Fractional flow reserve-guided percutaneous coronary intervention vs. medical therapy for patients with stable coronary lesions: meta-analysis of individual patient data

Aims To assess the effect of fractional flow reserve (FFR)-guided percutaneous coronary intervention (PCI) with contemporary drug-eluting stents on the composite of cardiac death or myocardial infarction (MI) vs. medical therapy in patients with stable coronary lesions. Methods and results We performed a systematic review and meta-analysis of individual patient data (IPD) of the three available randomized trials of contemporary FFR-guided PCI vs. medical therapy for patients with stable coronary lesions: FAME 2 (NCT01132495), DANAMI-3-PRIMULTI (NCT01960933), and Compare-Acute (NCT01399736). FAME 2 enrolled patients with stable coronary artery disease (CAD), while the other two focused on non-culprit lesions in stabilized patients after acute coronary syndrome. A total of 2400 subjects were recruited from 54 sites world-wide with 1056 randomly assigned to FFR-guided PCI and 1344 to medical therapy. The pre-specified primary outcome was a composite of cardiac death or MI. We included data from extended follow-ups for FAME 2 (up to 5.5 years follow-up) and DANAMI-3-PRIMULTI (up to 4.7 years follow-up). After a median follow-up of 35 months (interquartile range 12-60 months), a reduction in the composite of cardiac death or MI was observed with FFR-guided PCI as compared with medical therapy (hazard ratio 0.72, 95% confidence interval 0.54-0.96; P=0.02). The difference between groups was driven by MI. Conclusion In this IPD meta-analysis of the three available randomized controlled trials to date, FFR-guided PCI resulted in a reduction of the composite of cardiac death or MI compared with medical therapy, which was driven by a decreased risk of MI.

Summary: Mounting evidence shows that right ventricle (RV) function carries independent prognostic influence in various disease states. This study investigated the incidence and impact of permanent RV infarction in patients with inferior ST-segment elevation myocardial infarction (STEMI) and culprit lesion in the right coronary artery (RCA). The results revealed that permanent RV infarction occurred in 20% of these patients and independently predicted the size of the final left ventricular infarct.

Summary: This study examined the incidence of ventricular tachycardia (VT) and ventricular fibrillation (VF) in patients with heart failure and their relationship with mortality. The results showed that VT/VF events were uncommon but strongly associated with mortality in heart failure patients. These events seem to be markers of disease severity rather than risk of sudden death.

Summary: This study investigated the geographic differences in patient characteristics, treatments, and outcomes in global clinical trials. Patients from Asia, especially South Asia, differed from patients in other regions in terms of age, weight, comorbidities, type of myocardial infarction, and treatment. On the other hand, patients from Latin America did not differ significantly from European or North American patients. These findings highlight the importance of considering regional differences when designing global clinical trials.

Summary: This study analyzed the effects of two behavioral nudging letter strategies on influenza vaccination rates in patients with heart failure (HF). The results showed that the vaccination rate in HF patients was relatively low, but the behavioral nudging letter strategies were effective in increasing the vaccination rate and had no negative impact on the use of guideline-directed medical therapy (GDMT) in the long term.

Summary: This study aimed to assess the relationship between patient-reported knowledge of self-management and clinical outcomes in patients with heart failure and reduced ejection fraction (HFrEF). It was found that poor self-rated knowledge may be associated with higher rates of mortality. Evaluation of knowledge of 'self-efficacy' may provide prognostic information and a guide to which patients may benefit from further education about self-management.

Summary: The prevalence of NAFLD ranges from 25% in the general population to 90% in obese patients scheduled for bariatric surgery. NAFLD can progress to NASH, leading to serious complications. Weight loss and lifestyle changes are the current treatment options, and bariatric surgery has shown short-term improvements. However, the extent and long-term effects of surgery on NAFLD/NASH are still uncertain. The factors involved in the regression of NAFLD/NASH after bariatric surgery have not been fully understood.
