A boy with X-linked agammaglobulinemia experienced progressive global motor decline, cerebellar syndrome, and epilepsy. All standard polymerase chain reactions for neurotropic viruses were negative on cerebrospinal fluid and brain biopsy. Next-generation sequencing allowed fast identification of a new astrovirus strain (HAstV-VA1/HMO-C-PA), which led to tailor the patient's treatment, with encouraging clinical monitoring over 1 year.

Summary: Persistent hepatitis is a common late-onset posttreatment complication in transplanted SCID patients. This study found that it is associated with chronic enteric viral infection and immune dysregulation, and identified risk factors such as myeloablation absence and defective B-cell function. Treatment options such as retransplantation or gene therapy can lead to remission of hepatitis.

Summary: Two species of pathogenic Listeria, L. monocytogenes and L. ivanovii, can asymptomatically colonize udders and tonsils of ruminants without fecal shedding. Their whole-genome sequences contain known core virulence and antibiotic resistance genes. This study highlights the importance of udders and tonsils as intra-host sites of Listeria colonization.

Microbiota-induced active translocation of peptidoglycan across the intestinal barrier dictates its within-host dissemination

Summary: Peptidoglycan, the major component of bacterial cell walls, plays a crucial role in affecting physiological and behavioral changes in mammalian hosts. However, the mechanism and regulation of its translocation from the gut to host tissues have not been fully understood. This study investigated the absorption and dissemination of peptidoglycan across the intestine. The results showed that peptidoglycan has a preference for certain host organs, particularly the brain, when absorbed through the gut. The translocation of peptidoglycan is an active process regulated by the microbiota-induced parasympathetic pathway through muscarinic acetylcholine receptors.

Summary: Using metagenomic next-generation sequencing and reverse-transcription polymerase chain reaction, we detected the Aichi virus genome in tissues of patients with primary immune deficiency and unexplained multiorgan inflammatory involvement. We showed evidence supporting the causality of Aichi virus in these cases.

Virus Pop-Expanding Viral Databases by Protein Sequence Simulation

Summary: The improvement of knowledge about unknown viruses in the virosphere is crucial in virology. However, current evaluation methods for metagenomics tools do not consider their ability to detect novel or distant viruses. Simulating realistic evolutionary directions is essential for benchmarking and improving these tools. The Virus Pop pipeline is introduced here as a tool for simulating realistic protein sequences and expanding databases to better detect distant viruses. It has been demonstrated that Virus Pop is capable of generating simulated sequences that closely resemble real protein sequences and can identify novel pathogenic viruses not included in the databases.

Chikungunya virus cell-to-cell transmission is mediated by intercellular extensions in vitro and in vivo

Summary: The Chikungunya virus (CHIKV) forms long intercellular extensions that connect infected and neighboring cells, facilitating efficient transmission of the virus. These extensions are critical for CHIKV transmission, which occurs through endocytosis and endosomal acidification, rather than the CHIKV receptor MXRA8. In vivo experiments demonstrate that neutralizing antibodies are unable to block infection when the virus is transferred through infected cells. This study suggests that the contact sites of the intercellular extensions shield CHIKV from neutralizing antibodies, promoting efficient virus transmission both in vitro and in vivo.

JAK Inhibition in Juvenile Idiopathic Arthritis (JIA): Better Understanding of a Promising Therapy for Refractory Cases

Summary: Juvenile idiopathic arthritis (JIA) is a diverse group of diseases with different underlying physiopathology. Treatment options are limited for some patients due to severe disease, drug adverse events, drug allergy, or association with severe comorbidities. Janus Kinase (JAK) inhibitors show promise in providing an alternative treatment for these cases and may also be beneficial for patients with Mendelian disorders resembling JIA. However, more research is needed to evaluate their long-term safety and identify subsets of JIA patients who will benefit from these treatments.

Summary: The survival rate after hematopoietic stem cell transplantation for inborn errors of immunity has improved considerably, but many patients still experience poor health status and impaired quality of life. Abnormal graft function and chronic graft-versus-host disease are correlated with poor health and very poor health, which in turn directly affect the patients' HRQoL. Further studies are needed to examine the long-term impact of transplant procedure improvements on health status and HRQoL.

Infection in Heart-Lung Transplant Patient, France

Summary: In March 2022, a 61-year-old woman in France who had received a heart-lung transplant was diagnosed with chronic hepatitis characterized by increased liver enzymes. Metagenomic next-generation sequencing revealed the presence of an unknown circovirus, named human circovirus 1 (HCirV-1), in a liver biopsy sample. The viral genome was undetectable in blood samples from 2017-2019, but became detectable in September 2020 and reached high titers in January 2022, concurrent with liver damage indicated by elevated liver enzyme levels and high viral copies in the liver and blood.
