summary: Due to environmental changes, vector-borne diseases are becoming more common. This study investigated the seroprevalence of four major vector-borne pathogens in patients suspected of having Scrub Typhus. It was found that many patients had antibodies to multiple pathogens, highlighting the importance of considering co-infections in clinical practice and the laboratory.

Novel Mutations Conferring Amoxicillin Resistance in Helicobacter pylori in South Korea

Summary: Helicobacter pylori is a major causative agent of various gastrointestinal diseases and its eradication is often hindered by antibiotic resistance. This study aimed to identify clinical strains of H. pylori with amoxicillin resistance and analyze the genetic variations associated with it. Analysis of 368 clinical strains revealed amoxicillin resistance in 8.7% of the strains. Whole-genome sequencing analysis identified specific genetic variations in multiple genes related to amoxicillin resistance. Six specific genetic variations in PBP2 were found to be associated with high amoxicillin resistance. Consideration of amoxicillin resistance is important in the clinical setting for the failure of H. pylori eradication.

Molecular and Clinical Features of Fluconazole Non-susceptible Candida albicans Bloodstream Isolates Recovered in Korean Multicenter Surveillance Studies

Summary: We studied the mechanism and clinical features of acquired fluconazole resistance (FR) in 14 fluconazole non-susceptible (FNS) isolates of Candida albicans recovered from bloodstream infections in Korea. Mutations in drug-target gene ERG11 and transcription factor genes TAC1, MRR1, and UPC2 were compared between FNS and fluconazole-susceptible isolates. We found that amino acid substitutions in ERG11 and TAC1 likely contribute to FR in C. albicans bloodstream infections, and most FNS infections occur without azole exposure.

Virulence Traits and Azole Resistance in Korean Candida auris Isolates

Summary: This study analyzed the virulence traits and azole resistance mechanisms of 104 Candida auris isolates collected from Korean hospitals from 1996 to 2022. The results showed a high fluconazole resistance rate among C. auris isolates in Korean hospitals, with two clades identified and clade II isolates exhibiting higher resistance. The isolates from different clades were sensitive to other drugs, and clade II isolates displayed reduced thermotolerance, competitive growth, and virulence.

Summary: After the introduction of PCV13 in Korea, there have been changes in the serotype distribution and antimicrobial resistance of invasive S. pneumoniae causing IPD. Non-PCV13 serotypes are increasing, and there are differences in antimicrobial resistance according to specific serotypes. Continuous monitoring of serotypes and antimicrobial resistance is crucial for the appropriate management of S. pneumoniae infections.
