Exercise training (ET) and secondary prevention measures in cardiovascular disease aim to stimulate early physical activity and to facilitate recovery and improve health behaviours. ET has also been proposed for heart failure patients with a ventricular assist device (VAD), to help recovery in the patient's functional capacity. However, the existing evidence in support of ET in these patients remains limited. After a review of current knowledge on the causes of the persistence of limitation in exercise capacity in VAD recipients, and concerning the benefit of ET in VAD patients, the Heart Failure Association of the European Society of Cardiology has developed the present document to provide practical advice on implementing ET. This includes appropriate screening to avoid complications and then starting with early mobilisation, ET prescription is individualised to meet the patient's needs. Finally, gaps in our knowledge are discussed.

Summary: This study aimed to assess acute changes in left atrial (LA) function during incremental aerobic exercise in heart failure patients and compare them with healthy subjects. The results showed that patients with mildly reduced ejection fraction showed a proper increase in LA reservoir function, which helped maintain stroke volume during exercise.

The Improvement of Left Atrial Function after Twelve Weeks of Supervised Concurrent Training in Patients with Heart Failure with Mid-Range Ejection Fraction: A Pilot Study

Summary: Left atrial dysfunction is associated with exercise intolerance and poor prognosis in heart failure (HF). The effects of exercise training on atrial function in patients with HF with mid-range ejection fraction (HFmrEF) are unknown. This study aimed to assess the effects of a supervised concurrent training (SCT) program on left atrial function in HFmrEF patients. Results showed that SCT improved left atrial and left ventricular function in HFmrEF, with significant increases in exercise duration, distance walked, and measures of atrial strain compared to the control group.
